Information and Instructions
Massachusetts General Laws chapter 152 requires all employers to provide workers' compensation for their employees.
Pursuant to this statute,an employee is defined as "...every person in the service of another under any contract of hiie,
express or implied, oral or written."
An employer is defined as"an individual,partnership,association,corporation or other legal entity,or any two or more
of the foregoing engaged in a joint enterprise, and including the legal representatives of a-deceased employer, or the
=eaciv�nr trustee!Dian individual.partnership, association or other legal entity, employing employees. However the
owner of a dwelling-house having not more than three apartments and who resides therein;or the occupant of the
dwelling house of another who employs persons to do maintenance, construction or repair work on such dwelling-house
or on the grounds or building appurtenant thereto shall not because of such employment be deemed to be an employer."
NML chapter 152, §25C(6)also states that"every state or local licensing agency shall withhold the issuance or
renewal.of a license or permit to'operate a business or to construct buildings in the commonwealth for any
applicant-who has not produced acceptable evidence of compliance with the insurance coverage required."
Additionally,MGL chapter 152,.§25C(7)states`Neither the commonwealth nor any of its political subdivisions shall
enter into any contract for;the performance of public work until-acceptable evidence of cflmpliauce with the in.�ance
requirements of this chapter have been presented*to the contracting authority."

necessary,supply sub-contractor(s)name(s), address(es)and phone numbers) along with their certificate(s) of
insurance. Limited Liability Companies'(LLC)of Limited Liability Partnerships(LLP)with no employees other.than the '
members or partners, are not required to carry workers' compensation insurance. If an LLC or LLP does have
employees,a policy is required. Bp advised that ibis affidavit may be submitted to the Department of Industrial
Accidents for confirmation of insurance coverage. Also be sure to sign and date the affidavit. The affidavit should
be returned to the city or town that the application for the permit.or.license is being requested,not the Department of
Industrial Accidents,- Should you have any questions regarding the law-or'-if you are required to obtain a workers'
compensation policy,please call the Department at the number listed below. Self-insured companies should enter their
self-insurance license number on the appropriate-line.

Remarks: *Concrete mixture contained mid-range water reducer. I was informed by Dave with J.K. Scanlan
that Peter Zoni of Cape Cod Ready Mix would control the addition of water. **The third load delivered yielded
results in excess of that allowed by the project specifications. The truck was empty by the time the results
were obtained. I notified Dave with J.K. Scanlan as well as C.C.R.M. dispatch. I was informed by C.C.R,M, that
a reduction in the air entrainment would be made on the next load (No. 5). Tests were conducted on this load
to insure adequate adjustment of air entrainment. A cursory reinforcing inspection was performed as I was
informed that the structural engineer had already been out to inspect the reinforcing.

1. The rebar in the retaining wall between Building"A"and Building`B"was reinspected(see field inspection
Report 21); the following was observed:`
a. Rebar on the low grade side, in the section of wall running parallel to the two buildings,was noticed to be
touching the outside of the form. Richard Scanlan was informed that the rebar should be corrected to have
2"clear cover before placing concrete. Rebar at this location was corrected before leaving the site.
b. Rebar in the remaining area of the retaining wall appeared to have the correct spacing,1size, and..geomgtry.
c. Two rows of rebar were observed to be located approximately 2"from high grade and 2"from ow grade.
d. All vertical bars appeared to.be#5.at 12"O.C.
e. All horizontal bars appeared to be#5 bars at 12"O.C.
f. All corner bars appeared to have a minimum of 2'-0"on each side of the bend. '
g. All horizontal bars appeared to be epoxied into the previously placed foundation waVf!for Building

