Pursuant to 310 CMR 40.0410, Immediate Response Actions were undertaken to evaluate the presence of any Imminent Hazards and to mitigate potential human exposures and environmental impacts associated with petroleum impacted soils discovered during the removal of an 18,000 split compartment underground fuel storage tank (UST) containing gasoline and diesel fuel as part of the facility closure. Under the Immediate Response Action, environmental assessment was conducted to define the extent and magnitude of such impacts resulting on the removal and management of some 200 cubic yards of petroleum-impacted soils. Soil samples collected at the extent of excavation reported all concentrations of extractable petroleum hydrocarbons (EPH) and target polycyclic aromatic hydrocarbons (PAHs) and volatile petroleum hydrocarbons (VPH) with target benzene, toluene,ethylbenzene and xylenes (BTEX) compounds below the applicable and most stringent Method 1 Risk Characterization Standards. Similarly, results of groundwater analysis from existing and newly-installed monitoring wells also report all concentrations of EPH/PAHs and VPH/BTEX compounds below the applicable and most stringent Method 1 Risk Characterization Standards. These analytical results for endpoint soil sampling and quarterly groundwater sampling represent a preponderance of evidence as the scientific rationale and justification to opine a condition of No .Significant Risk in support of an Immediate Response Completion statement with Permanent Solution Statement without conditions under the provisions of 310 CMR 40.0427 and 40.1040 respectively. This work was conducted under LSP oversight in a manner consistent with the MCP 1 EMERGENCY SPILL RESPONSE ® WASTE SITE CLEANUP ! The facts and statements herein are, to the best of our knowledge, a true and accurate representation of the Site activities, remedial response actions and environmental conditions associated with the project and support LSP Opinions acknowledged by the certifications on the enclosed BWSC-104 and BWSC-105 forms. ENVIRONMENTAL CONDITIONS [Refer to Appendix A] The subject property at 326 West Main Street in Hyannis, MA, is located northeast of the intersection of West Main St. and Pontiac St. [Refer to Figure 1]. The coordinates for the subject property are Latitude 41.64894 N, Longitude 70.30833 W. The property contains approximately 0.96 acres of land area and is developed by a 3,526 square foot (SF) building originally built in 1950. The property has been in use as a gasoline filling station, auto repair facility and convenience store, but is currently vacant. The surrounding area is heavily developed with commercial and residential properties. The Site, as that portion of the subject property where petroleum hydrocarbons were released or otherwise came to be located, is a portion of the subject property containing the area of the former tank grave, the area of the former diesel fuel dispensers and an portion of the area of the former gasoline dispensers as a polygon in the eastern portion of the property. The Site contains some 3,000 SF of land area (0.08 acres) as shown on the Site Plan. Topography at the subject property is relatively flat with a gentle slope from southeast to northwest. Environmental assessment activities have reported groundwater at 18-20' below grade across the site. Regional groundwater contours indicate a southerly flow direction towards Vineyard Sound [Refer to Figure 2]. Multiple groundwater-gauging events over the course of the project have confirmed site-specific groundwater flow to the south [Refer to Site Plan]. A small unnamed pond, located approximately 725' south of the Site, is identified as the primary environmental receptor. According to the MassGIS Priority Resource Map, the subject property is located within the Zone II Wellhead Protection Area of one of the Town of Barnstable Public Water Supply (PWS) wells. One of the PWS wells is shown approximately 1,000 feet southwest of the Site. The subject property and surrounding area are also shown as within a medium-yield potentially- productive aquifer and within the EPA-designated Sole Source Aquifer for all of Cape Cod east of the Cape Cod Canal. A small freshwater wetland is shown approximately.1,000' south of the Site and is characterized as an Area of Critical Environmental Concern (ACEC). An area of protected open-space is depicted approximately 450' south of the Site and is likely associated with the PWS wells. Two NHESP-potential vernal pools are shown on the map, one approximately 1,000' north of the,Site and one approximately 1,000' southeast of the Site [Refer to Figure 3]. Based on the site being located within a Current Drinking Water Source Area (CDWSA), by virtue of being within the Zone II