Carpenters working on pre-engineered roof truss tail ends at the south gable along the east side of the structure. Foundation contractors installing final lengths of reinforcing in bottom of footing formwork at front lobby expansion. OBSERVATIONS: All existing roof truss tails at the north gable and four (4) of the tails at the south gable had been compromised, having been cut back two (2)to three(3) additional inches in from the outside face of the exterior wall sheathing, leaving only one and one-half(1 '/2) inches of bearing on the exterior wall plate at those truss ends. ISSUES DISCUSSED W/ SUPERINTENDENT: Mr. Paulino of this office informed Mr. Roberts and the carpenter foreman that the truss tails ends were cut back too far and that they should stop work immediately, until further notice. INFORMATION OR ACTION REQUIRED: Again, following a phone conversation with Mr. r Dempsey of this office, Mr. Paulino informed Mr. Roberts and the carpenter foreman to add solid vertical blocking at all overcut tails to bring them back flush with the exterior face of sheathing. In addition they should install a second layer of%" plywood sheathing, glued and screwed to each side of every cut truss from the outside face of the wall sheathing back to the vertical stud at a distance of four (4) feet. Finally add a horizontal 2x4 from outside face of wall sheathing to 2x4 wall stud at four (4) feet each side of truss along the bottom chord and bearing on the exterior wall. I, Richard J. OBSERVATIONS: Existing roof truss tails closer to the front of the building are comprised of 2x4 and 2x4 bottom chords while the truss tails closer to the rear of the building are comprised of 2x6 top chords with 2x4-bottom chords. In addition, the 2x6 trusses have ripped, triangular, shaped solid blocking at the tail ends while the 2x4 trusses do not. ISSUES DISCUSSED W/ SUPERINTENDENT: Mr. Roberts informed Mr. Paulino (of this office) that the tail ends of the trusses at both of these locations and at one (1) location along the west side of the structure needed to be cut flush with the exterior wall sheathing. INFORMATION OR ACTION REQUIRED: Following a phone conversation with Mr. Dempsey of this office, Mr. Paulino informed Mr. Roberts and the carpenter foreman to add solid blocking between the top and bottom chords of those trusses without triangular shaped blocks at the tail ends. In addition they should install a new 2x4 vertical member between the top and bottom chords, four (4) feet back from the outside face of the exterior wall sheathing at all trusses prior to gluing and screwing 1/4"thick plywood sheathing to each side of every truss from outside face of wall sheathing to new vertical stud. The final approval letter from Hilton was e-mailed and included items revised from the original submittal dated 17 November 2011. 2. Wall tile edge trim profile at showers'and tub surrounds was selected and approved.The profile is Schluter,Jolly Aluminum clad tile edge trim with polished chrome finish. Trim profile to match tile thickness. , 3. The Electrical Contractor's Engineer must make sure that all areas meet the minimum lighting levels and control types as noted in the DoubleTree Standards Manual. 4. The Guest Bathroom exhaust fan was moved closer to the ceiling located above the vanity light.BMA recommended relocating the exhaust grille to the ceiling.The grille needs to align with the center of the vanity bowl. 5. The Bathroom accessories(toilet paper holder with two wall supports,two single robe hooks,and 24" towel bar)were approved by the Owner and Hilton and ready for the Owner to purchase. 6. Because of the long lead time for the floor and desk lamp BMA sent the specifications to other vendors to find matching lights. BMA and the Owner inspected the tile layouts in several Guestrooms and determined that the tile layout in Guestroom 424 is the layout to use for all future Bathrooms: The floor tiles will have a '/4" maximum grout thickness in order to minimize tile cutting. Since the grout and tile colors are similar the grout lines will blend for a uniform appearance. Tile layouts for other bathrooms to be determined. 8. The wall tiles at the tubs and showers must have the minimum tile grout thickness that will work with the specified tile and will provide sufficient adherence without spalling. Unsanded epoxy grout is required to achieve this. The wall tile shall run from floor to ceiling. The tile at the edge of the tub leaves a triangular gap where there could potentially leave a large grout spot. Option to resolve this is to cut the tile to match t�a radius of the tub edge. The radius is small and may not be an issue.BMA recommends the Contractor lay out a bathroom for review and approval. 9. BMA e-mailed two exterior renderings to the Owner for their marketing purposes. 10. BMA provided floor plans to the Owner's Sprinkler Contractor for design of a new sprinkler system. 11. The sliding door panels at the Bar are still in the scope. The carpet and tile floor finishes will terminate at the door track line. 12. BMA presented the Lobby furniture and finishes.All items were approved by the Owner. BMA will proceed with finalizing the Public Area submittal to Hilton for approval. 