Paton: On February 12, 2004, 1 visited the site to review the progress of construction. I met with Mr. David Nolan,the superintendent with DMR Construction. The following items were noted: 1. All structural steel framing has been completed. Structural K-bracing has been installed per the structural drawings. 2. It was noted that the interior footing for column C2 was installed approximately 8 inches off center, therefore the column is eccentrically loaded on the footing. This footing was, formed 6 feet square instead of 5 feet square therefore the eccentricity does not present a structural concern. 3. Light gage framing was in progress, with approximately two-thirds of the exterior wall studs in place. The deflection head detail was not installed per the structural drawings. Due to the lower building height and smaller snow loading for this store, I am not going to require that the deflection bead be installed. However, the contractor should be aware that the detail will be required in the future. -4, Work completed to date looks good and is in geneTal compliance with the construction drawings. 5. Roof trusses are on site and ready to be installed. I will perform my final site observation when the trusses have been installed, braced,and hurricane tie downs are in place. All perimeter footings have been poured. The entire perimeter foundation wall was scheduled to be poured today. The wall pour was estimated at 65 cubic yards of concrete. The first concrete truck was on site during my observations, 2. All of the formwork was in place, The reinforcing cages at the perimeter piers looked good, The vertical dowels from the continuous footing into the wall have been installed. 3. One of the continuous #5 perimeter wall reinforcing bars had not been installed. This bar will be"wet stuck"in the top of the wall after the concrete has been placed. 4. Comer bars were on site but had not been installed. 5. 1 met with the technician from UTS, who was on-site casting cylinders. The 4000-psi mix concrete was slumping 4 1/4 inches and has a polar.set admixture. I asked thi technician to verify that the missing Wall reinforcing was placed. 6'. The temperature at the time of the pour was about'35 degroe's Fahrenheit.' 'I askdd the Superintendent what measures he was taking to prevent the concrete from freezing. He told me that the entire wall was to be draped with thermal blankets after the concrete has been placed. The weather is supposed to remain fairly constant until Wednesday, January 14, 2004,when temperatures are expected to drop dramatically. Hurricane clips and permanent truss bracing shall be installed and inspected prior to any work being concealed by other trades or materials. The general layout conforms to the contract documents. The truss manufacturer has visited the site to make general observations. All work observed and completed thus far appears to conform to the contract documents and project specifications. The space between the suspended acoustical ceiling and the bottom chord of the wood roof truss system is not required to be sprinklered per NFPA l 3 Section 4-13.1.1,Exception No. 8, We have provided 5/8"thick Type"X"GWB fire taped on the ceiling side of the bottom chord of the roof trusses. All exposed wood will be encapsulated in the same manner. This will e'fffcetively isolate the combustible roof construction from the concealed above ceiling space. The attic space above the GWB is 100%sprinklered as well as the below ceiling spaces. Materials within this concealed space are of a noncombustible type and consist primarily of I•IVAC duct work, sprinkler and water piping. The ceiling system is a Class A tile with a flame spread rating of less than 25. We feel that the intent of the code requires sprinklers or draftstopping where combustible floor or roof construction is exposed to the concealed space. We feel that we meet the intent of the code by isolating all combustibles above the ceiling membrane that could propagate the spread of fire. .All smaller spaces within the building will have full heighl walls with.GWB continuous to the'underside of the roof truss on at least one side of the wall and thus acting as draftstops between spaces. No spaces exceed 1,000 sf except for the retail area. McCarthy, Geotechnical Dept. Mgr. PURPOSE: A site visit was made to the referenced project to review the earthwork construction. On December 19 the contractor was observed stripping the site of the organic laden surface soils to expose the underlying inorganic soils. The Geotechnical Report and associated Soil Boring Logs prepared by William S. Zoino dated October 2003 were reviewed as pertaining to-this visit. The footings are designed to gain bearing support from atop the parent inorganic soils. As such, the contractor is to fully remove the organic laden soils from within