of the Barnstable PWS well, the GW-1 groundwater NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 3 OF 21 EVEVIEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 category applies at the Site, pursuant to 310 CMR 40.0932(4)(a). The GW-3 groundwater category also applies at the Site based on a potential source of discharge to surface water, as per 310 CMR 40.0932(2). The GW-2 groundwater category does not apply at the site, pursuant to 310 CMR 40.0932(6), as the average depth to groundwater has been measured as greater than 15' .below grade at the site. Based on frequency/intensity of use and accessibility of impacted soils, the S-1, S-2 and S-3 (GW-1/GW-2/GW-3) soil categories apply to Method 1 Risk. Characterization per 310 CMR 40.0933. To consider future uses of the property without the need for deeded restrictions, the most stringent S-1 (GW-1/GW-3) Method 1 Risk Characterization standards are also considered. BACKGROUNWASSESSMENT UST Removal On March 1, 2019, CommTank personnel were removing an 18,000-gallon, three- chambered, gasoline/diesel underground storage tank (UST) at the subject property, as part of a facility closure. CommTank was hired by a third parry to complete this work on behalf of the property owner. In the process of removing the tanks, the property owner became concerned when an accident occurred at the property, in which a mini-excavator fell into the excavation, resulting in a release of diesel fuel and an OSHA investigation of the accident. The property owner contacted BEA to oversee the ongoing work at the property associated with the UST Closure. 1, KIM .. 2 � 4 Y mil. ,' '' 1• .... Photo 1: Subject UST prior to removal. After recovering. the mini-excavator from the excavation, CommTank personnel performed a Limited Removal Action (LRA) resulting in the removal and recycling of 4.41 tons of impacted soil directly related to the excavator accident. CommTank later completed a "Tank Closure and Limited Removal Action Outcome Report" (5/21/19), included for reference in the previous IRA Status Report. NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 4 OF 21 IMMEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 .During the UST removal, CommTank personnel reported impacted soils were encountered and stockpiled. A PID reading of 710 ppmv was reported as would triggering a 72- Hour Notification Condition, pursuant to 310 CMR 40.0313(2). CommTank then removed and stockpiled approximately 20-cubic yards of impacted soils associated with the elevated headspace readings observed and removed the tank with the benefit of a crane. Although there was no evidence of tank leakage, BEA subsequently confirmed PID readings in the stockpiled material exceeding 100 ppmv and in the southern sidewall area of the UST grave. After consultation with CommTank and the property owner, BEA made Notification to the MA DEP on March 2, 2019. Soil samples from the tank grave and stockpiled soils were independently submitted by BEA for laboratory analysis. Laboratory results from the tank grave indicated concentrations of volatile petroleum hydrocarbons (VPH) and target analytes above the applicable S-3/GW-1 Method 1 Standards, from the soil samples collected from the southern sidewall and bottom of hole areas in the southern portion of the tank grave. Laboratory results from the stockpiled soils reported concentrations of petroleum, metals, polychlorinated biphenyls (PCBs) and volatiles as below the most stringent S-1/GW-1 Method 1 Standards. Gasoline Dispensers and Lines BEA.also oversaw the work performed by CommTank for the removal of the gasoline supply lines to the dispenser islands. Again, BEA collected independent samples and submitted them for laboratory analysis. This work included overseeing the removal of the gasoline supply lines with soil sampling and laboratory analysis. These results similarly identified concentrations of volatile petroleum hydrocarbons (VPH) and target analytes in soils, above the applicable S- 3/GW-1 Method 1 Standards in the vicinity of the former dispenser pump sump. 1 r Photo 2: Supply lines prior to removal. NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 5 OF 21 IMMEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 GeoProbe Borinj4s Subsequent assessment activities performed under the IRA included Geoprobe borings for soil sampling and monitoring well installation, with laboratory analyses of soil and groundwater samples. This work was performed to define the extent and magnitude of significantly-impacted soils and qualify threat of groundwater impact. Laboratory results from the Geoprobe borings were used to delineate two areas of soil impact. The first area was estimated at approximately 15' x 10' x 15' deep corresponding to the southern end of