d. Dowels appeared to be hooked at the ends, which appeared to be the required 10".
e. #3 ties were observed at approximately 10" O.C.
f. A small tool with a wooden handle was at the bottom of the form closest to the street on Building"C".
CEC informed JKS that it was still acceptable to pour footing.
g. Two forms had dirt piled in the center of the rebar. JKS was informed that the dirt under the footing must .
be level. This was corrected before leaving the site.
h. JKS was informed by CEC that there was some concern with the compaction of the soil under some of
the footings. JKS informed CEC that Briggs had been on-site and tested the areas in question. JKS was
informed that it was acceptable to place concrete if the areas had been tested and meet contract document
criteria, and that CEC must be provided with the reports stating such.
3. The crawl space wall under Unit 1 in Building"A"was observed for rebar placement and the following was
noted:
a. Rebar in the short wall,was all in place and appeared to be of the correct spacing, size, and geometry.
b. Rebar for the tall wall (this wall was removed due to means and methods issues)was about 50 percent
complete. JKS informed CEC that they were waiting to receive the remaining rebar.
c. All rebar that was in place appeared to be of the correct size, spacing, and geometry.
d. All bars appeared to be#5, with spacing of 12"O.C. vertical and 16"O.C. horizontal.
e. JKS requested to take pictures of finished rebar and forms and submit them to CEC so they may pour
concrete on 9/14/07. CEC informed Richard Scanlan that this is acceptable with the following
conditions:
• Pictures must be taken to show remaining rebar in place.
• Pictures must clearly show location of wall.
• Width of the wall and rebar location in the wall must be clearly marked.
• All pictures must be submitted before placing concrete.
• JKS must obtain confirmation from CEC that the pictures are acceptable before placing concrete.

conforming with CEC plans.
• All continuous horizontal rebar appeared to be#5 bars spaced 12"O.C. as specified.
• All vertical rebar appeared to be#5 bars spaced 16"O.C. and is non-conforming to CEC plans, as 12"
O.C. bars are specified.
• 2"Dia. PVC weeps appeared to be installed for drainage as specified.
• All rebar doweled into existing concrete walls appeared to be set using the specified epoxy.
• All hooked footing dowels appeared to be 12"O.C. and lap with the vertical rebar spaced at 16"O.C.
This is non_conforming with CEC plans.
• Only one row of rebar exists within the wall form and CEC specifies two. This is non-conforming
with CEC plans.
• The one row of rebar that exists within the wall is placed approximately 4"to 6"from the inside of the
wall form(high side). CEC specifies 2"clear. This is non-conforming with CEC plans.
• It appears that the rebar is doweled 12" O.C. into building"A"and`B"and consistent with a 24" lap
at all corner bars. Horizontal rebar runs straight into building"A"and is lapped 20"with existing
building"B"dowels.
All non-conforming items were noted to J.K. Scanlan so that revisions could be made.
2. It was observed that the footings for building"A"(southwest side)were in accordance with CEC plans.
• All footing form and rebar geometry appeared to be sized and placed in accordance with CEC plans.
• Footing dowels appeared to be#5 bars spaced at 16" O.C. with the specified hook size as on CEC plan.

addition, the existing steel beam at the northeast section of building"A"was looked at and will be analyzed
for load capacity.
1. It was observed that the forms and rebar placed for retaining wall sections were in accordance with CEC
plans with the exception of a few rebar locations not placed 2" clear from the inside of the wall form.
• All continuous horizontal rebar appeared to be#5 bars spaced 12"O.C. as specified.
• All vertical rebar appeared to be#5 bars spaced 12"O.C. as specified. .
• 2"Dia. PVC weeps appeared to be installed for drainage as specified.
• All rebar doweled into existing concrete walls appeared to be set using the specified epoxy.
• Rebar located in wall section "C"on SK-I measured 4 1/2"clear from inside of wall form, not 2"
clear as specified on.CEC plan.
• Rebar located in wall section"B"at angle only on SK-1 measured 7" clear from inside of wall form,
not 2"clear as specified on CEC plan.
• All form geometry appeared to be sized and placed properly as specified.
• It was also observed that a hole was dug under the footing of buiding"D"for a drain pipe to pass
through. CEC advised J.K. Scanlan to fill in with concrete when work has been completed.
2. Slab preparation at Building".D"was observed and was in accordance with CEC plans.
• Transverse bars in strip footings appeared to be#4 bars at 12" o.c., as specified.
• Continuous longitudinal bars in strip footings appeared to be 4-#4 bars equally spaced as.specified.
• Specified WWM appeared to be set with appropriate chairs and in accordance with CEC plans.

