` We received a call reporting an oil spill behind the Baybridge Club 278 Main St.
We responded with E-823- Kleamenakis, Nagorka, McCarthy, and Kristofferson.
Upon our arrival we investigated and found a drum with frialator oil behind the Sunnyside restaurant was
iipp.ed over and ran down the hill behind Baybridge and into a storm drain. The owner of Sunnyside states
the drum was about a third of the way full which equates to about 15-20 gallons. The spread was about 30
i:t long and 8 ft wide.
We used speedy dry to dike the drain was also confined the spill with speedy dry and absorbent pads.

The Board of'Healtli was called and Don Demurest responded.
f.One client of Baybridge said he thought he knew who dumped the drum, but the director was unable to

also is making the restaurant owner secure the barrel so this won't happen again. ;
Eventually it )vas decided to get the DPW to sand the area. The would then sweep up the sand and pump the

Dear Ms. Wingard:
At your request, on March 21s`,we investigated a potential indoor air quality problem at
the Baybridge facility, 278 Main Street,Hyannis, MA. The purpose of this investigation was to
determine if the ambient air inside the facility contained mold or any other unusual contaminants
that could potentially lead to employee health problems.
The following Executive Summary provides an overview of our findings. The
remainder of the report provides details regarding the; Tests and Testing Conditions,Findings,
Conclusions and Recommendations.
Executive Summary
❖ The building is a sieve to rainwater and groundwater intrusion. This is an open
invitation to mold growth and structural rot.
❖ Wild animals and birds are free to enter the building through open windows
and rotted facia boards. Many diseases are associated with animal droppings
and infected animals. ,
❖ Penicillium is the most prominent mold found in the building. Penicilliurn is a
known allergen and the levels in all three areas are high enough to affect
sensitized individuals.
❖ Particulates (dusts and paint) generated on the second floor should be isolated
from employees and clients on the first floor.
TESTING and TEST CONDITIONS
1. Three air samples were taken to determine the presence of mold spores. The first sample
was collected in the Cafeteria, the second in the front office and the third in the EED unit office.
Normally an additional sample is collected outside which is used as a baseline for comparing the
indoor results. We elected not to include the outdoor sample because it was raining and rain will
significantly depress the outside mold levels, effectively making them useless.
2. SKC AirchecV personal monitoring pumps were used to collect the spores. The pumps
were connected to mixed cellulose ester filter cassettes and operated at 2.4 liters per minute for
approximately 90 minutes. Each pump was calibrated using a standard rotameter before testing
commenced and again after the testing was completed. No operating anomalies were noted.

3. A"tape-lift"sample was collected from between the first floor ceiling and second floor
joists. This sample was used to determine the presence/absence ofStachybotrys chartarum_ S.
chartarum is a particularly hazardous mold that doesn't produce many spores therefore is easily
missed in the airborne samples.
FINDINGS
1. An inspection of the exterior of the Baybridge facility revealed numerous holes in the
eaves (pix#1 and#2, enclosed). These holes were definitely large enough to allow pigeons,
mice,rats,raccoons, squirrels and other wildlife into the facility. (NOTE: Pigeon droppings are
often associated with a very hazardous mold called Aspergillus fumigates. Diseases carried by
other animals are well documented, like rabies.)
2. Exterior windows(2"d floor)were left open allowing rainwater into the facility(pix#3,
enclosed).
3. A significant amount of rot was found on the outside of the facility.
4. It was reported by May Institute employees that numerous roof leaks occurred in the past
and that some still exist.
S. Many(fiberglass) ceiling tiles were water stained, indicating a sustained problem with
rain water leaks.
6. The annular space between the ceiling tile and the second floor has numerous holes and
large openings (pix#4, enclosed). This allows mold spores, construction dust and other
particulates to easily migrate from the second floor into the May offices on the first floor_
7. According to May employees, demolishing work had recently been done on the second
floor.
8. Paint is chipping from the bead boards on the first floor ceiling above the drop-ceiling
(pix #5, enclosed). This paint might contain Lead(I did not test the paint) and, if so,the May
employees and clients are potentially exposed to Lead dust.
9. There was standing water in several trenches in the basement(pix#6, enclosed)and,
apparently, the basement walls leak profusely when it rains.
10. A strong, distinctive"musty" odor was detected in the basement.
11. We noted several fire safety and employee safety issues, like open electrical boxes, that
are beyond the scope of this inspection but, clearly, a potential threat to employees and clients
and should be not overlooked.
12. The following table lists the results of the mold testing. The most significant information
has been printed in RED.