the UST grave. The second area of impacted soils was estimate at approximately 20' x 10' x 5' deep and was associated with the gasoline dispensers in the southern portion of the property. Groundwater testing in April 2019 included sampling five existing monitoring wells and. one new monitoring well installed during the Geoprobe borings. Results of groundwater testing reported all concentrations of EPH/PAHs and VPH/ BTEX analytes as non-detect or below the applicable GW-1 and GW-3 Method 1 Risk Characterization standards. - } sT- +y Photo 3: Geoprobe borings being conducted in area of UST grave. At the time the IRA Plan was submitted, CommTank was scheduled to return to the Site to complete UST Closure Assessment activities, including removing the diesel fuel lines and soil sampling along the lines. Additional soil impact was also observed surrounding the former ` diesel fuel dispensers in the area of the dispenser pump sump. The report further noted that additional soils in the area of the diesel dispensers were also subject to removal, based on the assessment activities proposed. Diesel Dispensers and Lines On April 29, 2019, BEA personnel returned to the site to oversee the removal of the supply lines from the area of the diesel fuel dispensers. Two supply lines (one to each dispenser) and one return line (to the northern dispenser) were observed after the lines were exposed. The lines were removed and soil samples were collected from beneath each dispenser location and every 10' along the length of the lines. Elevated PID readings and a gasoline odor were observed in the sample collected at the end of the supply lines. This sample was also noted to be adjacent to the area of the recently- removed 18,000-gallon gasoline/diesel UST. Soil samples were later submitted for EPH with 4 target PAH analysis. Laboratory results were received on May 8, 2019. The results reported significant EPH impact to soils in the area below the southernmost diesel dispenser. These results indicated the need to remove soils from beneath the diesel dispenser and from the area at the end of the supply lines as corresponding to the southern end of the UST grave. Laboratory results confirmed concentrations of fractional EPH and target PAHs below S-1/GW-1 Method 1 Risk Characterization Standards in soil samples collected from the 4-6' interval in this area (HB-1A: 4-6' and HB-2: 4-6). These findings were reviewed with MA DEP during communications on May 21, 2019, wherein the previous verbal and written approvals for soil removal included all petroleum impacts as from a common source. This understanding was confirmed wherein some 200 yards of significantly impacted soils identified at the property would be removed under the IRA from within the three areas identified during the UST Closure Assessment. IRA PLAN IMPLEMENTATION [Refer to Appendix B] Soil Removal—Gasoline Disp--nsers (Area 2) Dowling Corp., Inc. was chosen by the client as the soil removal contractor after receiving two competitive bids for the proposed work. Dowling Corp. personnel obtained the necessary permits from the local authorities and began work at the site on June 17, 2019. On Monday June 17, 2019, the dispenser canopy in the southern portion of the property was demolished and the material taken off site. Jar headspace screening with a PID and a Dexsil PetroFlag field test kit were used to determine the extent of significant impact during soil removal activities. The final dimensions of the excavation for Area 2 were approximately 17' x I P, to a depth of 5'. Soil samples from the excavation were later submitted to a MA Certified laboratory for VPH/BTEX analysis. The IRA Status Report was filed on July 1, 2019, with the analytical results pending at the time the filing was made. Results of laboratory analysis were reported on June 28, 2019. The results reported concentrations of fractional VPH and target BTEX as Non-Detect, wherein the laboratory reporting limits were below the S-1/GW-1 Method 1 Risk Characterization Standards. During soil removal operations in this area, no impacted soils were observed, despite inspecting the area twice and collecting sidewall and bottom of hole samples for field screening. As such, the operator was instructed to backfill the area and no samples were submitted for analysis from this area. The VPHBTEX concentrations reported in the "Supply Lines 3" sample are considered to be incidental impact associated with the UST closure. Regardless the excavation from Area 1 would also encompass this area. u _ Photo 7:Area of diesel supply lines being investigated. Soil Removal-UST Grave(Area 1) Excavation activities were next directed to the southern end of the tank grave (Area 1 on Site Plan). The operator was instructed to remove the clean