Proposal: Construction of a new 5,219+/- sq ft restaurant with drive-thru. Property is subject to
existing Cape Cod Commission(CCC) DRI Decision. Existing lots are currently paved
for parking.
Dear Attorney Ford:
Please be advised that subsequent to the formal site plan review meeting on September 3,2015,revised
plans for the above.proposal has been fourid to be approvable subject to the following:
❖ Approval is based upon, and must be substantially constructed in accordance with plans .
entitled "Site Development Plans for Proposed Chick-fil-A with Drive-Thru"consisting of
17 Sheets;prepared for Chick-fil-A by Bohler Engineering, Southborough, MA, Scale 1"_
20'. Plans are dated August 27, 2015 with final revisions to Sheets L-1.0 Landscape Plan
and C-2.0 Site Plan on December 2, 2015, incorporating comments from initial site plan
review meeting, GMD meetings, ZBA and VHB comments; and, "Stormwater Drainage
`Analysis-Chick-fil-A, 104 Enterprise Road",prepared by Bohler Engineering,
Southborough,MA dated August 21, 2015.

❖ Final floor plans depicting the#of bathrooms;seats, storage, sinks,etc, must be submitted to
the Health Department for approval at the building permit stage.
❖ The designs of the water and sewer tie-in must be approved by DPW. Contact Hans Keijser,
Water Dept. Supervisor, 508-778-9617 ext. 3502; and Town Engineer, Roger Parsons, 508-
790-6302.
❖ Photometric plan meeting CCC Technical Bulletin for Lighting Standards depicting zero (0)
light spillage onto abutting'properties must receive approval from Roger Parsons, Town
Engineer.
❖ The granting of a special permit from the Zoning Board of Appeals will be required.
❖ Final Cape Cod Commission approval of the modification of the existing DRI will be,
required.
❖ Applicant must obtain all other applicable permits, licenses and approvals required.
Upon completion of all work, a registered engineer or land surveyor shall submit a letter of
certification,made upon knowledge and belief in accordance with professional standards that all
work has been done in substantial compliance with the approved site plan(Zoning Section 240-105
(G). This document shall be submitted prior to the issuance of the final certificate of occupancy.
A copy of the approved site plan will be retained on file.
