Scanlan Co.,Inc.(JKS) Mike Breen(JKS) f A field inspection.was made,upon the request of David Robins(JKS)to verify placement of rebar in accordance with CEC drawing in the;west half of BulglC footing.CEC observed that the rebar was placed as noted below: 1:~ CEC observed that the geometry,width,and depth of the footing appeared to be in accordance with CEC drawings. 2. Vertical dowels in wall were placed at approximately 16"OC and were#5 bats in the center of the footing as specified by CEC:Height of the dowels appeared to be approximately28"above the top of the footing.It was also observed that the transverse bars were placed at.16"OC and were#4 bars. CEC called for these bars to be placed at 18"OC. Since the spacing was closer than specified,the spacing is acceptable.The continuous longitudinal bars were evenly spaced across the width of the footing and were 4-#4 bars, 3"clear from the bottom of the footing,as specified by CEC.Lap lengths for these bars were at least the specified 40-bar-diameter called for by CEC. 3. Corner bars in the footing appeared to be placed correctly.One corner bar lap length was insufficient,and this was remedied before CEC left the site. 4. David Robins discussed with CEC that they were on top of compaction and concrete testing with Briggs.It was discussed, per phone conversation with David Morand(CEC),that certification for compaction under footings was to be provided to CEC for review and,if CEC does not receive certification,then foundation in areas where no certification can be verified may be ripped out. It was observed that the forms and rebar placed for retaining wall sections were in accordance with CEC plans with the exception of a few rebar locations not placed 2" clear from the inside of the wall form. • All continuous horizontal rebar appeared to be#5 bars spaced 12"O.C. as specified. • All vertical rebar appeared to be#5 bars spaced 12"O.C. as specified. . • 2"Dia._PVC weeps appeared to be installed for drainage as specified. • All rebar doweled into existing concrete walls appeared to be set:using the specified epoxy. • Rebar located in wall section"C"on.SK-1 measured 4 1/2"clear from inside"of wall form,not 2" clear.as'.speeifiedon`C-EC plan. • Rebar located in wall section `B"at angle only on SK-1 measured`7"clearfrom inside of wall form, not 2"clear as specified on CEC plan. • All form geometry appeared to be sized and placed properly as specified. • It was also observed that a hole was dug under the footing of buiding"D"for a drain pipe to pass through. Scanlan to inspect the final punchlist items and to perform a final walkthrough for Buildings"A",`B";an "C';fit e following was observed: Building"A" Unit 1 • The floor framing appeared to be framed in accordance with CEC plans and the approved shop drawings with the following exception.Th6 LPI-20 I joists,which frame into the LVL beam at the front of the building,used Simpson HU412 hangers instead of the specified Simpson HU312 hangers.The Simpson HU412 hangers are too wide for the 1-joists and need to be removed and replaced with Simpson HU312 hangers. • The Simpson LS90 angles were placed at the sill to the rim board,and the rim board to the wall above. Unit 2 4 • A set of stairs was being built to the deck at the front side of the building. It was observed that no footings were placed under the main support posts for the landing. CEC recommended to Jim that footings be placed to prevent settling of the stairs. Jim informed CEC that it is his intent to place footings to support the stair landing. y Unit 3 5 • It was observed that 2 Simpson SDS screws were present where the valley meets the ridge.These valley beams needed to have the supports located in the wall below to extend up to the underside of the valley beam.It could not be observed if this was completed due to the drywall already being placed.As noted in Inspection Report 44 these areas were to be left open. Scanlan shall submit a written document on company letterhead and submit to CEC to indicate this work has been completed. • It was observed that the ridge beam had no mechanical fastening to the support element. Connection shall be made with either Simpson L or T brackets, or Simpson A35 angles. • The rafters that frame into.the,ridge beam did not have any.hangers.Provide proper hangers as specified on sheet A- S4.1 or Simpson.A35 angles. General • CEC noted that the steel columns in the basement had concrete placed around the base.It appears that a bond breaker was used at the base of these columns. • The LVL beam in the fire room bears on top of the steel column and is attached to the steel column with 2 lag screws and 2 hex head screws(refer to picture).Remove the hex head screws and replace with the appropriate size lag screw. None of the walls.had double#5 top bars;instead a single#5 bar was tied to the rebar in the center of wall. CEC approved this change in all walls before visiting the site. • All corner bars appeared to be present with a minimum of 40 bar diameter lap. 3. It was observed that the rebar in the southeast wall had approximately 1-1/4"clear cover in the middle_ section of the wall. Upon informing Dave Robbins of this,the bars were adjusted to the approximate center of the wall. 4. CEC noticed that the dowels for attaching the CMU wall above to the demising concrete walls were not placed.Dave Robbins informed CEC that they are going to wet place the dowels. CEC allowed JKS to wet stick the dowels in building "C"only,and informed David Robbins that all the other buildings must have the dowels tied to the rebar mat. 5. Rebar on the northeast footing of building"B"was noticed to have been bent. JKS informed CEC that the bars were bent by the excavator and asked CEC to provide a fix for the rebar. It was observed that the rebar placement for the balance of the footings in Building"B"was in accordance with CEC plans. • Vertical dowels appeared to be#5 bars and allow for at least a 30" lap,as specified. • Footings appeared to have the correct depth and width. • Continuous bars appeared to be of the correct quantity and consisted of#4 bars equally spaced across the footing; • All stepped footing locations appeared,to have a 2 horizontal to 1 vertical slope.All rebar appeared to-have 3" clear cover from the bottom of footing. The rebar appeared to be of the correct size and have been properly bent to a 2 horizontal to 1 vertical slope. 3. CEC informed David Robbins that the slab on grade haunch detail should maintain the same elevation along the entire perimeter of the'slab. 4.