3. The existing steel beam at the northeast, I"floor of building"A"was inspected by CEC.
• The existing S8 steel beam is bearing 4-5"on the existing concrete foundation wall.
• It appears to have some rust and oxidation with an approximate 4"dia. hole cut out for pre-existing
plumbing or drainage. It is recommended by CEC to weld a steel plate covering the cut hole. It is also
recommended to weld anew C8 x 13.75 A36 steel channel onto the existing steel beam flange to add
reinforcement.

walkthrough for Buildings"A' "B';and"C' the following was observed:
Building"A"
Unit l
• . .The floor framing appeared to be framed in accordance with CEC plans and the approved shop drawings with the
following exception.The LPI-20 I joists,which frame into the LVL beam at the front of the building,used Simpson
HU412 hangers instead of the specified Simpson HU312 hangers.The Simpson HU412 hangers are too wide for the
I-joists and need to be removed and replaced with Simpson HU312 hangers.
• The Simpson LS90 angles were placed at the sill to the rim board,and the rim board to the wall above.
Unit 2
• A set of stairs was being built to the deck at the front side of the building. It was observed that no footings were
placed under the main support posts for the landing. CEC recommended to Jim that footings be placed to prevent
settling of the stairs.Jim informed CEC that it is his intent to place footings to support the stair landing.
Unit 3
• It was observed that 2 Simpson SDS screws were present where the valley meets the ridge. These valley beams
needed to have the supports located in the wall below to extend up to the underside of the valley beam. It could not be
observed if this was completed due to the drywall already being placed.As noted in Inspection Report 44 these areas
were to be left open. Scanlan shall submit a written document on company letterhead and submit to CEC to indicate
this work has been completed.
• It was observed that the ridge beam had no mechanical fastening to the support element. Connection shall be made
with either Simpson L or.Tbrackets,or Simpson A35 angles.
• The rafters that frame,into..the ridge beam did not have any hangers. Provide proper hangers as specified on sheet A-
S4..1 or Simpson.A35.,angles
Unit 4

side,had one side of the flange cut above the stairs to the basement.It appears that the flange may have been torch

• It was observed that the existing steel beam,which is shown on CEC plans scheduled to be cut to a cantilever on each
side,had one side of the flange cut above the stairs to the basement. It appears that the flange may have been torch
cut.
General
• CEC noted that the steel columns in the basement had concrete placed around the base.It appears that a bond breaker
was used at the base of these columns.
• The LVL beam in the fire room bears on top of the steel column and is attached to the steel column with 2 lag screws
and 2 hex head screws(refer to picture).Remove the hex head screws and replace with the appropriate size lag screw.
Building"B"
• A final walk through of Building`B"revealed no issues that need to be resolved.
Building"C"
• The plywood gussets for the roof trusses in Units 13-15 were observed.The gussets were 29"x20"with 3 rows of
nails approximately 4"o.c.
• A final walk through of Building"C"revealed no issues that need to be resolved.
Building"D"
• It was noted that the CMU walls already placed had steel wire from the top of the wall extending to the steel angles
and foundations on either side..

As a follow up to our November 14,2007,Building`B"Structural Wood Framing Punchlist field inspection report,
Coastal Engineering Company,Inc. (CEC)has revisited the site on Thursday,November 15,2007 to review the
completion of noted deficiencies found previously,as indicated within the attached report. The purpose of this follow
up inspection was to verify that the outstanding wood framing deficiencies have been resolved so that the contractor
may obtain the Building Department's release of building framing for application of insulation and interior finishes.
Based upon our November 15ffi follow up field visit,we hereby approve the wood framing corrective actions,overall
wood framing construction,and connections for Building`B: at the Tradewinds Condominium complex in
Barnstable,MA.Please be advised that this approval is only for the Building`B"framing and is not intended as the
substitute for the final controlled construction affidavit,which will be submitted at the end of the project once all
requirements have been satisfied.

