4 , . Dear Mr. Wisentaner:
On Friday, August 31 , 1990, Donna Miorandi, Health Inspector
for the Town of Barnstable, performed an on-site inspection
with Lt. Eric Hubler of the Hyannis Fire Department and Mr.
Richard Bearse of the Town of Barnstable Building Department at
the property leased by you d/b/a Hyannis Fabrications and
located at 59 Ferndoc Street, Hyannis .
While on site the following violations were observed:
ARTICLE XXXIX. CONTROL OF TOXIC AND HAZARDOUS MATERIALS.
Section 5 (c) . The Board of Health requires that containers of
toxic or hazardous materials be stored on an impervious,
chemical resistant surface compatible with the material being
stored, and that provisions be made to contain the product in
the case of accidental spillage.
MASSACHUSETTS HAZARDOUS WASTE REGULATION: 310 CMR 30 . 680,
30 . 690. Each container and tank must be clearly and visibly
labelled throughout the period of accumulation with the
following:
-the words "HAZARDOUS WASTE"
-the name of the waste (e.g. , waste oil, acetone)
-the type of hazard (s) (e.g. , ignitable, toxic, dangerous
when wet, corrosive)
-date on which the accumulation began .
Each container must be in good condition.
Wastes containing toxic or hazardous materials shall be removed
and disposed of in accordance with the Massachusetts Waste
Management Act. Ch . 704 of the Acts of 1979 .

Under the Massachusetts Right-to-Know Law employers must
provide workers with information about hazardous substances
contained in Material Safety Data Sheets (MSDSs) . Employers
must give municipal coordinators copies of all MSDSs . The
Health Department is in receipt of the following MSDSs :
acetone, balsa wood, hi-temp paste wax but several others
including Azrez Resin Solution, Co-Plas, Inc. , Polylite
Polyester Resin, and Metal Treat have not been supplied to this
department.
There is also spray painting being performed on site without
any spray booth.
This building is in a location known as the Well Protection
Overlay District of the Zone of Contribution. The area is
critical to our public supply wells for the Town of
Barnstable's drinking water. You are reminded that this use is
a prohibited use under the Town of Barnstable's Zoning By-Law.
You are directed to correct these violations within fifteen
( 15) working days of receipt of this notice.
Please be advised that failure to comply with an order could
result in a fine of not more than $200 . Each separate day's
failure to comply with an order shall constitute a separate
violation.

Dear Customer:
Ashfand Chemical Company, through a computerized system, provides its customers with a material safety
data sheet the first time a product is purchased. If the material safety data sheet is changed, a copy of the
revised sheet is sent to customers who purchased the product in the previous twelve months. If a customer
has not purchased a particular product for, twelve months and then purchases it again, another copy of the
matcrial safety data sheet is sent.
The material safety data sheet is addressed to the attention of the PLANT MANAGER/SAFETY DIRECTOR
and is mailed to the invoice address and the ship to address when that is different. You should direct the
material safety data sheet to those responsible for managing or designing operations involving the use of the
product and to those who use or handle the product and may potentially be exposed to it. If you would like
the MSDS to be sent to an additional location, please address a request to Ashland Chemical at the address
above. Please include a copy of page one of the MSDS with the request.
If you distribute or resell Ashland Chemical products, you may have an obligation under the law to forward
the information contained on Ashlands MSDS to your purchasing customers. If you distribute or sell Ashland
products in Ashland factory packed drums, you may accomplish this by forwarding Ashlands MSDS to your.
customers purchasing such products. However, if you blend or repackage Ashland products before distribution
or resale, you should use Ashlands MSDS only as a resource in preparing your own MSDS to send to your
customers. In case of a blend, your MSDS must take into consideration hazards associated with each com-
ponent of the blend. Ashlands MSDS will serve as a valuable resource for those component parts supplied
by Ashland. Note that if your product is packaged for distribution/resale on the' consumer market, you should
consult the Consumer Product Safety Act and all other applicable laws or regulations as additional warning
requirements may apply.