overburden soils to a depth of 3' below grade and then stockpile the material for later reuse as backfill. This material consisted mostly of hardener and fill material used to backfill the UST grave after the tank was removed. The impacted soils were placed on plastic sheeting for later off-site transport and recycling. Soil removal in this area focused on the southern sidewall and bottom of hole area and progressed from south to north. Soil samples were collected from the extent of excavation and were screened in the field with a PID, by jar-headspace method, until field screening results reported significant impact had been removed. The final dimensions of the excavation for Area 1 were approximately 30' x 25' in an oval shape, to a depth of 12-15'. The excavation was larger than anticipated due running sands encountered. Soil samples from the excavation were later submitted to a MA Certified laboratory for EPHAPAHS and VPHBTEX analysis. A sample from the clean overburden stockpile was also submitted for EPH/4PAHs and VPH/BTEX analysis. Dowling Corp. personnel returned to the site on June 19, 2019, to complete backfilling the excavations with clean fill. Results of laboratory analysis were reported on June 28, 2019. The results reported all concentrations of fractional VPH and target BTEX analytes as Non-Detect, wherein the laboratory reporting limits were below the S-1/GW-1 Method 1 Risk Characterization Standards. Trace concentrations of fractional EPH were reported in a single sidewall sample (SW-NW: 3 15') as well as within the clean overburden stockpile sample. The concentrations reported were well below the most stringent S-1/GW-1 ,Method 1 Risk Characterization Standards. A copy of the completed Bill of Lading is included for reference in Appendix C. Groundwater Monitoring On July 17, 2019, BEA personnel returned to the Site to collect samples from selected groundwater monitoring wells. Monitoring wells MW-2, MW-3, MW-5, and MW-6 were chosen based on their location downgradient of the areas of soil removal. The monitoring wells were sampled for EPH/PAHs and VPHBBTEX laboratory analyses, using low-flow sampling techniques. Results of the July groundwater monitoring event were received July 25, 2019. The results reported trace concentrations of MtBE in MW-2 and low concentrations of VPH fractional C5-C8 Aliphatics and target BTEX analytes in MW-6. Concentrations of all remaining analytes in all other wells were reported below the GW-1 and GW-3 Method 1 Risk Characterization Standards. Laboratory detection limits for Benzene and for C9-C 10 Aromatics were above the GW-1 Standards.. As such, BEA returned to the Site on August 21, 2019, to resample MW-6. Results of the I August resampling event reported all concentrations of EPH/PAHs and VPHBTEX as non-detect. The results are summarized below in table 4. For the purposes of risk characterization, the strictest S-1 (GW-1) Method 1 Risk Characterization standards are also considered based on the future use of the property. i i NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 15 OF 21 IMMEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 Soil Laboratory Analytical results from soil samples collected at the extent of excavation, from the UST grave (Area 1) reported all concentrations of fractional VPH/BTEX and EPH/PAHs as Non-Detect with the exception of trace concentrations of fractional C19-C36 Aliphatics, below S-l/GW-1 Standards, in a single sidewall sample. Similarly, analytical results from soil samples collected at the extent of excavation from the area of the diesel fuel dispensers reported all concentrations of EPH/PAHs as Non-Detect, with concentrations of VPH/BTEX also reported as Non-Detect in the sample collected from the bottom of hole in this area. In addition, laboratory results, from soil samples collected at the extent of excavation from the gasoline dispensers in the southern portion of the property (Area 2), also report all concentrations of VPH/BTEX as Non-Detect. Trace concentrations of fractional EPH were also reported in the clean overburden stockpile sample used as backfill at the site, however these concentrations were all well below the applicable (S-1, S-2, S-3/GW-1 and GW-3) Method 1 Risk Characterization Standards. As such, this laboratory data supports a finding of No Significant Risk, relative to potential exposure risks to humans, impacts to environmental receptors and potential leaching to groundwater for all current and future foreseeable use of the soils at the Site in support of a Permanent Solution. Groundwater The site is mapped within the Zone II for a Town of Barnstable PWS well and is therefore within a current drinking water source area (CDWSA). Based on groundwater gauging conducted over the course of