To Whom It May Concern,
At the request of Mr. Stuart Bornstein, I met with him and his licensed electrician,Noel Santos of Santos Electric on
January 13, 2018 at the subject property. The purpose for our meeting was to review and discuss methods of
securing bundles of wire to the building structure above the corridors that lead to the conference and meeting rooms
in the basement of the building,prior to the installation of the suspended ceiling grid. —
At the time of my arrival,all wires above the east/west corridor had been bundled and secured with plastic strap ties
to an existing conduit rack system that is suspended from the precast concrete floor planks at the first floor. Please
see photographs#1 and 42,attached hereto.
Wires above the north/south basement corridor will be bundled in similar fashion, following the removal of
mechanical ductwork within the ceiling plenum. These wires,however, will be strap-tied to a new Unistrut trapeze
system that will also be attached to the underside of the first floor, precast.concrete planks as agreed with Mr.
Santos. Please refer to photographs#3 and#4,attached.
In my professional opinion, this method of suspension for 1.00 percent of the wires precludes any reliance for their
support upon the suspended ceiling grid and eliminates any concern for public safety in this regard.

Proposal: Installation of a pool and concrete patio informal review of impervious surface area.
Dear Mr. Bornstein:
Please be advised that the above proposal received an administrative approval at the site plan
review staff meeting held March 14, 2017 subject to the following:
• Approval is based upon plan entitled""Proposed Pool Site Plan- 1127 Route 132, Hyannis,
MA'dated February 16, 2017,prepared for Stuart Bornstein; and PE stamped
Memorandum dated March 9,2017 to Paul Roma, Building Commissioner, outlining an
impervious surface rationalefor the courtyard area resulting in a decrease of approximately
300 s.f. of impervious area,both prepared by Daniel A. Ojala,PE,PLS Down Cape
Engineering, Inc. .
• Applicant must obtain all other applicable permits, licenses.and approvals required
including but not limited to, amendment of existing alcohol license to reflect the addition
of the outdoor pool area. Contact Maggie Flynn, Consumer Affairs 508-862-4674 for
assistance.
Upon completion of all work, a registered engineer or land surveyor shall submit a letter of
certification,made upon knowledge and belief in accordance with professional standards that all
work has been done in substantial compliance with the approved site plan(Zoning Section 240-
105 (G). This document shall be submitted prior to the issuance of the final certificate of
occupancy.
A copy of the approved site plan will be retained on file.