13. A trace amount of Chaetomium was found on the tape-lift sample.
Conclusions and Recommendations
1. The physical structure needs a lot of work to eliminate the infiltration of water through
the roof. open windows, eaves and basement walls. Water ultimately leads to mold problems, if
not now, then certainly in the future. I recommend hiring a competent"Restoration" company to
effect all improvements. These companies will not only make the appropriate changes but will
take the necessary precautions to contain all airborne particles so May employees and clients will
not be exposed during the renovations.
2. Open windows, rotted wood, dislocated screens, etc. are allowing vermin into the
building. Diseases linked to wild animals are a totally unnecessary hazard to May employees
and clients. All animal and bird droppings need to be cleaned up and affected areas thoroughly
disinfected. Again, this is a job for competent restoration professionals, not the May staff. There
is too much risk of generating hazardous airborne particles during the cleanup and exposing
Baybridge staff and clientele.
3. Openings between floors are allowing particulates into the first floor area. All openings
should be covered and sealed as soon as possible. We recommend installing high(er) efficiency
filters in the furnaces to reduce the ambient dust levels. Filtrete brand(or similar filters)will
routinely remove 92%of all particles down to 2 microns from the air.
4. The chipping paint above the first floor tiles should be tested for Lead content. If Lead is
found then this paint needs to be removed in order to eliminate employee and client exposures.
5. Standing water in the basement is a major concern with respect to mold growth.
Appropriate grading of the soil and removal of the water is imperative in order to avoid serious
mold infestations in the future.
6. Penicillium is the most predominant mold found in any of the samples. Penicillium is a
known allergen and the concentrations we found inside the Baybridge facility are definitely
entering the levels which affect sensitized individuals. In spite of this,I don't think this is the
only causation of employee symptoms. Nuisance dusts including gypsum particles, garden-
variety dust/dirt, animal droppings,rotted wood fibers, etc. are all potential sources of respirator
irritation and each one of these items is clearly evident in the Baybridge facility.

7. Fortunately,the tape lift sample did not find anyStachybotrys chartarum. Only trace
amounts of Chaetomium were found and this mold poses no risk to Baybridge stag or clients.
In summary, it is obvious to me that Baybridge is not up to the(physical) standards I
have witnessed in any other May Institute facility. The structure is not well maintained, the
interior spaces are inherently dirty,rainwater and ground water intrusion create a natural habitat
for molds and there are a number of safety problems. This structure needs some serious
renovation work in order to bring it up to (May) standards. Until this is done, I believe that
employees are very likely to experience respiratory related health problems when working in this
building.

The Public Health Division has no objection to a larger dumpster being placed at
278 Main Street, Hyannis. Please contact the Fire Department at 775-1300 to
inquire whether its proposed replacement against a building would be allowable.
However, the_dumpster shall be screened from public view per the Board of
Health, Part VII, SECTION 1.00, nuisance Control Regulation No. 1. Therefore,
you are required to install fencing or some other form of year-round screening.

We are requesting a variance to the current approved trash removal method for
the above mentioned property as it pertains to the Site Plan Review dated
October 28, 1999.
We currently have a one yard dumpster, which is having to be emptied twice a
week due to the large volume of waste that is being generated by the occupants.
We would like to get permission to keep a two yard dumpster in the rear and
against the building in a manner as to not to interfere or disrupt pedestrians or
the merchants in the immediate area. We feel that there is adequate space for a
two yard dumpster and would be grateful if you would give our request strong
consideration.