13. Because of the Owner's concern with a low back chair,BMA selected a new Guestroom lounge chair with a higher back.The chair is identical to the chair approved by Hilton with the exception that the height of the back increased from 37"to 42"high. BMA will submit this to Hilton for review and approval. 14. BMA contacted the Furniture Rep. for the Guestroom furniture regarding the Hilton requested changes, to the Guestroom furniture.The changes included the addition of floor length panels below the nightstands and extend the headboard behind the mattress and terminate at the top of the box spring. East Wing: 15. EIFS at the gable walls were applied without tenting and heating per the manufacturer's written instructions. Staining and discoloration'occurred. BMA recommends either applying the product as specified or wait until warmer weather if the Owner wishes to reduce costs. 16. Remove scalloped shakes at the North Wall and replace with square shakes so all'shake walls match. 17. BMA's intent for the cedar shakes is to let them naturally weather to grey. 18. Gable work at east side almost complete,gable at west side in progress. Center Core: 19. Floor cutting in progress for the new structural footing. 20. The additional floor area for the concierge desk was approved. BMA will issue a revised drawing. 21. Foundation insulation is in progress at all new foundations.Vertical board and 24"horizontal board installed. In addition, the 2x6 trusses have ripped, triangular, shaped solid blocking at the tail ends while the 2x4 trusses do not. ISSUES DISCUSSED W/ SUPERINTENDENT: Mr. Roberts informed Mr. Paulin (of this office) that the tail ends of the trusses at both of these locations and at one (1),location along the west side of the structure needed to be cut flush with the exterior wall sheathing. RANGE: 58F UPDATE ON PREVIOUS REPORTS: Carpenters to reinforce existing pre-engineered roof truss tail ends prior to cutting and removing tail ends per direction given by Mr. Paulino of this office during site visit number 001. WORK IN PROGRESS: Mechanical contractors and interior fit-up contractors on site; Masons prepping north gable wall along the east side of the structure for stucco. Carpenters working on pre-engineered roof truss tail ends at the south gable along the east side of the structure. Foundation contractors installing fmal lengths of reinforcing in bottom of footing formwork at front lobby expansion. OBSERVATIONS: All existing roof truss tails at the north gable and four (4) of the tails at the south gable had been compromised, having been cut back two (2)to three (3) additional inches in from the outside face of the exterior wall sheathing, leaving only one and one-half(1 1/z) inches of bearing on the exterior wall plate at those truss ends. ISSUES DISCUSSED W/ SUPERINTENDENT: Mr. Paulino of this office informed Mr. Roberts and the carpenter foreman that the truss tails ends were cut back too far and that they should stop work immediately,until further notice. The grille needs to align with the center of the vanity bowl. 5. The Bathroom accessories (toilet paper holder with two wall supports,two single robe hooks, and 24" towel bar)were approved by the Owner and Hilton and ready for the Owner to purchase. 6. BMA and the Owner inspected the tile layouts in several Guestrooms and determined that the tile layout in Guestroom 424 is the layout to use for all future Bathrooms. The floor tiles will have a '/4" maximum grout thickness in order to minimize tile cutting. Since the grout and tile colors are similar the grout lines will blend for a uniform appearance. Tile layouts for other bathrooms to be determined. 8. The wall tiles at the tubs and showers must have the minimum tile grout thickness that will work with the specified tile and will provide sufficient adherence without spalling.Unsanded epoxy grout is required to achieve this. The wall tile shall run from floor to ceiling. The tile at the edge of the tub leaves a triangular gap where there could potentially leave a large grout spot. Option to resolve this is to cut the tile to match the radius of the tub edge. The radius is small and may not be an issue.BMA recommends the Contractor lay out a bathroom for review and approval. 9. BMA e-mailed two exterior renderings to the Owner for their marketing purposes. 10. BMA provided floor plans to the Owner's Sprinkler Contractor for design of a new sprinkler system. 11. Because of the Owner's concern with a low back chair,BMA selected a new Guestroom lounge chair with a higher back. BMA contacted the Furniture Rep. for the Guestroom furniture regarding the Hilton requested changes to the Guestroom furniture. The changes included the addition of floor length panels below the nightstands and extend the headboard behind the mattress and terminate at the top of the box spring. East Wing: 15. EIFS at the gable walls were applied without tenting and heating per the manufacturer's written instructions. Staining and discoloration occurred. BMA recommends either applying the product as specified or wait until warmer weather if the Owner wishes to reduce costs. 