3. In reviewing the rebar,CEC noted that there was some rebar placed into the ground which does not comply with ACI.
CEC informed Mike Breen about this and this was similarly—commented to Mike.by Alex Cleofas of Briggs.The rebar
that was noticed at the time that was placed into the ground was removed.Upon final check before leaving the site,
CEC noticed another rebar placed into the ground.Mike Breen was'infortned of this and told CEC to make a note of it;
rebar was still in the ground When CEC left the site.
4. CEC also noticed that there:was some rebar in the northwestfooting that was touching the side of the form. CEC
informed Mike Breen of this and that the rebars needed to have 2 inch clear cover from the side ofithe form.Tine same -
response was indicated as above`The rebar was still touching the side of the form when CEC`left the site.
The following was noted at stepped footing locations:
• Bent bars for the stepped footings that were shop bent was for a'1 foot step footing.The step;that were-forme-on
site were 2 foot stepped footings.:The ground started to slope approximately 3 feet before�hb step,fog wed by�.� ,

fairly accurate,though the bars were field bent and bends did not conform to ACI or CRSI standards for bar bends.
The bars on the upper side of the step had approximately 3"clear cover to the ground,the bars on the low side of
the step footing had approximately 3"clear cover to the ground,though bar bends were approximately 90 degrees.
• The step in the footing just south to the previous footing location,which was also designed as a haunch footing and
changed to a strip footing(CEC approve d th� a request before forming),had the following issues:
➢ The step was a 2 foot stepped footing and the bars were bent for 1 foot step footing.
The ground slope was greater then the 1 to 2 ratio specified by CEC and_as_required by ACI.
5. Upon reviewing-the-step-footings CCEC`informed-Mike Breen that the rebar was not as specified and that the rebar
should be fixed before pouring the concrete-Mike Breenen proceeded to start pouring the concrete after being informed
of this.

A field inspection was made upon the request of David Robins(JKS)to verify placement of rebar in accordance with CEC
drawing in the north half of Building"B"walls. CEC observed that the rebar was placed as noted below:
1. CEC observed that the geometry and thickness of the wall appeared to be in accordance with CEC drawings.
2. Horizontal bars in wall were placed at 16" OC in the center of the wall. CEC specified#5 bars at 12"OC in the center of
the wall.Upon informing David Robins of this,the shop drawings were checked and showed the horizontal bar spacing to
agree with CEC
3. Vertical dowels in wall were placed at 16"OC and were#5 bars in the center of the wall as specified by CEC. Lap length
of vertical dowels was hard to view so no determination was made on acceptability.
4. Corner bars in the walls appeared to be placed correctly on approximately 50%of the corner laps. Some corner laps were
correct on both sides,but a majority had at least one side where the minimum lap length was not met. In a few cases, lap'
lengths were not met on either side of the corner bars.
5. In all but the west wall(rear wall),the 2 #5 bars top and bottom were not observed to be present.
6. Dowels in Section C of CEC drawings B-S5 in the frost wall to the adjacent slabs were rot present. -
7.' CEC observed that the diagonal bars over the location,of the previously poured step footing were placed as.specified by
GEC's sketch issued to JKS on 6/5/07 at 10:00 a.m.
8. David Robins informed CEC that the wall fonns will be stripped and the rebar redone in its entirety,to meet CEC
specifications and to be re-inspected by CEC before concrete pouring.
9. The continuous shear key at the top of the footing was not continuous.Upon discussion With David Morand(CEC), it was
learned that David Robins informed David Morand the shear key was not originally placed and had to be saw cut.

A field inspection was made upon the request of David Robbins(JKS)to verify placement of rebar in accordance with CEC
drawings in the north half of Building"B"walls.CEC observed that the rebar was placed as noted below:
1. The low frost wall at the garage area appeared to have the correct bar spacing and size.It was also observed that the dowels
for the apron slab and garage slab were tied to the wall rebar and that the hooks were parallel with the wall.These dowels
are to 4be bent perpendicular to the wall after the forms are stripped.CEC discussed this with David Robbins,before visiting
the site,and approved this method.
2. All the other walls on the north half of building"B"were observed to have horizontal rebar at approximately 12"O.C. in
the wall and appeared to be#5 bars as specified by CEC.CEC noted that there were no double top bars,instead a single top
bar in the middle,of the wall.This was discussed prior to.visiting the site,and CEC approved the change to ease
construction.
3. Vertical bars were observed to be approximately 16"O.C.and appeared to be#5 bars as specified by CEC.It was also
noted that the dowel lap from the footing dowel into the wall was approximately 2-1/2 ft.—3 ft. lap as specified by CEC.
4. Corner bars appeared.to have in required lap length and appeared td.be#5 bars has specified:

