This status requires that the following actions be taken: The business is to complete and mail in the attached "Application for Permit to Store and/or Utilize More Than I I I Gallons of Hazardous Materials". The license/permit is to be posted in the building upon receipt. The Hazardous Waste Generator Identification Number MV5087755030 is not registered/listed with the Massachusetts Department of Environmental Protection(DEP). An appropriate Hazardous Waste Generator Identification Number is to be obtained by you from the DEP. Please be aware that, Hazardous Waste Generator Identification Numbers are site specific, in the event of a move, a business needs to document the end of activity at its former location and re-register with DEP for the new location. (The DEP Business Compliance Division Hazardous Waste Line telephone number is 617-292- 5898.) The saturated absorbent pads and free floating liquid in the Oil Room"pit" is to be removed and disposed of by a licensed waste hauler. For your convenience, attached is a copy of a plan, dated December 12, 1994 indicating that the Oil Room "pit" is constructed of eight inch thick, reinforced concrete walls and floor, and has had two applications of epoxy coating. BALISE FORD January 10, 2012 Corrective Actions Safety Kleen was"on site today to clean the Oil Room pit. A Clor-d-test was performed to determine that the oil was not contaminated. The saturated absorbent pads from the bottom of the pit were placed in a metal drum for later manifesting off site. The remaining oil in the pit was flushed with water, sucked up and pumped into the tanker truck. The pit was then treated with a non-hazardous degreaser, flushed with water, and again the fluid was pumped into the tanker truck. The oily waste water will be delivered to Environmental Compliance Corporation, Stoughton, Massachusetts for treatment. Once cleaned, the pit will be relined with absorbent pads to prevent the build up of free flowing oil.
