Property is developed with a 1%story single family dwelling(2004)containing 3 bedrooms and 2 1/2
baths on 1 acre located in the RF district. ,

RFS submitted by Cotuit Fire Dept. An elderly woman and adult son reside here. The smoke alarms went
off and the son was unable to disable them so he shut off the power at the panel.This triggered the
battery response from the detectors. A Verizon utility worker in the area heard the commotion and
notified the Cotuit FD. Police and Health responded as well. Staff noted the house had no heat and was
35 degrees inside.The elderly woman was found in bed wearing a very thin nightgown.The son was
noted to be inebriated and removed from the site by BPD. The woman was transported to CCH.
Gene ordered the power disconnected as the source for the alarm was not identified. BPD noted an
odor of smoke on the second floor and found that the son had lit a candle. It had burned down but was
discovered before it ignited a plastic tray.
Health condemned the property as there is no heat or power to the house. The owner's other son was
notified and advised to obtain the services of an electrician and permits to restore the power. The
property is said to be for sale.

COMPLIANCE STATEMENT: The proposed building design described here is
consistent with the building plans, specifications, and other calculations
submitted with the permit application. The proposed building has been
designed to meet the requirements of the Massachusetts Energy Code.
The heating load for this building, and the cooling load if appropriate,
has been determined using the applicable Standard Design Conditions found

AIR LEAKAGE:
( ] Joints, penetrations,' and all other such openings in the building
envelope that are sources of air leakage must be'sealed. When
installed in the building envelope, recessed lighting fixtures
shall meet one of the following requirements:
1. Type IC rated, manufactured with no penetrations between the
inside of the recessed fixture and ceiling cavity and sealed or,
gasketed to prevent air leakage into the unconditioned space.
2. Type IC rated, in accordance with Standard ASTM E 283, with no
more than 2.0 cfm (0.944 L/s) air movement from the the
conditioned space to the ceiling cavity. The lighting fixture -
shall have been tested at 75 PA or 1.57`lbs/ft2 pressure
difference and shall be labeled.

MATERIALS IDENTIFICATION:

Dear Sir:
This office represents Carol Ball Ryan, Trustee of Ryan Nominee Trust,the
current owner of the above referenced property, who is seeking to build a single family
residence on the property. The purpose of this letter is inform your office as to when the
subject property was last held in common ownership with any adjoining lots.
The subject lot was legally created on an ANR plan approved by the Planning
Board on March 7, 1983. Said plan was recorded in the Barnstable Registry of Deeds in
Plan Book 373 Page 26. The subject lot is shown as Lot 6 on said plan. The lot contains
43,560 square feet and has 150 feet of frontage.
The subject lot has two adjoining parcels: Parcel 9-8 currently owned by Anthony
J. and Janet G. Cincotta and Parcel 10 currently owned by Sumner and Carol A.
Kaufman.
The last time the subject parcel was held in common ownership with either of the
adjoining parcels was June 2, 1983 when the subject property was conveyed into separate
ownership. At the time the parcel was conveyed into separate ownership it conformed to
then existing zoning and at all times thereafter the lot has been separately owned.
