Dear Mr. McCloskey:
Please find enclosed a copy of our local sign code, a'copy of warning notice #3080 and
photographs showing the display of the prohibited signs at the Hyannis location of Acme
Glass. I am writing to make you aware that each violation may result in a$100.00 ~
citation per violation. For example, the two subject signs as displayed in the photograph
qualifies for more than one offense under Chapter 240 Section 61 of the Zoning Code.
That provision identifies prohibited signs including banners & movement and portable
signs. You may also be cited for failure to obtain a sign permit and display of signage
exceeding the proper allotment. As indicated above, this may result in.four separate
violations per sign totaling$800.00 a day.
Please take immediate action to remove the illegal signs in order to avoid further action.

Dear Tom:
I represent John Ellis the owner and operator of John's Auto Body, doing business in
(Hyannis) Barnstable. Mr. Ellis has entered into a purchase and sale agreement for the property
located at 123 Barnstable Road, Hyannis, MA 02601.
The most recent use of the property is as a licensed,vehicle repair shop, with a primary
focus on auto glass repair. Mr. Ellis' intended primary use of the property is to evaluate vehicles-+ 40C
and assess the damage for the insurance carriers._ Though he may use the property for auto body. `D2we
repair and paint, if any, would be water based.
It is my opinion that the proposed use,though still nonconforming, would not be more
detrimental to the neighborhood and bring the property into greater conformity with the intent of
the bylaw than which presently exists under its present use.
As such, I asked you please approve the proposed use as described above in order that my

Dear Mr. McCloskey:
Please find enclosed a copy of our local sign code, a copy of warning notice #3080 and
photographs showing the display of the prohibited signs at the Hyannis location of Acme
Glass. I am writing to make you aware that each violation may result in a$100.00
citation per violation. For example,the two subject signs as displayed in the photograph
qualifies for more than one offense under Chapter 240 Section 61 of the Zoning Code.
That provision identifies prohibited signs including banners & movement and portable
signs. You may also be cited for failure to obtain a sign permit and display of signage
exceeding the proper allotment. As indicated above, this may result in four separate
violations per sign totaling$800.00,a day.
Please take immediate action to remove the illegal signs in order to.avoid further action.

Information and Instructions
Massachusetts General Laws chapter 152 section 25 requires all employers to provide workers' compensation for their.
employees. As quoted from the i'law", an employee is.defined as every person in the service of another us er any contract
of hire, express or implied, oral or written.
An employer is defined as an individual,partnership, association, corporation or other legal entity, or any two or more of
the foregoing engaged in ajoint enterprise, and including the legal representatives of a deceased employer, or the receiver or
trustee of an individual,partnership, association or other legal entity, employing employees. However the owner of a
dwelling house having not more than three apartments and who resides therein, or the.occupant.of the dwelling house of
another who employs.persons to do,maintenance, construction or repair work on such dwelling house or on the grounds or
building appurtenant urtenant thereto shall not because of such employment.be deemed to bean employer.
MGL chapter 152 section 25 also states that every state or local licensing agency shall withhold the issuance or renewal
of a license or permit to operate a business or to construct buildings in the.commonwealth for any applicant who has
not produced acceptable evidence of compliance with the insurance coverage required. Additionally, neither the
commonwealth nor.any.of its political subdivisions shall enter into any contract for the performance of public work until
acceptable evidence of compliance with the insurance requirements of this chapter have been presented to the contracting .
authority.
