Therefore,theBarns tab le Conservation. Co-^tisSionn,,reby finds that the following conditions are
necessary.in accordance with the Performance Standards set forth in the regulations. to protect those inter•
ests checked above.The Commis—Inn orders that all work shall be performed
in accordance with said conditions and with the Notice of Intent referenced above. To the extent that the fol•
lowing conditions modify or differ from the plans, specifications or other proposals submitted with the Notice
of Intent.the conditions shall control.
General Condition:
1, Failure to comply with all conditions stated herein. and with all related statutes and other regulatory meas•
ures.shall be deemed cause to revoke or mod;fy this Order,
2. This Order does not grant any property rights or any exclusive privileges: it does not authorize any injury
to private property or invasion of private rights.
3. This Order does not relieve the permittee or any other person of the necessity of complying with all
other applicable federal, state or local statutes. ordinances, by-laws or regulations.
a. The work authorized hereunder shall be completed within three years from the date of this Order unless
either of the following apply:
(a) 'the work is a maintenance dredging project as provided for in the Act: or
(b) the time for completion has been extence� to a specified date more than three years, but less than

the extended time period are set forth in this Order.
5. This Order may be extended by the issuing authority for one or more periods of up to three years each
upon application to the issuing authority at least 30 days prior to the expiration date of the Order.

bris. including but not limited to lumber. bricks, p;aster, wire. lath, paper, carcocarc. pipe, tires, ashes,
refrigerators. motor vehicles or parts of any of the foregoing.
7. No work shall be unce►taken until all acministrative appeal periods from this Orcer have elacsed or, if
such an appeal has been filed, until all proceec:n-s before the Department have peen completed.
8. No work shall be undenaKen until the Final Orcer has oeen recorcea in the Re:sir,/ of Deecs or the Land
Court for the district in wnich the land is locate:. within the chain of title of the ailected property, In the
case of recorced land, the Final Orcer snail also oe notea ,n the Re-:stry's Gramcr Index under the name
of the owner of the land upon which the propcsed work is to be done. In the case of registered land, the
Final Orcer shall also be noted on the Land Cc-in Camficate of Title of the owner of the lano upon which

5. ) All portions of the structure permitted herein shall be CCA-treated
or the equivalent. No creosote treated materials shall be used.
6. ) The Natural Resources Dept. shall be notified in writing at least
15 working days prior, to the start of work: at the site, to inspect the
area for shellfish. If deemed necessary by the Shellfish Constable,
shellfish shall be removed from the work: area to a suitable site and/or
replanted at the locus following construction. The foregoing .measures
for the protection of shellfish shall ensue at the e.--.,:pense of the
applicant. The applicant shall ensure that -the Natural Resources Dept.
receives a copy of the plan of record and any forthcoming plan
revisions.
7. ) Float stops shall be used to prevent the grounding of the float on
the substrate.

maximum ratings shall be dock-ed at the float. At no time shall any

Compliance for the work herein permitted. Where a project has been
completed in accordance with plans stamped by a registered professional.

statement by such a professional person certifying substantial
compliance with the plans and setting forth what deviation, if any,
exists with the record plans approved in the Order shall accompany the
