WAIVER OR CONTINUANCE ASSENT FOR NOTICE OF INTENT
I hereby request or assent to the following action (check the appropriate item in the
checklist below) to my Notice oflntent (NO.I) application (SE3-6404 ) in
accordance with the MA Wetlands Protection Act MGL Ch. 131 Sec. 40 and the Town of
Barnstable Code -Ch. 237 (Wetlands):
1. A waiver for the opening of the hearing on my NOi application within 21 days of
the NOi submittal ---
