RE: Site Plan Review#014-19 Commercial Kitchen for Lease

Proposal: Use of a space previously used as a food pantry for a,commercial kitchen for lease
to others. A contract with a cleaning company is proposed to clean between each
user of the kitchen.
Dear Mr.-Reed:
The above proposal was administratively approved by-the Site Plan Review Committee at the
informal site plan review meeting held March 5, 2019 subject to the following:
• Existing and proposed informal floor plans depicting the location and type of equipment.
• The location and type-of cooking equipment that receives final approval will need to be
permanent and may not shift to accommodate those leasing the kitchen per Hyannis FD.
• One contact for the person responsible for inspections will need to be identified per
Hyannis FD.
• Hood and fire suppression system will need to be certified and inspected annually per
Hyannis FD. Contact: Deputy Chief Dean Melanson, Hyannis FD 508-755-1300.
• Signage and/or'any exterior changes will require approval of the Hyannis Main Street
Waterfront Historic`District. "Contact: Karen Herrand, Administrative Assistant 50&
862-40.64.
• A food prep sink will need to be added to the plan.
• A food service permit will need to be obtained from the Board of Health by each entity
leasing the kitchen.

• If entities leasing the kitchen space are preparing food to be served to a 3rd party at a
restaurant,that entity would also require a State permit. Health Dept. Contact: David
Stanton, Chief Health Inspector 508-862-4647.
• E-mail dated March 15; 2019 from DPW confirming that after performing a dye test,the
existing 37bay sink in Unit D6 is plumbed into the existing 1,000 gallon exterior grease
trap. The grease trap appeared to be functional. Contact: Griffin Beaudoin, Interim
Assistant Town Engineer 508-790-64.00.
Applicant must obtain all other applicable permits, licenses and approvals required.