transverse bars were placed at 16"OC and were#4 bars. CEC called for.these bars to be placed at 18"OC. Since the
spacing was closer than specified,the spacing is acceptable..The continuous longitudinal bars were evenly spaced across
the width of the footing and were 4-#4 bars,3"clear from the bottom of the footing, as specified by CEC. Lap lengths for
these bars were at least the specified 40-bar diameter called for by CEC.
3. Corner bars in the footing appeared to be placed correctly. One corner bar lap length was insufficient,and this was
remedied before CEC left the site.
4. David Robins discussed with CEC-that they were on top of compaction and concrete testing with Briggs. It was discussed,
per phone conversation with David Morand(CEC),that certification for compaction under footings was to be provided to
CEC for review and,if CEC does not receive certification,then foundation in areas where no certification can be verified
may be ripped out.

CEC drawings.

3. CEC noticed that the stepped footings where incorrect with regard to the slope of rebar. The observed slope appeared to be
approximately 1:1,according to design specification the slope for the stepped footing should be 2:1. CEC requested for JKS to
use pour stops in these areas and refrain from placing concrete until the rebar slope is remedied.
4. Rebar in the frost wall at the garage location of Building"B"was noted to have 2#5 bars tied to vertical bars placed atop the
poured footing: The bars were loosely.placed'and could easily be moved`by hand. These bars should be tied in place and secured.
CEC observed vertical dowels at approximately 16"OC and a horizontal bar at.12"OC. Since David Robbins was not on site .
during the inspection of the frost wall at Building"B"he was informed of the deficiencies via phone conversation at 9:30am on
6/15/07.
5. CEC noted that the formed keyways in all poured footings appear to be inconsistent and left with loose concrete and rough edges.
Also it was observed that other material including rebar cut-offs where left atop footings. Formed key ways should be free from
all foreign debree prior to continuing with additional pours.
6.. CEC observed JKS placing concrete for the footing in Building"C"before leaving the site.

observed that the rebar was placed as noted below: l
1. It was observed that the stepped footings that had the incorrect slope and rebar bends in building_"C"were poured(see field, ti
inspection report#5).Per later discussion with David Morand(CEC),it was learned-that placement of those footings were
approved.
2. The rebar placement and the geometry of the rear frost wall at building"B"appeared to comply with CEC drawings.
3. The stepped footing at the south side of building"C"appeared to be in accordance with CEC contract drawings.The
following exception was noted.CEC calls for the footing reinforcement at the bottom of the top stepped footing to extend
30"bar diameters beyond the beginning of the slope for the bottom of the'footing.It was noted that these were bent and tied
to the sloped bars for the stepped footing.JKS was informed that this was acceptable,but it should be corrected for the
balance of the stepped footings.
4. The stepped footing at the north side of building"C"appeared to be in accordance with CEC drawings.
5. David Robbins asked CEC about a detail for the retaining wall between building"C"and"D".CEC informed JKS that they
will have to check into that matter back at the office since that would be an additional service that the owner needs to
authorize.

A field,inspection was'made upon the request of David Robins(JKS)to verify placement of rebar in accordance with CEC
drawing in the north half of Bum ild gT'.13-walls.CEC observed that the rebar was placed as noted below:
1: CEC observed_that the geometry and thickness of the wall appeared to be in accordance'with CEC drawings.
2..:;Horizontal bafs.in wall':wer'e placed at 16"OC in the center of the wall. CEC'specified#5 bars at 12"OC in the center of
the wall.Upon informing David Robins of this,the shop drawings were checked and showed the horizontal bar spacing to
agree with CEC.
3: _ Vertical dowels in wall were placed at 16"OC and were#5 bars in the center of the wall as specified by CEC. Lap length
of vertical dowels was hard to view so no determination was made on acceptability.
4. Corner bars in the walls appeared to be placed correctly,on approximately 50%of the corner laps. Some comer laps were
correct on both sides,but a majority had at least one side where the minimum lap length was not met. Ina few cases,lap
lengths were not met on either side of the corner bars.
5. In all but the west wall(rear wall),the 2-#5 bars top and bottom were not observed to be present.
6. Dowels in'Section C of CEC drawings B-S5 in the frost wall to the adjacent slabs were not present. _
7. CEC observed that the diagonal bars over the location of the.previously poured-step footing were placed as specified by
CEC's sketch issued to JKS on'6/5/07 at 10:00 a.m.
8. David Robins informed CEC that the wall forms will be stripped and the rebar redone in its e 1 tirety toDgeet CEC
specifications and to be re-inspected by CEC before concrete pouring.
9.. The continuous shear key at the top of the footing.was not continuous.Upon discussion with David wand CEC),ait was

