This included the handicapped bathroom on the main floor, the ramp on the left side of the structure which enable passageway from the lower level seating area to the upper level seating area, and the emergency egress locations. Based on my review it is my professional opinion that the restaurant is in compliance with all applicable Codes and is therefore ready for an occupancy permit. The size is inadequate to support the design loads for which it was intended. Two additional 16"L.VLs shall be added to the outside of'the existing.LVLS. The existing roof structure was reinforced with additional collar ties and additional studs supplemented existing studs at the exterior walls. The subgrade has been compacted and flowable fill will be used to level the surface for the slab on grade pour. Rough plumbing and conduit for running wires is in place under the slab. Electrical wiring has been started. -The rough frame is complete on the "red" building. The upstairs has been converted into an office space (was originally an apartment). As a result the layout has been re-arranged. There was no plan provided for the office space on the design drawings. I spoke with Phil concerning the existing stairs and whether or not they would be allowable and suggested he contact the building department for a determination. Being responsible for the controlled construction of the project I could not make that determination. -The outside bar rough frame was complete. The existing door on the right sidewall became a window and the existing window on the left sidewall became a door. A header beam was added in order to take out the center post at the front counter. DeAngelo Phil Miller(Miller-Starbuck) Location: 599 Main Street Hyannis, MA Building Permit# Remarks: 9:00 am-10:00 am - 36 degrees: sunny The following was observed: Demolition of the existing finishes (wall board, flooring, ceilings) was complete and the existing structural conditions were open for review. -In some areas the framing�(roof rafters, sills, exterior walls) the framing has deteriorated.to the point where it needs to be replaced or reinforced. These areas were identified and Miller-Starbuck will provide the necessary framing which this office will review,for.its adequacy. In cases where the sill has rotted one alternate discussed was to pour the slabs on grade with a curb. If this option is pursued a new sill will be placed and anchored into the curb. -When the first floor ceiling was removed (and second floor framing.exposed) at Ahe rear retail building it was observed that there was a significant deflection in the floor framing..Oncb the. plumbing is re-worked either a column will be added (down to the block foundation wall)or the beam will be reinforced as required to support the .required design loads.: -This office will be providing miscellaneous design services for existing conditions that were not observable before the framing was exposed-similar to the area noted above. There was no standing water or debris at the bottom.of the excavation. In addition, the excavation for the four 18 diameter sono-tubes(2 at the outside corner and two at mid-span from the existing building to the front corners) was down to 4 feet below grade: -Reinforcing steel for the slab was also in place. It consisted of#4s at 12" on center in both directions which was lapped mini<mumof 12' at all locations. In addition, there 244s (top and bottom) in the haunched slab spanning the perimeter of the slab.Vertical#4s spaced at 24 on center were tied into the 244s in the haunched section. In addition, there were 244 dowels extending out from 4 sono-tubes. Finally, the horizontal steel was doweled into . the existing building foundation. The clearance between the underside of the reinforcing steel and the finished grade was maintained by the use of brick pavers. Excavation for the five 18" diameter sono-tubes..at the right side of the building(supporting the six foot extension) was-complete. The bottom of the excavation was four feet below grade and'the bottom of the excavations were free of debris and standing water. -No deviations or omissions (other than*what was noted above)were observed. The interior post is supported'by-a 46 top plate (5'-1 rough.opening), The rafters have been installed and are toe-nailed at each end. The exterior sheathing (1/2 plywood) has been nailed to the studs at 4" on center.. Anchor bolts (5/8" diameter)with washer plates were observed at 36" on center. Main Restaurant The six foot addition on the right side of the existing structure has.been framed. The window headers consisted of 3-9 %" LVLs continuous along the entire length with 372x6.jack studs between each window. The 2x10 rafters spaced at 16" on center were connected to the outside wall with Simpson H2.5 ties and to the.inside ledger board with Simpson L09 angles. The ledger board was connected at each stud with 4 nails. The exterior sheathing was nailed to the studs at 4°on center. Bar Area -The low roof framing was connected to the exterior wall with Simpson H2.5 hangers at the exterior.wall and to the 3-16" LVLs at the existing rear-wall with Simpson flush.mount.hangers. The 3-16" LVLs were connected at each end with 3-2x6 posts. The high roof rafters were also attached to the LVLs with-Simpson flush mount hangers. In addition a truss configuration was added to every other rafter.
