ELECTRICAL Incorrect phone jack in kitchen. Ceiling light fixture in foyer not installed. No switch for the above fixture. No post light installed. No switch for the above light. INTERIOR SURFACES Plastered walls throughtout the dwelling have been applied in an unworkman like manner, (Joints not feathered out, screws and nails not set, trowel marks not filled, wall receptacles and light switch holes cut to large. ) Wall -across from downstairs bath does not appear to be plaster. Hole in wall under kitchen cabinets . E Ceiling in foyer needs painting where it was patched . I3. : Ceiling and wall in garage has water damage. Ceiling and wall in downstairs bedroom has water damage. Foyer ceiling fixture is scratched. Underlayment in two of the upstairs bedroom are not properly fastened to the subflooring. Underlayment in the ,area where the kitchen and dining area meet are not fastened to thesubflooring. Repair from damage from circular saw to hardwood is not acceptable. Several other areas of damage to hardwood floor were not properly corrected. P Y Butt ends of the hardwood flooring and bull nosing at the fireplace extended hearth have been installed up to 2" out of square. Many of the butt ends of the hardwood flooring at the baseboards have been' cut cut to small for the baseboard to cover them. Hardwood flooring cut at every door casing was cut too short and as a result door casings do not cover the gaps . Hardwood by the front door appears to have water damage. Face nails in hardwood floors are not set and filled. Metal capping for carpets at each door are missing. Carpet in master bedroom closet not seamed. r Carpet is upstairs large bedroom is not seamed in the closet or IC behind the entry door. STAIRWAY Interior stairway to second floor is not completed and installed with poor craftsmanship. (Rises are #2 pine and full of knots, rises are of different heights and not properly fitted, there are 1/4" spaces at seams, treads are different sized, rail balusters are nailed to front of the stair treads, handrail and balusters do not match, etc. ) Stairway to basement top is nailed to ferring not plywood. INTERIOR DOORS The use of shims for all existing interior doors was eliminated and therefore are loose and not closing properly. All door casings were cut too short and do not sit on the flooring. Stain is needed on most doors and several door jams. Nails for interior trim are not set and filled. Paint and stain door and casing to garage to match others in the dewelling. Most miter joints open at door and window casings . Door to -downstairs bathroom needs to be reversed. Closet door- in upstairs large bedroom is dragging along the carpet. Several door raised panels have cross gain shrinkage. Two of the bedroom closets need door slider guides. Upstairs large bedroom closet door is stained in two different colors. Closet door in foyer is two small (height and width) for opening. :Dowstairs bathroom door has a large chip out of it. " Archway jam at foyer was cut to small . Stain . on glass slider is incomplete. EXTERIOR DOORS Paint front door. Rubber stops missing at jam for sliding glass door. Many pieces of baseboard through out need nailing. Several pieces of baseboard are cracked or broken. Most baseboards need a coat of urethane. WINDOWS Interior window sills have been installed too low in some cases up to 1/2" . A few of the window jams are in need of a wood filler. Window in upstairs large bedroom does not open properly. Twelve (12 ) wooden grille inserts are missing. One (1 ) wooden grille . insert is broken. Diningroom window casing is split. Cranks to kitchen windows are not anchored. CABINETS AND KITCHEN Three center wood styles to kitchen cabinets are not screwed together. Utility cabinet is not installed. End panel on one base cabinet is missing. One base cabinet style is not ,screwed to adjoining one. One cabinet door is warped. Two sets of cabinet doors are not aligned. Damage from counter installation on wooden style above drawer base. Five ( 5 ) extra sliding drawers are missing. Caulking needed at top of back splash at sink wall. Back splash has 3/8" gap on' stove wall . Average of 1/2" space between hardwood floor and base of cabinets. Drawer in downstairs bathroom vanity needs to be aligned. Fillers- fell out of downstazrs. bathroom vanity when chimney fell . r CLOSETS Closet shelf in large bedroom is warped and has no center support. Linen closet is missing four ( 4 ) shelves. MISCELLANEOUS ITEMS DEQE letter of compliance. Survey of property. Return ladder. J.- $ 5 still due me for living expenses from March 1st to March loth while the chimney was being rebuilt. Currently being billed by Stephen Jones (plumber) ( $241 .25 plus interest) for draining pipes while chimney was being rebuilt. Plimouth Vacuum will not install unit until -bill is paid ($150.00 ) . AT&T bill ,($29.80) for phone that cannot be installed because of incorrect jack. (There is an inspection report, photographs, and receipts to verify the above information) May 15,_ 1987 i Alleged Violations 2101 . 9.1 Openings: There shall be no openings from a private garage directly into a room used for sleeping purposes . other openings between the garage and residence shall be equipped with doors providing a fire rating equivalent to twenty (20 minutes Note: This door is installed, but with a half inch gap at the bottom. 2101-1 Figure of Stair Detail shows 9" minimum run and 8 1/4" maximum rise. Note: Runs are a mimimum of 9" , but vary up to 1/2" and risers are vary between 8" and 8 1/2" . 2101 .11 - Handrails and guardrails: Handrails having minimums and maximum height of thirty (30) inches and thirty-four (34) inches respectively, measured vertical from the nosing of th treads shall be provided on at least one(1) side of stairways of three (3) or more rises Note: The top five stairs do not have a handrail . 2102 . 3 Footings : All exterior walls, bearing walls, columnms and piers shall be supported on solid masonry, or concrete footings, or other approved structural systems which shall be of sufficient design to support safely the loads imposed as determined from the character of the soil. Note: There is some question whether or not the concrete blocks are sufficient footing for back deck. 2102.8 . 