16. Remove scalloped shakes at the North Wall and replace with square shakes so all shake walls match. 17. BMA's intent for the cedar shakes is to let them naturally weather to grey. 18. All counter heights to be verified for correct ADA height per drawings,including the Communal Table,Bar,Restroom Counters,etc. The granite that was substituted at these locations may be thicker than the specified stone resulting in the counters being too high,i.e.:the accessible counter at the Bar is installed at 361/a"AFF and the lavatory counters at Men's&Women's Restrooms are installed higher than the 34"specified. Construction materials and debris must be removed from all egress routes. 6. Fire extinguishers in Corridor cabinets appear to be out of date and must be inspected or replaced. 7. All interior ramps and sloped walks exceeding 5%grade must have handrails with extensions installed at both sides as indicated on the drawings. Contractor to verify all interior and exterior locations and correct accordingly. 8. The PTAC unit at the West Wing Corridor must have the cord removed that is plugged into an adjacent wall outlet. The unit must be hard wired for a finished appearance. Contractor to verify all similar situations and correct as needed. 9. An accessible King room was reviewed for completion. The following deficiencies were noted: • One of the grab bars was still missing and must be installed. • The bathroom door has two(2)robe hooks mounted in a high location',at least one(1)hook must be mounted at a max.height of 48". • The adjustable shower is mounted in the wrong location. • A fold down seat is not installed as required. Contractor to verify proper installation of all fixtures,grab bars,and accessory items in ALL accessible Guestrooms and Guest Bathrooms with corrections made per issued plans as needed. 10. Valence installation has been initiated at Guestroom draperies. The ceilings are not level and gaps are visible at one or both ends of the valence where it meets the ceiling. The valence must be scribed for a clean fit. As an alternate,the valence could be held off about 11/a' from the ceiling and a stainless steel or aluminum angle closure installed at the back side of the valence as an accent similar to the casegood details. If this alternate is pursued a mock-up installation is needed for review and approval. There are currently mock-up valences installed in a white painted finish and a stained wood finish to match the casegoods. BMA recommends installation of the white painted finish. 11. The stairwell at the pool is incomplete and must still have the fire rated doors installed as well as the fire rated enclosure at the walls and ceilings. The MEP systems piping at the ceilings shall not pass through the rated stair enclosure and these pipes must be separated by a rated drywall ceiling or fire rated ACT system if access is required. Construction of this rated stair enclosure is a critical part of the work and all such egress routes and exit access corridors must be completed prior to occupancy. 12. The Restaurant has painting and tile flooring work in progress. The wood floor is schedule for re- finishing within the next few days. 13. Renovation work at the lower level Meeting Room adjacent to the indoor Pool has not been initiated. 14. Renovation work at the access Corridor to the Gym has not been initiated. 15. The indoor pool is substantially complete and ready for FFE items. 16. On December.7,20111 spoke with your construction supervisor Mike Roberts and then with you. We discussed the renovations and additions in progress at the DoubleTree and the determination that I have made that this construction would require sprinkler protection. Since that time we have had discussions and come to an agreement on your compliance with the requirements of Chapter 148, section 26G of Massachusetts General Laws. Below 1 will cover our code review and the final determination/agreement we have reached. Chapter 148,section 26G of Massachusetts General Laws now requires a sprinkler system be installed in any building that has an addition added when the aggregate total square footage of the existing structure and the addition exceeds more than 7,500 square feet. This law exempts residential occupancies from this requirement. We needed to accomplish a code review and consult with various agencies to make our determination as to how the law applied to this hotel. Our review takes into consideration the code review done by the architect (BMA Architectural Group),as well as the past history of the building. We find that the building is a mixed-use occupancy consisting of the following use groups, A-2. Function & meeting rooms,breakfast area,restaurant,bar area. A-3 Fitness area,indoor pool,lobby. B Business offices. M Retail space. R-1 Hotel guest rooms. S-1 Various related storerooms. S-2 Various related storerooms. U Carport. We noted that many of these use groups are not ancillary to the R-1 use