Modification of the existing Chapter 40B Comprehensive Permit is proposed. Dear Attorney Freeman: At the formal site plan review meeting held Feburary 4, 2016,the above project received an advisory review prior to application to the Zoning Board of Appeals to for modification of the existing Chapter 40B Comprehensive Permit for this site. • Plans reviewed are entitled"Site Plan Phase 5 & 6 of land in Hyannis, MA, 550 Lincoln Road Extension"prepared for Living Independently Forever, Inc. by Down Cape Engineering, Inc., Yarmouthport, MA, dated January 28, 2016, scale 1"=30'. • It was identified that relief for reduction of required parking will need to be included in the modification of the comprehensive permit. • Addressing and labeling of all buildings on the site will need to be formally resolved and reflected in the comprehensive permit. Consultation with Hyannis Fire Department and DPW will be needed. • DPW has requested locations of existing utilities and lines be added to the plan as well as the rim inverts and manhole diameters. Final approval must be obtained from the DPW regarding requested drainage calculations and perc. tests; drainage and utility design. DPW has also requested an as-built sewer plan for Building E. r . Upon completion of all work, a registered engineer or land surveyor shall submit a letter of certification, made upon knowledge and belief in accordance with professional standards that all work has been done in substantial compliance with the approved site plan(Zoning Section 240-105 (G). This document shall be submitted prior to the issuance of the final certificate of occupancy. A copy of the approved site plan will be retained on file. Sincerely, Ellen M. ON PROPOSED ADDITION. Dear Mr.Perry: I would like to take just a minute to thank you for all the help you have given us over the past several months and once again with our new request to seek the elimination of an elevator in the addition. This would be accomplished by upgrading the size of the building thus allowing the formerly proposed physical fitness program to move from the basement of the new addition to the first floor of the newly proposed addition. I understand that you would have to review a completed set of drawings showing our proposal before any final approval or rejection could be made but it was encouraging to hear that you would consider allowing us to eliminate the elevator by keeping all program elements on the first floor. Again,thank you for all your time on this and I hope to have more complete information sometime in late summer. The interior walls were constructed with a piece of strapping on top but they do not line up under strapping attached to the trusses. The result is"floating walls"held still by the offsets in the design. I would recommend anchoring these walls to prevent any further sagging of sheetrock along the edges. (L.I.F.E. c550 Lincoln Road Extension Hyannis, MA 02601 Re: Verification of Compliance with Terms of Comprehensive Permit Zoning Board of Appeals Nos. 1992-68 & 1995-20 Dear Sir or Madam: Please be advised that I am writing to obtain verification that you are complying with the terms of the comprehensive permits referenced above. Specifically, the comprehensive permits require that 4 of the 16 units shall be available for rental in perpetuity to an eligible low or moderate income individual or family, the maximum rental of which shall not exceed that allowed by the Department of Housing and Community Development under the Local Initiative Program (LIP). In order to assure that you are in compliance with the terms of the comprehensive permit, we request that you provide the following documentation for each of the four (4) affordable units: 1. Name of tenants and number of tenants occupying unit; 2. Identification of unit being occupied by tenant(s); 3. Verification of gross income of tenant (income tax returns or other verifiable documentary evidence of income for last twelve months); 4. Rent being charged to tenants including any incidental charges.. I would appreciate it if you would provide me with the above information within the next thirty (30) days. Once I obtain and review the information provided, I would like to establish an ongoing monitoring process so that the required information can be provided to Building Commissioner and the Zoning Board of Appeals on yearly basis.
