Please contact the Health Division at 508-862-4644 to inquire about the local town regulations and policies. Note: If result of the exam is “Fail,” you must wait three months before reapplying. A payment fee will be processed, if the result of exam is “Passing”. Permits will not be delivered at the time of inspection* I understand that prior to backfilling any system installed; I must call for inspection before 1:00 pm, on the day the system is ready for inspection. A certificate will not be issued until all connections have been made and sealed and all stone has been spread, if applicable. A certificate of compliance will not be issued until after designer certificate is received. System will be inspected within 48 hours after notification, not counting Saturdays, Sundays or Holidays barring emergencies when no inspector is available.* I agree to furnish the Board of Health an "AS-BUILT" diagram on a 4’' by 6" card, - showing the location and size of all parts of the septic system and location of risers BEFORE OR AT THE TIME OF INSPECTION. This includes exact distances from the nearest two corners of the building or other permanent feature to center of the clean out cover of the septic tank, distribution box, and leaching area (cover or inspection port). One copy shall be required by the Inspector before compliance certificate is issued. Repairs or additions to system shall also be diagrammed.* I agree to furnish a signed and stamped 'Installer & Designer Certification Form" as a requirement to receive a Certificate of Compliance.* I am aware that additional inspections due to faulty installation or not being ready after calling for inspection will result in an additional $25.00 re-inspection fee. This applies to repairs and/or new construction.* I understand I am responsible for the installation of all systems where the permit is issued in my name and sub-letting to parties not licensed in the Town of Barnstable could result in the revocation of Disposal System Installer's Permit.* Any soil-absorption system constructed in driveways and/or turnaround areas, shall be vented, regardless of type of driveway (i.e. paved, stone, shell etc.) and shall be constructed of H20 loading.* I understand that once I have obtained the permit and commenced construction or have excavated for the system that I am responsible for its completion.* I have read and understand all of the above requirements.* All conditions must be met to obtain a Septic Installer Permit. I fully understand that my Disposal System Installers Permit is issued contingent upon my observing all of the State regulations contained in 310 CMR 15.000, the State Environmental Code, Title 5; all other laws and regulations of the State and the Town of Barnstable. I realize that non-compliance with the above paragraphs could result in the suspension or revocation of my Disposal System Installer's Permit and possible legal action by the Town of Barnstable.
