WAIVER OR CONTINUANCE ASSENT FOR NOTICE OF INTENT
I hereby request or assent to the following action (check the appropriate item in the
checklist below) to my Notice of Intent (NOI) application (SE3-003-6328 ) in
accordance with the MA Wetlands Protection Act MGL Ch. 131 Sec. 40 and the Town of
Barnstable Code-Ch. 237 (Wetlands):