A field inspection was made at the request of Dave Robbins(JKS)to verify all framing conditions for`Buil� din—' B".
• It was observed that there were inadequacies with the gable bracing at each end of Building"B"..The mid-height
gable brace is applied to the sheathing and gaps are present.Also,the gable brace is not nested in the 2x6 shoe.The
bottom 2x4 truss chord braces are split and have no resolve at the gable end.These braces are piggybacked
approximately 2"and show only 1-2 nails at connections.The nailing penetration overall is poor,with a.few missed
connections. It was,also observed that there was no continuous horizontal bracing of the truss webs which should be
resolved for continuity.
• Missing posts under LVL's-Provide double 2x4 wall studs at these locations with mid-height bridging(typical).
• All post to beams at stairs require"T"or"L"Series Simpson Strong Ties(typical).
• Continuity in floor joist bridging is required to be placed.
• Exterior 4x6 post connectors are required(Simpson A23 Z-Max),used with stainless steel fasteners).
• Hurricane ties are required at roof framing layovers.
• Uplift straps were missing along some of the second floor sill areas along the east elevation.
• Two suspect shear connectors which attach the wall sills were removed to verify their penetration.Penetration was
found to be adequate at these areas.

As a follow up to our November 14,2007,Building"B".Structural Wood Framing Punchlist field inspection report,
Coastal Engineering Company, Inc.(CEC)has revisited the site on Thursday,November 15, 2007 to review the
completion of noted deficiencies found previously,as indicated withvn the attached report. The purpose of this follow
up inspection was to verify that the outstanding wood framing deficiencies have been resolved so that the contractor
may obtain the Building Department's release of building framing for application of insulation and interior finishes.
Based upon our November 15d'follow up field visit,we hereby approve the wood framing corrective actions,overall
wood framing construction,and connections for Building"B: at the Tradewinds Condominium complex in
Barnstable,MA.Please be advised that this approval is only for the Building"B"framing and is not intended as the
substitute for the final controlled construction affidavit, which will be submitted at the end of the projeci once all
reqtiirements have been satisfied_

As you are aware,Coastal Engtineering Company, hic. (CEC)has performed the Building"C" Framing Punchlist
Report No. 33 on"Tuesday,November 27,2007(report attached).CEC has perfon-red a follow up field inspection on
November 30,2007,to verify that the outstanding punch-list items have been resolved per our initial punch-list
report.Additionally,we are in receipt of the updated sealed truss design doettments,forwarded by David Robbins of
Scanlan,which indicate:that the existing roof trusses may support the weight and placement of the iIVAC units upon
the bottom chords,in lieu of the top chords.
As such,we hereby approve the final wood and structural steel framing construction associated with Building"C"for .
the Trade Winds Condominiums project.Please note that his letter represents the framing approval only,and is not
intended for the final structural affidavit sir-off of the project.

As requested, please find information pursuant to the installation.of the toilet exhaust system
at the above-referenced project.
The ductwork systems from separate toilet rooms may be combined at the point of terminatio
at the exterior of the building if the following items are provided as specified:
The toilet exhaust fans are approved and meet specifications, Schedule Dwg_ DA2.1
• Each fan is provided with a back draft damper
• All ductwork shall be sealed air tight
• The termination at the exterior of the building shall be- sized to accommodate the total
CFM.
With regard to the exhaust ductwork, length of run, design criteria, exhaust fens for toilets
labeled EF-1 were specified to exhaust the required cubic feet per minute of air @ 14" static
pressure per 100 ft. of equivalent run which is more than adequate for this project, refer to
Dwg. M2.1 for fan requirements.
Please Note: Shop drawing review was performed 9/27/07 and 10/19/07.
Some equipment was requested to be resubmitted and further information to_be provided for
some items. To date, I have seen "nothing." Four (4) copies have been returned to the
architect on the above dates for your use.
Please review your files and provide information requested.