the project, the average depth to groundwater at the site is greater than 15' below grade. Based on these criteria,the GW-1 and GW-3 groundwater categories apply at the site, considering Method 1 -Risk Characterization, as consistent with the provisions of 310 CMR 40.0932. Groundwater has been sampled on a quarterly basis three times over the course of the project, in April, July and October 2019. In addition, monitoring well MW-6 was resampled in August 2019, after laboratory detection limits were unable to meet GW-1 Standards for Benzene and C9-C10 Aromatics. Each time the wells were sampled, all concentrations of VPH/BTEX and EPH/BTEX were reported below the GW-1 and GW-3 Method 1 Risk Characterization Standards. In April 2019 the wells were also sampled for VOCs and Total Lead, which were all reported as Non-Detect in all wells. As such, based on the results of groundwater analyses, No Significant Risk is apparent related to groundwater at the Site, in support of a Permanent Solution. Soil Vapor Subsequent to contaminated soil removal operations, all end-point areas report discrete VPH/BTEX concentrations as Non-Detect (ND) with Reporting Limit (RL) concentrations less than the strictest.S-1/GW-1, Method 1 Risk Characterization standards. Trace concentrations of fractional EPH in a single sidewall sample and from the clean overburden stockpile sample do not represent a potential vapor intrusion pathway, based on the low concentrations of the analytes NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 16 OF 21 IMMEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 reported. As such, vapor intrusion is not a concern at the site where in there is no complete vapor intrusion pathway, as supported by physical testing and other "lines of evidence" consistent with the current Vapor Intrusion Guidance policy, WSC-16-435. GARDENING BEST MANAGEMENT PRACTICES [Refer to Appendix E] Laboratory results report concentrations of EPH/VPH and all target analytes below the most stringent S-1 (GW-1/GW-3) criteria. However, exposure risks in consuming vegetables grown in impacted soil are not considered in the Method 1 soil standards. Pursuant to 310 CMR 40.1056(2) 0) (1), Gardening Best Management Practices (BMPs) are not required because laboratory analyses support a Permanent Solution Statement without conditions. Gardening BMPs are nonetheless recommended to further reduce potential exposure to materials in the soil, should the property owner wish to grow vegetables for consumption within the area of the defined.Site. Implementing BMPs such as those -referenced in Appendix E will facilitate safer gardening in a wider range of site conditions. Not every BMP is necessary for every Site. A combination of BMPs may help reduce the potential for additional exposure risks not identified by this investigation specific to the petroleum hydrocarbons in the Defined Site [Refer to Site Plan, Appendix A]. DATA USABILITY ASSESSMENT & REPRESENTATIVENESS EVALUATION The following presents a discussion of the.Site information used to support the Permanent Solution.Statement as required by.310 CMR 40.1056(2) (k) and incorporates the guidance provided in MA DEP Policy WSC-07-350 and the Quality Control and Quality Assurance policies of BEA [Refer to Appendix F]. It provides an evaluation and demonstration of the. adequacy of the spatial and temporal data sets used to support the PSS, and makes conclusions on the accuracy,precision and sensitivity of the data used. Conceptual Site Model Petroleum impacted soils were discovered during the removal of an out-of-use 18,000- gallon gasoline/diesel UST. PID readings over 100 ppmv triggered a 72-hour notification condition, prompting verbal notification to the Mass DEP and the ensuing Immediate Response Action. Significant impact to soils was reported in the soil samples collected from the southern sidewall and the bottom of hole sample. Based on the concentrations of VPH reported, a minor release of gasoline from the UST piping was suspected. Based on the assessment activities conducted, the petroleum impacted soils extended to the groundwater interface at approximately 18' below grade. Minor groundwater impact was also reported in a downgradient well, below the applicable GW-1 and GW-3 Method 1 Standards. During subsequent UST Closure activities, impacted soils were also discovered surrounding the gasoline dispensers south of the UST and the diesel dispensers east of the UST.. In both of these locations, shallow soil impact was observed to approximately 5-6 below grade, apparently from minor leaks from the dispenser piping. In discussions with the DEP Case Officer, the Department would allow these soils to be removed and managed under the subject NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 