group as they are used completely independently,such as the function rooms,gym,and restaurant.We noted that the main corridor serving the first floor guest rooms is also the main transit corridor from the center core business and function areas to the west wing restaurant,and function rooms.We also noted limited separation of the residential and commercial spaces in some areas with the exit corridors having only glass doors as a barrier. Existing protection systems within the building consist of a limited area sprinkler system protecting the entire west wing basement(fitness,pool,function,and storage areas).There is a pre-existing non-conforming standpipe system in some stairwells that consists of a mix of 1.5 inch and 2.5 inch outlets with no occupant hose.There is a complete fire alarm system that monitors the sprinkler system and manages elevator recall. Local smoke detectors are installed in the guest sleeping rooms. Our findings are, - The building is a mixed-use occupancy with various commercial uses as well as a residential R-1 hotel guest room use. - Many of the commercial uses are not ancillary to the R-1 usage. - The additions are being installed in various areas of the commercial usage but contained to the first floor center core area. - The commercial usage areas alone exceed 7,500 square feet in aggregate area before the additions. - Major renovations will also be occurring in some of the commercial areas,specifically the center core assembly rooms and lobby. - The Architect did not list M.G.L. Chap. 148,section 26G in the outline of their codes review. - The owner is upgrading the building wide fire alarm system. - Sufficient water volume and pressure are available as provided by the municipal water company. - The renovations occurring to the first floor restaurant area do not add aggregate square footage to the structure and would not be considered major renovations under the IBC or Mass State Amendments.This commercial space is separate from the center core space and used autonomously. After our-review,we found that M.G.L. Chapter 26G does apply.I spoke with Mr. Roberts regarding this and informed him that an order to'sprinkler protect the commercial areas would be forth coming. Following this discussion a second meeting was held between Mr. Bornstein,Mr.Roberts and Deputy Chief Melanson. We discussed various issues that included potential costs and construction challenges,removing the commercial additions,removing some of the additions,protecting only the center core commercial areas,and appealing the order. Mr. Bornstein contacted his Fire Protection Engineer Maurice Pilette via phone.After hearing the situation,Mr. Pilette explained that this is a conflict of interest situation for him as he is the Vice Chaif of the Sprinkler Appeals Board that would hear and investigate any appeal on this order. He did however,explain the law and appeals processes along with the coming schedule of hearings. We then discussed a potential resolution that would involve sprinkler protecting the center core commercial area as this location is where the additions are to be installed.All parties had additional research to conduct and the meeting concluded with a tentative agreement that we would all investigate this option. On December 8,2011 Mr.Bornstein contacted me via phone and proposed that he would fully sprinkler protect the center core commercial area including the second floor office area.This discussion was followed up by a hand delivered letter(attached) outlining this proposal. My research indicated that this would be an agreement that would be compliant with the intent of M.G.L. Chapter 26G in this mixed use building.I gave Mr. Borstien verbal approval and assured him I would provide him with documentation to memorialize the entire issue—this letter. Below I have provided the final sprinkler order from the Hyannis Fire Department. Chapter 26G does apply. As such,you are ordered to comply with the requirements of this law and fully sprinkler protect all areas of the center core commercial area.This will include the entire vestibule and lobby area,the function rooms,storerooms bathrooms and offices on the first floor.The stairwell landings in Stair 176 shall be protected to the entry doors into the West Wing.The second floor office suite shall be protected.The Building Commissioner of the Town Of Barnstable shall address any dispute regarding the occupancy classification of a given area. You may appeal this order,within 45 days after you receive it,to the to the Automatic Sprinkler Appeals Board as provided in section two hundred and one of chapter six.The board may grant a reasonable waiver from the provisions of this section,or may allow the installation of a reasonable alternative or modified system of automatic sprinklers upon reviewing the characteristics of buildings that have architectural or historical significance.To request an appeal application,or for further information,call the Administrative Assistant at (978) 567-3181.All appeals must be filed within 45 days (calendar days) after receiving service of this notice.