1 Wood in contact with the ground: All wood in contact with the ground and supporting permanent structures shall be approved treated wood. All wood below two (2) inches above surrounding grade, or in locations subject to ponding of water and/or dampers shall be of approved wood type or treated ! (pressure) . Note: The wood between the garage floor and walls is not treated. 2104 . 3 . 1 General: Exterior coverings shall be installed in accordance with this section, Table 2104-7 and manufacture' s a recommendations. Note: Cedar shingles at rear corner of the garage have been installed with a 1/2" space between shingles and garage wall at corner. Similar flashings shall be installed at the intersection of chinmeys or other masonry construcion with frame or stucco walls, with projecting lips on both sides under stucco copings; under_ and at the ends of masonry, wood or metal copings and sills; continuously above all projecting wood trim; at wall and rook intersections ; under build-in gutters; at junctions of chimneys and roofs; in all roof valleys and around all roof openings . Note : Both the chimney for the fireplace and the chimney for the oil burner are not flashed correctly. The front stoop is not flashed at all . 2106 . 5 Attic Access : An accessible attic opening not less than twenty-two ( 22 ) inches by thirty( 30 ) inches shall be provided Ao any attic area with clear headroom of three( 3 ) feet or more. Note : The attic access measures 17 1/2" by 20 1/2" . 2107 . 3 . 4 Flashing: -Roof valley flashing shall be provided of riot less than No. 28 galvanized sheet gauge corrosion-resistant metal and shall extend at least eight(8 ) inches from the center line each way, and shall have a splash diverte.r rib not . less than three-quarter ( 3/4 ) inch high at the flow line formed as part of the flashing. Sections of flashing shall have an end lap of not less than four ( 4 ) inches . 2107 . 3 . 5 Other material : Roof valley flashing may be of laced composition shingles, applied. in a approved manner, with in underlay of not less than twelve (12 ) incites wide laid face down, and the top layer not less than twenty-four ( 24 ) inches wide laid face up. Note : There is water penetration in tow areas under the valley between the garage and the foyer . 2108 . 2 .7 Cleanouts: Every chimney flue shall be provided with an approved cleanout having a tight-fitting cover.• Such cl.ennouts shall be installed at least twelve (12) inches below the lowest chimney opening. Note : There is no cleanout in the chimney flue . 2108 . 2 . 9 Smoke test : Masonry chinmeys shall be proved tight by a smoke test after erection and before being put into use. Note : No smoke test was done . The Hyannis Fire I)epa.rt- ment informed me that the fireplace was not fit to burn in. 2108 . 3 . 2 . 6 Termination (height ) : Masonry chimneys shall extend at least three ( 3 ) feet above the highest point where they pass through the roof of a building and at least two ( 2 ) feet higher than any portion of a building within ten ( 10 ) feet . For fireplaces with an opening of less than six( 6 ) square feet, the hearth shall extend not less than sixteen (16 ) inches in front and not less than eight ( 8 ) inches on each side of the fireplace opening. For fireplaces with an opening of six ( 6 ) square feet or more, the hearth shall extend . not less than twenty ( 20 ) inches in front and not less than twelve (12 ) inches on each side of the fireplace opening. Such hearths shall be properly supported of reinforced to carry their own weight and all imposed loads. An Independent Building Inspection Report and two carpenters gave the same evaluation. Repair from damage from circular saw to hardwood is not acceptable. There is damage to the hardwood floor near the fireplace that was obviously done with a circular saw. The repair . that was attempted in not acceptable. Several other areas of damage to hardwood floor were not properly corrected. No-house delivered in good shape. .These are similar to the one described above. Butt ends of the hardwood flooring and bull nosing at the fireplace extended hearth have been installed up to 2" out of square. Will discuss. There is nothing to discuss. The corner is 2" out of square. This is poor workmanship and need to be corrected so the fireplace can be completed. Many of the butt ends of the hardwood flooring at the baseboards have been cut too small for the baseboard to cover them. Will discuss. There is nothing to. discuss. This is obvious poor workmanship. Hardwood flooring cut at every door casing is cut too short and as a result door casings do not cover the gaps. Will discuss. Same as above. Hardwood by the front door appears to have water damage. The hardwood appears . to have damage. There `is light coming through the bottom -of the door. My assumption is the damage - is from water. Inside of the bottom of the fireplace hearth is 1" higher than the .hardwood floor, it ,is .to be flush. f The floor of the fireplace firebox was to be flush with the finished hardwood floor. The balusters were chosen from a photograph for style. When it was realized that they would not fit the handrail (which was selected by the builder) either I should have been made aware of the problem and had an opportunity to make another selection or. had the carpenter been knowledge in stair building he would have known there was a need for a subrail which would have. eliminated the problem. The use of shims for all existing interior doors was eliminated and therfore are loose and not closing properly. No-doors have expanded _ since de_live�ry-probably due to humidity. The problem is not related to humidity. The Independent Building Inspection Report found that the use of shims was eliminated, this was done the March 21st when the humidity was low. Casings do not fall of the wall due to humidity. All door casings were. cut too short and to not sit on the flooring.