17 OF 21 IMMEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 RTN as they were related to the same UST system. Approximately 200 cubic yards of petroleum impacted soils were removed from the three areas identified and was transported off-site to a licensed disposal facility. Soil samples collected from the extent of excavations were analyzed for EPH with PAHs, VPH and BTEX, or both, depending on the contaminants of concern. Results of the end-point soil sampling reported all concentrations of EPH/VPH and target analyte concentrations as Non-Detect or at concentrations below the most restrictive S-1/GW-1, Method 1 Risk Characterization standards. Five existing monitoring wells and one monitoring well installed over the course of the project have been sampled quarterly, three times (April, July, October) over the course of the project. Low concentrations of VPH and related compounds have been reported in downgradient wells MW-2 and MW-6 over the course of the project, however all concentrations were reported below the applicable GW-1 Method 1 Risk Characterization standards. Field Screening PID screening and field.observations were used to qualify significant impact in soils and to make informed decisions in the field, in advance of submitting samples for laboratory analyses. In some cases,Dexsil PetroFlag testing was conducted to qualify TPH concentrations in the field. Samples with higher levels of odor or PID readings were assumed to be more contaminated than those samples with lower levels. In all cases, visual observations, field screening results and laboratory analytical data correlated well. Sampling Locations Petroleum-contaminated soil removal was supported in the field by PID/Dexsil PetroFlag screening as well as visual/olfactory observations, as work progressed from contaminated soil to clean soil. PID/Dexsil PetroFlag screening was used to qualify the horizontal and vertical extent of contaminated soil, in support of soil removal. EPH and/or VPH laboratory analysis were used to evaluate the impacted soils based on the nature of the contaminants (gasoline and/or diesel fuel). Soil samples were collected from representative sidewall and bottom-of-hole areas for laboratory analyses from the extent of soil removal as critical samples. All endpoint soil samples were preserved for risk-based EPH/PAH and/or VPH/BTEX analyses. The samples were placed in appropriately preserved laboratory containers in the field and stored on ice in a cooler,pending shipment to the laboratory under a properly executed chain-of-custody. A single groundwater-monitoring well was installed to supplement existing monitoring wells already on-site. The monitoring well installed by BEA was performed outside the release area to qualify groundwater quality immediately .downgradient of the release area and intermediate to identified receptors. The monitoring well screen was installed across the groundwater interface, as representative of petroleum impact to groundwater. Field duplicates or trip blanks were not collected nor considered necessary because the Disposal Site was small, the removal operations had produced clean conditions along the majority of exposure points, QA/QC protocols were employed and documented with Method Blanks and Spikes performed by the laboratory. Temporal Data Soil samples were collected subsequent to contaminated soil removal, which was completed in a single day. Due to soil removal being the remedial strategy, temporal soil data was not applicable to consider final exposure risks, wherein laboratory analysis of endpoint samples are relied on for risk characterization. Groundwater monitoring was performed quarterly, three times over the course of the project. The samples were collected during times that coincided with the Spring (April), Summer (July) and Fall (October) seasons, to qualify groundwater variability over the change of seasons. Three quarterly rounds was deemed sufficient based on the magnitude of impact reported and laboratory analytical results reporting all concentrations of EPH/VPH and target analytes, in soil samples,as Non-Detect, or below S-1/GW-I Standards. Field Completeness No data gaps were identified within the analytical data set. Critical soil data from sidewall and bottom-of-hole areas of each excavation represent the extent of significant soil impact and the limits of contaminated soil removal. In addition, data from groundwater testing establishes no significant impact to groundwater from the subject release. The sum of the critical data supports the PSS, wherein QA/QC thresholds and Presumptive Certainty requirements were met under the Data Usability and Representativeness policy. Data Inconsistency No inconsistent data was identified. Visual/olfactory