Public Health Division � ATEOMR�� 367 Main Street,Hyannis MA 02601 Office: 508-790-6265 Thomas A.McKea FAX: 508-775-3344 Director of Public February 13, 1996 Elizabeth Warren 51 Westminister Road Centerville, MA FINDING THAT THE DWELLING LOCATED AT 51 WESTMINISTER ROAD CENTERVILLE IS UNFIT FOR HUMAN HABITATION AND DETERMINATION OF POTENTIAL IMMEDIATE DANGER The dwelling owned by you and occupied by you located at 51 Westminister Road Centerville was inspected by Donna Miorandi, Health Inspector for the Town of Barnstable on February 13, 1996 at 11:30 a.m. due to a request for an inspection from Fire Chief John Farrington, Centerville-Osterville-Marstons Mills Fire Department. Attached is a copy of the inspection report. The following violations were observed: I 10 5 CMR 4 .602 B : Flooring contained excessive amounts of feces, urine, and cat v hair throughout. 105 CMR 410.550: Infestation of fleas, flies and maggots observed inside the dwelling. 105 CMR 410.352: Refrigerator was inoperable., Also, insects were observed inside the refrigerator. 105 CMR 410,500: Several windows were missing from the bedrooms; other windows were loose and partially detached from hinges. �- 105 CMR 410.352 (B): The toilet and the bathtub contained feces and build-up of foreign debris. 105 CMR 410.451: Overgrown trees and shrubs were obstructing the front exit/ passageway making it difficult to walk to and enter through the front door of the dwelling. 105 CMR 410,602: Several tree limbs and branches observed on the ground behind the (� dwelling. '`� The violations listed above as 410.550 and 410.602 (B) are also listed under 105 CMR 410.750 and are deemed as conditions which may endanger or impair the health, safety, and well-being of any person occupying the premises. Therefore, I find that this dwelling is unfit for human habitation. This letter shall serve as notification to you, the owner and occupant of the dwelling located at 51 Westminister Road, Centerville, that this dwelling is unfit for human habitation and shall further serve as notification to you that the conditions inside the dwelling may endanger or impair the health, safety, and well-being of any occupant of the premises. The hearing may result in an order of condemnation requiring you, the owner and occupant to secure the dwelling or portion thereof and requiring you to vacate the dwelling. At the hearing, you will be given an opportunity to be heard, to present witnesses or documentary evidence and to show why the dwelling should not be closed-up and to show why an order to vacate should not be issued. Attached is a copy of the inspection report.
