The truck was empty by the time the results were obtained. I notified Dave with J.K. Scanlan as well as C.C.R.M. dispatch. I was informed by C.C.R,M, that a reduction in the air entrainment would be made on the next load (No. 5). Tests were conducted on this load to insure adequate adjustment of air entrainment. Rebar in the short wall,was all in place and appeared to be of the correct spacing, size, and geometry. b. Rebar for the tall wall (this wall was removed due to means and methods issues)was about 50 percent complete. JKS informed CEC that they were waiting to receive the remaining rebar. c. All rebar that was in place appeared to be of the correct size, spacing, and geometry. d. All bars appeared to be#5, with spacing of 12"O.C. vertical and 16"O.C. horizontal. e. JKS requested to take pictures of finished rebar and forms and submit them to CEC so they may pour concrete on 9/14/07. Unit 2 • A set of stairs was being built to the deck at the front side of the building. It was observed that no footings were placed under the main support posts for the landing. CEC recommended to Jim that footings be placed to prevent settling of the stairs.Jim informed CEC that it is his intent to place footings to support the stair landing. Unit 3 • It was observed that 2 Simpson SDS screws were present where the valley meets the ridge. These valley beams needed to have the supports located in the wall below to extend up to the underside of the valley beam. It could not be observed if this was completed due to the drywall already being placed.As noted in Inspection Report 44 these areas were to be left open. Scanlan shall submit a written document on company letterhead and submit to CEC to indicate this work has been completed. • It was observed that the ridge beam had no mechanical fastening to the support element. Connection shall be made with either Simpson L or.Tbrackets,or Simpson A35 angles. • The rafters that frame,into..the ridge beam did not have any hangers. Dowels in Section C of CEC drawings B-S5 in the frost wall to the adjacent slabs were rot present. - 7.' CEC observed that the diagonal bars over the location,of the previously poured step footing were placed as.specified by GEC's sketch issued to JKS on 6/5/07 at 10:00 a.m. 8. Rebar in the frost wall at the garage location of Building"B"was noted to have 2#5 bars tied to vertical bars placed atop the poured footing: The bars were loosely.placed'and could easily be moved`by hand. These bars should be tied in place and secured. CEC observed vertical dowels at approximately 16"OC and a horizontal bar at.12"OC. Since David Robbins was not on site . during the inspection of the frost wall at Building"B"he was informed of the deficiencies via phone conversation at 9:30am on 6/15/07. 5. CEC noted that the formed keyways in all poured footings appear to be inconsistent and left with loose concrete and rough edges. Scanlan Co.,Inc.(JKS) ' 4 Steve, Shawn Lee Construction A field inspection was made at the request of Dave Robbins(JKS)to verify all framing conditions for`Buil� din—' B". • It was observed that there were inadequacies with the gable bracing at each end of Building"B"..The mid-height gable brace is applied to the sheathing and gaps are present.Also,the gable brace is not nested in the 2x6 shoe.The bottom 2x4 truss chord braces are split and have no resolve at the gable end.These braces are piggybacked approximately 2"and show only 1-2 nails at connections.The nailing penetration overall is poor,with a.few missed connections. It was,also observed that there was no continuous horizontal bracing of the truss webs which should be resolved for continuity. • Missing posts under LVL's-Provide double 2x4 wall studs at these locations with mid-height bridging(typical). • All post to beams at stairs require"T"or"L"Series Simpson Strong Ties(typical). • Continuity in floor joist bridging is required to be placed. • Exterior 4x6 post connectors are required(Simpson A23 Z-Max),used with stainless steel fasteners). • Hurricane ties are required at roof framing layovers. • Uplift straps were missing along some of the second floor sill areas along the east elevation. • Two suspect shear connectors which attach the wall sills were removed to verify their penetration.Penetration was found to be adequate at these areas. The ductwork systems from separate toilet rooms may be combined at the point of terminatio at the exterior of the building if the following items are provided as specified: The toilet exhaust fans are approved and meet specifications, Schedule Dwg_ DA2.1 • Each fan is provided with a back draft damper • All ductwork shall be sealed air tight • The termination at the exterior of the building shall be- sized to accommodate the total CFM. With regard to the exhaust ductwork, length of run, design criteria, exhaust fens for toilets labeled EF-1 were specified to exhaust the required cubic feet per minute of air @ 14" static pressure per 100 ft. of equivalent run which is more than adequate for this project, refer to Dwg. M2.1 for fan requirements. Please Note: Shop drawing review was performed 9/27/07 and 10/19/07. Some equipment was requested to be resubmitted and further information to_be provided for some items.
