You are directed to correct the above listed violations within seven (7) days of receipt of this notice. You may request a hearing if written petition requesting same is received by the Board of Health within seven (7) days after the date order is received. However, this violation must be corrected regardless of any request for a hearing. Please be advised that failure to comply with an order could result in a fine of not more than $500. Each separate day's failure to comply with an order shall constitute a separate violation. You are also subject to non criminal citations of$40.00 for the first violation and $15.00 for each additional violation. Tickets will be issued daily until the violations are corrected. You are directed to correct the violation of 410.482 and 410.351 within twenty-four (24) hours of receipt of this notice. 'JJ-410.351: Light fixture in the pantry was not secured to ceiling. dO-410.500: Ballast cords for pantry window were broken. P10.480: Rear entrance door to apartment was not secure due to inoperable lock. 410.501: Frame of storm window in rear hallway was missing bottom piece. 410.480(E): Rear hallway window was not provided with a locking device. �410.602M: Attic of house contained a lot of trash ans household debris. A410.501: Window in first floor rear hallway had cracked pane of glass. 6-314 301: Rear entrance to first floor/basement stairwell did not have a door, only a screen door was provided. Tenants complained dPice and snow build-up in this area in the winter time causing dangerous conditions on the basement stairwell. 410.253: Only two light fixtures were provided at far end of basement. One fixture had a pull chain, and the other had no means of turning it on unless the bulb was screwed into the fixture. There was not enough illumination for the tenants to walk in the basement area safely. 14 0.501: Living room/dining room storm window had a cracked pane of glass. )t*410.500: Top of window in living room/dining room would ¬ stay up unless secured by window lock. 30U-L 410.480B): Front entrance door to house was not provided with a locking device. ©V'10.552: Front entrance screen door had torn screen. Mrs. Clark stated that- she did not believe this material was asbestos because she recently had ' contractor (Coastal :Energy Inc.) remove asbestos from the property. Mrs. *Clark promised to .send me a copy of her disposal receipt for this work. I-received her letter and a copy of the disposal sheet on October 21, 1988 (copies attached) . On October 24, 1988 I attempted to contact Mr. Daniel Messier of Coastal Energy Inc. by phone. Mr. Messier did not return my phone calls to his office_ so I was not. able to verify if his company investigated these panels.
