Dear Mr. Ojala,
You are granted permission on behalf of your client,. Bay Point LLC, to. construct
and utilize an innovative/alternative. (I/A) nitrogen reduction system at. 32
Pleasant Hill. Lane, Hyannis, Massachusetts.
This permission is.granted with the.following conditions:
(a) The wastewater effluent shall be tested once every six months for a
period of two years.for pH, BOD5, TSS, alkalinity, and TN.
(b) After two years, in the year 2008, the applicant shall be present at a
public Board of Health meeting for a determination of whether the
testing may be reduced.
(c) The system shall be installed in strict accordance with the revised
engineered plans dated revised May 10, 2006.
(d) The designing engineer shall supervise the construction of the onsite
sewage disposal system and shall certify in writing to the Board of
Health that the. system was installed in substantial compliance with the
revised submitted plans dated May 10, 2006.
(e) The applicant shall submit a copy of the signed two-year Operation.
and Maintenance Agreement (O&M) between the contractor and the

contractor shall conduct inspections to the I/A system a minimum of
twice yearly.

Certification for the MicroFAST 9.0 dated August 13, 2001.
The year round residential facility is proposed to be tested as follows:
Effluent from the System shall be monitored at least every six months.
The following parameters shall be monitored: pH, effluent CBOD5,
TSS, alkalinity and TN. Each time the System is monitored, the water
meter shall be read and the water use recorded.