observations and field.screening. results were well correlated. No inconsistency was identified between the field screening of soil samples and their respective laboratory results. Field screening and laboratory analytical results reported contaminants that were consistent with a release of gasoline and/or diesel fuel. Data Not Used The complete analytical data set associated with confirmatory end-point soil samples was considered. Soil samples from the extent of excavation were collected, representing discrete exposures in applicable soil categories as a measure of frequency and intensity of use. Groundwater samples were collected from monitoring wells downgradient of the release areas and intermediate to human and environmental receptors. The methods utilized (PID soil screening, Dexsil PetroFlag testing, EPHNPH analysis) all correspond to the contaminants of concern and are appropriate for a release of gasoline and diesel fuel Based on the Representativeness Evaluation under MA DEP Policy WSC-07-350 (REDUA), the analytical data quality of all critical samples relied on in support of the PSS were reviewed under REDUA. The analytical data provided in support of this PSS has met the method quality control requirements and performance standards for "Presumptive Certainty" as described in CAM VII A, Section 2.0 (a), (b), (c), and (d) or have been evaluated as comparable to CAM requirements. The validity and defensibility of the analytical data used to support the findings of the PSS for this Site with respect to accuracy, precision and completeness pursuant to 310 CMR 40.1056(2)(k) have therefore been satisfied [Refer to QA/QC—Appendix G]. The CAM narrative from laboratory results for groundwater samples collected on April 11, 2019 (Lab #L1915133) reported affirmative responses for Questions A through G, with negative responses for Questions H and I. The negative response for H refers to the samples submitted for VOC analysis, wherein the initial calibration did not meet the minimum response factor on the lowest calibration standard for 1,4-dioxane, as well as the average response factor for 1,4-dioxane. The initial calibration verification was outside acceptance criteria for two compounds but within overall method criteria. In addition, the continuing calibration standard was outside the acceptance criteria for several compounds; however it was within overall method allowances. Although these non-conformances may introduce non-directional bias, the analyses were of non-target analytes to exclude typical non-gasoline related impact. In addition, all of the VOC analytes were reported as Non-Detect. The negative response for Question I was related to analyzing a subset of MCP analytes, which refers to the Total Metals analysis for Lead. This analysis was used to screen for Lead, as a historical constituent of gasoline. As.such, the accuracy and precision of this data meets Presumptive Certainty under REDUA. The CAM narrative from laboratory results for groundwater samples collected on April 16, 2019 (Lab #L1915759) reported affirmative responses for Questions A through G, with negative responses for Questions H and I. The negative response for H refers to the samples submitted for VOC analysis, wherein the initial calibration did not meet the minimum response factor on the lowest calibration standard for 1,4-dioxane, as well as the average response factor for 1,4-dioxane. The initial calibration verification was outside acceptance criteria for two compounds but within.overall method criteria. In addition, the continuing calibration standard was outside the acceptance criteria for several compounds; however it was within overall method allowances. These non-conformances may introduce low bias as well as non-directional bias; the analyses were of non-target analytes to exclude typical non-gasoline related impact. In addition, all of the VOC analytes were reported as Non-Detect. The negative response for Question I was NOVEMBER 8,2019 VWM ASSOCIATES,LLC-326 W.MAIN ST.-HYANNIS,MA/BEA19-11193 PAGE 20 OF 21 IMMEDIATE RESPONSE ACTION COMPLETION-PSS,RTN 4-27691 related to analyzing a subset of MCP analytes, which refers to the Total Metals analysis for Lead. This analysis was used to screen for Lead, as a historical constituent of gasoline. As such, the accuracy and precision of this data meets Presumptive Certainty under REDUA. In the lab analysis for endpoint-soil samples collected on June 18, 2019 (Lab #L1926608), CAM narrative reported an affirmative response for Questions A through F, with negative responses for Questions G, H and I. The negative responses for questions G and H refer to VOC analysis for the contaminated stockpile sample. The contaminated stockpile sample is not considered critical data, wherein the