88 GREENWOOD AVENUE Hyannis A = 289 — 140 r 1 I I II o t i I Y C Crocker, Sharon From: Johnson, Chester (Gus) Sent: Friday, January 10, 2020 4:03 PM To: McKean, Thomas Cc: Groc:ker-,..Sharon Subject: 88 Greenwood'Ave__._ _ Tom, Sharon, As directed I proceeded to 88 Greenwood Ave at app 1045 am today, 1-10-20.There I spoke to the homeowner Linda Whitcomb who said that Tank Removal Services will be here today and she will call us when they arrive. I looked around the rear of the home and along the sides and could not see a fill for a UST, BUT, Linda assured me that they have located a pipe and are going to conduct a whistle test today. I left area. I went back to property at 145pm and there was no one home so I called Tank Removal Services and spoke to Jamie, he said that his worker will on property within the next half hour. I asked Jamie where the fill pipes were located and he assured me that he nor his workers have NEVER been on site. A short time later Jeff Eshbaugh from Tank Removal showed up on property. We searched around home and removed debris against the home and the rear deck to no avail. I decided to search out front of the home near where the gas service was installed and located both the fill and the vent pipe for the UST. A whistle test was conducted and the air suction directly sucked from the fill pipe sucked in from the vent pipe assuring us that the UST was still in place. The gas service line appears to be installed over the tank and the ground wire for the service is actually tied off to the vent pipe from the UST. We secured and left from the area. At no time was the homeowner home during this testing, we were on site for about half an hour, I was there in total just about an hour. Jamie was contacted about draining the tank and was instructed to please call Monday and speak to Tom McKean on how the Town and homeowner will have him proceed. Ongoing investigation at this time 1-10-20 1530hrs Sharon is attempting to attach photos of location and will forward this e-mail with photos back to us. CGJ 1 NOTE TO FILE: 88 Greenwood Avenue, Hyannis FROM: Sharon Crocker DATE: July 24, 2019 RE: UST - Tank location / Removal Tom McKean ok'd using 400 funds to cover cost to Rick Mahoney, Tank Removal Service, 508-778-8265 to try to verify whether or not there is a tank there. He had tried to identify this in past and dug along the gas pipe but once gas pipe turned, he had to stop as he needed to call DigSafe. He said he can also give it the "whistle" test. Tanks are designed with a whistle. The way you know the tank is full is that it will whistle. If the pipe is still attached to a tank, he can put something (a tool) in it and it'll draw the air and will whistle if still connected. He will also call DigSafe first and try to accomplish as much as possible with up to $400 — once he has permission from owner.
