The following recommendations are made to bring this facility into compliance with 105 CMR 410.000, STATE SANITARY CODE II - MINIMUM STANDARDS OF FITNESS FOR HUMAN HABITATION AND THE FEDERAL FOOD CODE: -Have the property treated again by Al, treat for cockroaches in the rooms, fruit flies in the end unit, and for mealy moths\pantry pests in the basement food storage area. Have Al check out the microwave that had cockroaches present in the clock to see if they can clean it out or if it just needs to be replaced. If they can rid the microwave of cockroaches, then sanitize it afterwards. -Clean the hood filters over the stove more often. This will help prevent fire and a food source for insects. The hood filters can be taken down and cleaned in the sinks, or washed in the new dishwasher (if they fit.) Clean the filters about once every two weeks or when build up starts to appear. Have the hood system itself cleaned professionally about every six months. -Install a vacuum breaker on the mop sink hose to prevent back siphonage. -Hang the mops up so they can air dry (provide a protective barrier on the wall so it does not get water damaged where it is hung to dry.) -Check food products when they come in for webs or other signs of insect infestation. Check the food storage areas occasionally to see if insects, or evidence of, are present. Sweep up the food storage area floors frequently, especially below the storage racks. -Have a competent adult pour a small amount of bleach down the drain (sink and shower) in the room that had the flies present-and let it sit for a few minutes to kill off any drain flies. Be sure no children are present while using the bleach. Let water run in the sink and shower for a short time to ensure the bleach is flushed through the piping and won't pose a danger. It is donated and is out of date. The cook has boiled rice with maggots in it and isnt even a real cook but a child care provider. The roach infestation is getting worse. One child woke up with two cockroaches in his mouth. They turned the heat off in March or April. They are having problems with the water and they have to turn off the electricty and the tenants are upset they will lose what they have in the fridge. The pots and pans arent washed. Hair nets arent worn. Caller said the units would fail section 8. Caller said that they have to live in sub humane environments and all they did was have a man lay a hand to them. They are being terminated when they complain. One of the staff is doing drugs and the caller said they reported them but now the caller is being forced out and she has three kids. Caller requests that a representative from the health department speak'to each resident. Caller said that whoever complains about the conditions gets terminated from the program. THE PEOPLE COOK THEIR OWN FOOD WITH ASSISTANCE FROM WORKERS, THAT HELP TEACH THEM HOW TO COOK. THEY DO HAVE A SERV-SAFE EMPLOYEE ON STAFF. THEY.DON'T WEAR HAIRNETS, AS PEOPLE DON'T WEAR THEM IN THEIR KITCHENS AT HOME. THEY DID HAVE A HEAT FAILURE, BUT IT WAS REPAIRED. THEY PROVIDED THE TENANTS WITH ALTERNATIVES WHILE THEY WAITED FOR THE HEATING SYSTEM TO BE REPAIRED. THE DID HAVE A WATER LEAK WHEN SOMEONE OUTSIDE HAD COME IN TO PAINT THE PLACE. THE HAD THE MAINTENANCE GUY CHECK IT OUT AND HE HAD TO TURN OFF A CIRCUIT BREAKER SO IT WOULD BE SAFE. THIS HAS BEEN CORRECTED. THEY ALSO WORK WITH PHIL FROM HAC. THE TENANTS DO THEIR OWN WASHING AND CLEANING OF THE POTS AND PANS. THE HOODS NEED TO BE CLEANED MORE FREQUENTLY IN THE KITCHEN. THEY NEED A VACUUM BREAKER OR TO CUT THE HOSE IN THE MOP SINK. THE NEED TO HANG THE MOP TO AIR DRY. THEY NEED TO CHECK FOOD PRODUCTS FOR MOTHS OR WEBS. THEY ARE PURCHASING ANEW DISHWASHER. THEY RECEIVE FOOD FROM THE BOSTON FOOD BANK(HARWICH) ONCE A MONTH. MOST PRODUCTS ARE WELL IN DATE. SOME OF THE BEST IF USED BY ARE SLIGHTLY OVER (DAY OLD BREAD)WHICH IS OK. PHFS ARE PURCHASED MORE OFTEN, AND NOT OUT OF DATE. A COCKROACH WAS OBSERVED INSIDE THE MICROWAVE CLOCK. THERE WERE SOME FOOD DEBRIS ON THE FLOOR. ONE ROOM HAD A FRUIT FLY INFESTATION. THERE WAS CUT FRUIT LEFT OUT ON THE COUNTER IN LOOSELY WRAPPED PLASTIC.. THEY ARE GOING TO CALL Al TO RETREAT THIS LOCATION.
