Review of Preliminary Subdivision Plan Application Preliminary Subdivision Plan Review The Town of Barnstable DPW Engineering Department has completed the requested review of the Preliminary Subdivision Form B submission and Preliminary Subdivision Plan titled “145 Barnstable Road Hyannis, MA 02601 (Hyannis Fire District)” Prepared by Baxter Nye Engineering & Surveying and dated 9/16/2025. The aforementioned documents were reviewed for compliance with the Town of Barnstable Planning Board Subdivision Regulations, and the following discrepancies between the regulations and submitted documents are listed below. • §801-23(B.)(1)(a) – The Preliminary Subdivision Form B does not include the printed name or signature of the owner. • §801-23(C.)(1) – The subdivision name and number were not provided on the preliminary subdivision plan • §801-23(C.)(3) – The owner information is provided in the general notes not within the title block as required. • §801-23(C.)(4) – The Professional Engineer’s seal and signature require a date, and the Professional Land Surveyor’s seal requires a signature and a date. • §801-23(C.)(6) – The abutter information was not provided for the parcels across Charles Street and Barnstable Road abutting the project TOB DPW Engineering Department Review of Preliminary Subdivision Plan November 6, 2025 Page 2 • §801-23(C.)(7) – Identify the materials of the existing roadway surface for Charles Street and Barnstable Road on the preliminary subdivision plan. • §801-23(C.)(7) – Include roadway label, ownership status, roadway classification, and surface material for Center Street. • §801-23(C.)(10) – Provide the missing bearing and distance dimensions to the right of way, lot 1, and lot 2 property lines. • §801-23(C.)(11) – Specify whether the existing drainage structures shown within the proposed subdivision will be retained or removed/abandoned. If they are to be retained, easements will need to be provided for systems located within multiple lots. • §801-23(C.)(16) – Depict the existing water service to 145 Barnstable Road per available HWS tie card information. • §801-23(C.)(16) – The 2” cement lined steel water main within the Charles Street right-of-way is not depicted and the water main within the Barnstable Road water main is not correctly depicted on the plan; revise the plan set to accurately reflect the water mains.
