A person who constrncts.more than one home in a two-year period shall not be considered a homeowner. Such "homeowner"shall submit to the Building Official on a form acceptable to the Building Official, that he/she shall be resuonsible for all such work performed under the building permit. (Section 109.1.1) The undersigned"homeowner"assumes responsibility for compliance with the State Building Code and other applicable codes, bylaws, rules and regulations. The undersigned"homeowner"certifies that he/she understands the Town of Barnstable Building Department minimum inspection prose ores and requirements and that he/she will comply with said procedures and requir Signs of er Approval of Building Official Note: Three-family dwellings containing 35,000 cubic feet or larger will be required to comply With the State Building Code Section 127.0 Construction Control, HOMEOWNER'S EXEMPTION The Code states that: "Any homeowner perfformin$work for which a building permit is required shall be exempt from the provisions of this section(Section 109.1.1-Licensing of construction Supervisors);provided that if the homeowner engages a persons)for hire to do such work, that such Homeowner shall act as supervisor." Many homeowners who use this exemption are unaware that they are assurning the responsibilities of a supervisor(see Appendix Q, Rules&Regulations for Licensing Construction Supervisors,Section 2.15) This lack of awareness often results in serious problems, particularly wbeq the homeowner hires unlicensed persons. In this case, our Board-cannot proceed against the unlicensed person as it would with a licensed Supervisor. The homeowner acting as Supervisor is ultimately responsible. To ensure that the homeowner is fully aware of his/her responsibilities, many communities require,as part of the permit application, that the homeowner certify that he/she understands the responsibilities of a Supervisor. On the last page of this issue is aform currently used by several towns. you may care t amend and adopt such a formlcertification for use in your community. 1 ne,t ommonweatrn of massacnusens Department of hidustrial Accidents Office of Investigations ' 600 Washington Street Boston,MA 02111 www massgov/dia Workers' Compensation Insurance Affidavit: Builders/Contractors/Electricians/Plumbers Auulicant Information Please Print Legibly Name (Business/Org nizationa ividual)'_ DO y J AC-oa 5 Address: GJ M"/)O(-i F1912 a l /Z 0 City/State/Zip: -- CfAv;259 Vic.[e /w Phone#: $ •7 t 3 q e/- a Are you an employer? NOVUS AESTHETICS qr g • A beautiful, random flame dances around an arrangement of ceramic fiber logs with hand-painted detailing of split and charred areas. Glowing embers and smoldering ashes complete the took for a . very realistic wood-like fire. • Available with a standing pilot or electronic ignition start-up system using natural or L.P.gas;three l different opening sizes are also available providing a variety of choices. •"A choice of two optional upgrade features that may be installed at the factory prior to shipment: refractory lining and a fan. • 27,000 Btu/hr.input for natural gas and 25,000 Btu/hr.input for L.P.gas.for 36"models;25,000 Btu/hr input for 33"models;24,000 Btu/hr.input for 30"models. INSTALLATION AND MAINTENANCE • Entire burner and ignition controls are one platform piece that easily lifts out to provide easy € adjustments, service and future upgrades. • A 1911s"depth allows installation with the use of minimal framing materials, lowering construction costs. • Top direct vent allows installation along interior or exterior walls, below grade,or in corners. • Sealed chamber uses only outside air for combustion, helping to preserve indoor air quality and prevent , negative air pressure problems. • A flexible stainless steel gas connector, manual shutoff valve and gas knockouts provide easy installation. SAFETY STANDARDS 1, • Thermopile-thermocouple standing pilot is an industry-leading safety application. • Tempered glass with a low visibility safety fire screen has been designed with a quick access system M1 to enable removal for cleaning in seconds, without the use of tools.Glass also has a protective edge to make handling easier and safer. • A high limit switch is a built-in,standard feature on all NOVUS appliances. • Spring-loaded expansion relief door relieves any pressure that may build up inside the firebox. ' • Approved for installation in bedrooms and mobile homes. • Tested and Listed to UL Safety Standards. • One-year limited warranty. z ' 1 1 Mode!
