WAIVER OR CONTINUANCE ASSENT FOR NOTICE OF INTENT

checklist below) to my Notice of Intent (NOI) application (SE3-6381 ) in
accordance with the MA Wetlands Protection Act MGL Ch. 131 Sec. 40 and the Town of
Barnstable Code -Ch. 237 (Wetlands):