results were used only for disposal characterization.The negative response for Question I was related to EPH analysis, wherein the limited PAR analytes as considered appropriate to evaluate a release of diesel fuel and for Total Metals analysis which was again related to the contaminated stockpile sample. For these reasons, the data meets Presumptive Certainty under REDUA considering Question I. The CAM narrative from laboratory results for groundwater samples collected on July 17, 2019 (Lab #L1931811) reported affirmative responses for Questions A through F as well as Question I, with a negative response for Questions G and H. The negative response for H refers to one or, more target analytes that did not achieve the. requested CAM reporting limits, specifically benzene and C9-C10 Aromatics. The lab .was contacted regarding the.reporting limits and indicated that they could not attain lower reporting limits. As such, BEA chose to resample that particular well for reanalysis. The CAM narrative from laboratory results for the groundwater re-sampling collected on August 21, 2019 (Lab 4L1937847) reported affirmative responses for Questions A through I, with no negative responses. As such, the accuracy and precision of this data meets Presumptive Certainty under REDUA. The CAM narrative from laboratory results for groundwater samples collected on October 2, 2019 (Lab #L1946465) reported affirmative responses for Questions A through I, with no negative responses. As such, the accuracy and precision of this data meets Presumptive Certainty under REDUA. In the review of critical data, the noted QA/QC issues associated with these analyses did not significantly affect risk analysis, and met Presumptive Certainty requirements under REDUA, relative to the interpretation of Significant Risk as defined under the MCP. CONCLUSIONS Remedial response actions conducted under the Immediate Response Action have resulted in the mechanical removal and off-site recycling of 297.68 tons (200 cubic yards +/-) of petroleum-contaminated soils from three separate areas including; the southern end of the UST grave, the gasoline dispensers south of the UST, and the diesel dispensers east of the UST. Soil samples collected at the extent of excavation have reported all concentrations of EPH/PAHs and VPHBTEX -as Non-Detect or below the most stringent S-1/GW-1 Method l Risk Characterization Standards, at each location. The sum. of this work documents remedial performance and is used to evaluate exposure risk to identified human and environmental receptors. The laboratory data reported represents a preponderance of evidence to support a finding of No Significant Risk under Method l Risk Characterization. A Permanent Solution Statement (PSS) without Conditions has been achieved to support the IRA Completion and project closure,per 310 CMR 40.1056. Environmental investigations are inherently limited because conclusions are drawn, and recommendations developed based on information from limited research and Site investigation to target a known release of fuel oil. She is reporting a fuel spill at the gas station. Fire Alarm dispatched Engine 823 on a still alarm. I responded with Firefighters D. Clough and R. Storie. On arrival at the station I met with employees. Bobbi Vatova, Tony Kostav, Rob Orma were all present. There was a female in a wheel chair holding a garden hose. She was identified as a neighbor. I inquired what happen and they stated they had a fuel spill approximately one gallon. They used a garden hose to wash down the spill. They called the owner and I spoke to him about spill procedures at the station. He was asked to report to the gas station. I spoke with the customer on the phone. She states the handle did not shut off while her car was being filled. She was at pump # 3. She alerted the staff by sounding her horn. She believes the spill to be 3 gallons. She only has a 10 gallon tank and the pump show 12 gallons. Her car was not completely empty. She was concerned for her safety with the amount of gasoline on the ground when they asked her to start and move the car. The employees used a hose to water down the spill and wash down her car. She then left the station Fire Prevention was notified and Deputy Melanson arrived with FPO Chase. FPO Chase tools photo. Barnstable Board of Health agent David Stanton arrived on location. The owner arrived at the station and met with Deputy Melanson. Julie Hutchinson for DEP was notified (508-946-2852). Bob Murphy of DEP will follow up on Monday (508-771-6088) We applied pink water to the spill area. The owner was notified to place pump # 3 out of service and forward a'copy of the repair order to the Hyannis Fire Department. The owner notified Northeast Petroleum Service to repair the pump.
