The following was noted:
1.1 Reviewed slab-on-grade at level 1 (Dwg. S1.0):
1.1.1 Slab-on-grade was observed to be in place.
1.1.2 Column base(E4)was observed to be plug welded in accordance with previous instructions
by A&M. E. Lavigne noted that Briggs Testing had observed the welding and would be
sending a report to confirm this.
1.2 Reviewed structural steel framing at level 2(Dwg. S2.1):
1.2.1 Location of deck openings was observed to be consistent with the drawings.
1.2.2 Elevator pit, rails and opening were observed to be consistent with the drawings.
1.2.3 Steel framing sizes and configuration are consistent with the referenced design drawing.
1.2.4 Steel joist seats were observed to be welded to the steel supports in representative locations.
1.2.5 Brace connections were observed to be welded.
1.2.6 Field moment connections were observed to be welded.
1.2.7 Joist bridging was observed in place consistent with the requirements of the drawings.
1.2.8 The steel form decking was attached to the structural steel supports. Deck welds were
observed in attachment configurations consistent with the drawings.
1.2.9 Wire mesh reinforcement was 90%complete. Continuous#4 bar,with#3 hooked end bars
@12"O.C.around perimeter was observed to be in accordance with the design drawings. E.

Lavinge noted that the#3 hooked bars were angled to the side to prevent them from
projecting above the pour stop, and that he planned on pouring the elevated slab on Friday
October 26,<200.7.
1.3 Reviewed structural steel framing at roof level (Uwg. S2.2):
1.3.1 Steel framing sizes and configuration are consistent with,the referenced design drawing,
beams were observed to be punched and ready to,receive the wood plate required for the
truss roof to be:installed.
1.3.2 The brace connections at the roof were observed to be welded.
1.3.3 Field moment connections were observed to be welded.
The above items were reviewed with the project superintendent prior to my departure from:the:site.