the building pad area and any remnant foundations,questionable fill and/or foreign matter. The footings(perimeter, interior,associated structures)should gain bearing support atop the parent soils and/or compacted structural fill. As such,the existing fill, buried organics,and other questionable/foreign matter should be fully removed from the Footing Zone of Influence. The Footing Zone of Influence is defined as that area extending laterally outward and downward at a 1 H:1 V splay from the edge(s)of the footing. Excavations that extend below the design foundation grade are to be replaced with compacted structural fill. Structural Fill below foundations should be placed in loose lifts not exceeding 12 inches for heavy vibratory rollers and 8 inches for vibratory plate compactors. All Structural Fill should be compacted to at least 95%of maximum dry density as determined by the Modified Proctor Test (ASTM-D 1557). The adequacy of the compaction efforts should be verified by field density testing. A UTS field representative has been assigned to the project to review the subgrade conditions and preparations. A soil sample extracted from the footing subgrade was transported to our Laboratory for Gradation Analysis and Modified Proctor Testing. This data is enclosed for review. Partially thawed material between trench and line E was excavated, broken up with the excavator bucket and tracks. Structural fill was then compacted with the vibratory roller. Onsite sandy material was being placed in 1.0' lifts along F line wall. Each lift was compacted with the plate compactor against the wall and with the vibratory roller 3 feet in from wall. Material was being excavated below 6' in retention pond. A coarse sand with little gravel was excavated and used for backfill. A sample was obtained and transported to the UTS lab for sieve and proctor analysis. Five field density tests were performed according to ASTM D2922 using the Troxler density gauge. Tests 1, 2 and 5 exceeded 95% of the modified proctor ASTM D1557. Tests 3 and 4 will be included pending completion of lab sieve and proctor testing. DATE: February 2, 2004 RE: FOUNDATION SUBGRADE Project No. 8634 Site visits were made to the referenced project to collect soil samples,perform dynamic cone penetrometer testing,and review foundation subgrade conditions. A test pit was excavated in the proposed detention pond for purpose of soil sampling. The test pit extended approximately 6 feet below existing grade, or design grade. The soil is proposed for reuse as structural fill. The material is a sandy gravel suitable for use as structural fill. The contractor was informed to segregate the upper 18 to 24 inches of organic laden soils from the sandy gravel. A soil sample was also collected at design grade elevation for gradation analysis. The laboratory results have been submitted under separate cover. The structural fill is being reworked as foundation backfill. Prior to backfilling the contractor is thawing the exposed subgrades that were frost penetrated. Other subgrades have been protected with hay and thermal insulation blankets. Thawed subgrades are being re-compacted and examined by UTS prior to backfilling. The contractor appears to have employed a satisfactory frost protection plan,and remedial action to progress with construction. It should be noted that crushed stone has also been in use at the project site,which should provide additional frost embedment protection. Several feet of backfill have been placed within the building pad. As such,the bearing subgrade was evaluated via dynamic cone penetrometer testing. In summary,the penetrometer (ASTM Special Publication #399; Dynamic Cone for Shallow In-Situ Penetration Testing) is a portable, hand operated steel assembly in which a 154 weight free falling 20 inches advances a conical driving head upon impact into the substrate. The density, measured by resistance, is recorded in blow counts per 1.75"increments. The blow counts within the soil structure underlying the crushed stone base averaged 8 to 15 strikes per 1.75" increments to a depth of 3'-8"below finish slab grade. These blow counts indicate a compact dense mass of soil structure. UTS remains on-site to monitor the subgrade preparations and perform compaction testing on prepared subgrades. The visual inspection of the field bolted connections revealed all connections tensioned- -- - and acceptable. 3. The visual inspection of the field welded brace connectionsbetween the first floor and roof revealed all welds complete and acceptable; reference detail 10 on the S3.1 dwg. 4. The visual inspection of the cantilever moment connections at the A-1 column lines re- vealed unacceptable top and bottom flange welds. No backing material was used to develop the full penetration requirement; reference detail 5 on the E-2 drawing. S.
