When the system was constructed,the Contractor failed to notify this office to perform the bottom of excavation observation as well as confirm the stone under the septic tank&"D-box". We had the Contractor re-excavate and expose the components so we could confirm the installation and perform the as-built survey. The Contractor was present when the as-built survey was being performed. We requested the Contractor to correct the elevation of the septic tank outlet tee, in order to provide the 2"minimum drop through the tank. This letter is to certify(declare)that, in my opinion, based on what we observed,the individual sewage disposal system presently constructed on this lot has been installed as noted above, in general conformance with the design intent and the provisions of Title 5 of the State Sanitary Code as well as the Town requirements. Any modifications made during construction are reflected on the As-built Plan. This declaration is limited to the provisions of the State Sanitary Code and local Board of Health requirements only. The backfilling & final restoration of the ground surface or other site features are specifically excluded from this declaration. It should be clearly understood that P.M.P. did not provide our client with construction observation or construction management services. The licensed installer is responsible to properly construct the system(PMP is not part of the contract with the installer). It is our understanding that the Board of Health performs certain observations during the construction,which are independent of the services performed by PMP. P.M. P. Associates, LLC does PMP guarantee the operation of the system. Two copies of the As-Built Plan are enclosed. Should the Board have any questions in regard to this letter,please contact me.
