Information and Instructions
Massachusetts General Laws chapter 152 section 25 requires all employers to provide workers' compensation for their
employees. As quoted from the "law", an employee is defined as every person in the service of another under any contras-
of hire, express or implied, oral or written.
An employer is defined as an individual, partnership, association, corporation or other legal entity, or any two or more of
the foregoing engaged in a joint enterprise, and including the legal representatives of a deceased employer, or the receiver c
trustee of an individual ,partnership, association or other legal entity, employing employees. However the owner of a
dwelling house having not more than three apartments and who resides therein, or the occupant of the dwelling house of
another who employs persons to do maintenance , construction or repair work on such dwelling house or on the grounds or
building appurtenant thereto shall not because of such employment be deemed to be an employer.
MGL chapter 152 section 25 also states that every state or local licensing agency shall withhold the issuance or renew.

Please fill in the workers' compensation affidavit completely, by checking the box that applies to your situation and
supplying company names, address and phone numbers along with a certificate of insurance as all affidavits may be
submitted to the Department of Industrial Accidents for confirmation of insurance coverage. Also be sure to sign and
date the affidavit. The affidavit should be returned to the city or town that the application for the permit or license is
being requested, not the Department of Industrial Accidents. Should you have any questions regarding the "law"or if you
are required to obtain a workers' compensation policy, please call the Department at the number listed below.

Please be advised that I have examined the record title at the Barnstable Registry of Deeds
regarding the above captioned premises which were conveyed to Lawrence J. Salters by Deed
dated May 16, 1998, recorded Book 11440, Page 246 (copy herewith). The subject premises
formerly were shown as Lot 7 and Lot 6 on a plan dated 1898 recorded at Plan Book 31, Page 43
(copy herewith). In 1976 the two lots were owned by Eleanor Haggerty which she acquired by
deed dated December 16, 1970, recorded Book 1597, Page 181 (copy herewith).
In 1976 Eleanor Haggerty subdivided the subject lots into Lots 6A and 7A which plan is
recorded in Plan Book 302, Page 73 (copy herewith). By deed dated April 1, 1976 Eleanor
Haggerty conveyed Lot 6A to Robert F. and Phyllis Bettle, said deed recorded Book 2322, Page
234 (copy herewith). She therefore owned Lot 7A separately as of April 1, 1976.
Thereafter Eleanor Haggerty deeded Lot 7A by deed dated May 21, 1982 recorded Book
3487, Page 198 to Francis G. Haggerty. From then until the deed to Lawrence J. Salters, Francis
G. Haggerty and his wife Natalie owned the subject lot and no other adjacent land.
