Dear Mr. Geiler:
In accordance with Section 107 of the Massachusetts State Building Code this office has reviewed the completed
MetroPCS equipment installation at the above-referenced location. The purpose of the review was to establish
compliance of the completed project with the approved design plans by this office entitled"HYN0016A Crown
875052 Hyannis"dated 08/17/12, RevO,and applicable provisions of the Massachusetts State Building Code.
With respect to MetroPCS's equipment installation,the change in inventory installed on the monopole was
completed under a different scope of work and building permit.A review of the approved structural analysis
reveals that the monopole is structurally adequate to support the microwave dish. It is my opinion that any minor
deviations from the design plans do not endanger the intended function of the facility or the safety of the public.
This letter is not intended to relieve any party responsible for construction from meeting the requirements imposed
by contract or other means, including commonly accepted industry practices.
Based on the inspection performed, I, Marc Chretien, licensed professional engineer number 40313, hereby
certify that to the best of my knowledge, information and belief,the work has been done in accordance with the
approved plans and the provisions of the Massachusetts State Building Code.
