Engineering Works 12 West Crossfield Road Forestdale, MA 02644 RE: 206 Annable Point Road, Centerville A = 211-032 Dear Mr. McEntee, You are granted variances on behalf of your client, Lee and Beverly Staniar, to construct an onsite sewage disposal system at 206 Annable Point Road, Centerville. The variances granted are as follows: 310 CMR 15.405(1): To construct a soil absorption system six feet away from the side property line, in lieu of the minimum ten feet separation distance required. 310 CMR 15.405: To construct a soil absorption system four feet above the maximum adjusted groundwater elevation, in lieu of the minimum five feet vertical separation distance required. 310 CMR 15.255: To allow the poly liner to be set along the strip-out boundary: 310 CMR 15.255: To provide a four feet reduction to the required five feet strip-out boundary, for a one foot strip-out around the perimeter of the soil absorption system. These variances are granted with the following conditions: (1) No more than four (4) bedrooms maximum are authorized at this property. Dens, study rooms, offices, finished attics, sleeping lofts, and similar-type rooms are considered "bedrooms" according to the MA Department of Environmental Protection. Q:\WPFILES\McEnteeStania206AnnablePointRoad2018.docx (2) The applicant shall record a properly worded deed restriction, signed by the owner of the property, 'at the Barnstable County Registry of Deeds restricting the property to four (4) bedrooms maximum. A copy of the recorded deed restriction shall be submitted to the Health Agent prior to obtaining a disposal works construction permit. (3) The septic system shall be installed in strict accordance with the engineered plans dated January 30, 2018. (4) The designing engineer shall supervise the construction of the onsite sewage disposal system and shall certify in writing to the Board of Health that the system was installed in substantial compliance with the submitted plans dated January 30, 2018. Physical constraints at the site severely restrict the location of the septic system due to its proximity to the lake and high groundwater. The proposed system appears to be designed to meet the maximum feasible compliance standards contained within the State Environmental Code, Title V. L Stanton, David From: McKenzie, Marybeth Sent: Friday, February 16, 2018 2:32 PM To: McKean, Thomas Cc: Stanton, David „ Subject: BOH meeting variance request for 206 Annabee Point Road Tom and Dave, I will not be at the staff meeting next week, but here are my notes pertaining to 206 Annabel Point Rd,Centerville: It is a summer rental property It is a OK fpr..:a 4 bedroom, It is in the'estuaries The 'retaining wall is only 2' so an engineer will not be required. They are requesting a 1' soil stripout instead of the 5', so they are looking for a 4' reduction. It is a pipe and stone system so only the bottom area is included in the design calcs, no sidewall area.S stripout on the sides may not be as utilized as the bottom area; making the variance Ld*47 fer 7 Pump calcs and curves submitted. Buoyance calcs included too. The waste is pumped to the d box, it is a gravity feed system, (the pitch of the pipe pitches down to the end caps) and when located under the driveway it should have a vent. When I spoke with Peter he said it was under pressure and you can't have a vent. May want to clarify this, I might be incorrect. 4 The big item is that the existing sewer line from the house, shown in brown color, crosses a main water line. He doesn't show it sleeved. When I brought this up with him he said that it is existing and it doesn't have to be sleeved. I told him I would be recommend that the line be sleeved because it the right thing to do at this time. I also think it is a main water line to other houses in the area.As I said he didn't like this idea and didn't feel he has to do it. BOH decision. He shows 4' separation from the lake el of 34.8 FYI He has a note that the outlet on the septic tank should be field verified. This tank may be in ground water at this .time, IDK, but the FG is at el 40 and the PC is at 41. BOH might want to address this Hope I covered everything. I'll see you on the 27 th and have a great week. • *' MAtzA Engineering Works, Inc.
