the report to the appropriate regional office of the Department of Environmental Protection. The original should be sent to the system owner
and copies sent to the buyer, if applicable, and the approving authority.

I am In receipt of the site plan review application regarding 844 Main Street Osterville and I submit the following
comments:
-This change of use, from retail to office, automatically triggers an inspection of the septic system components
by a DEP certified inspector(required by the 1995 Title V Code). The applicant or the owner shall hire an
inspector for this purpose and submit a completed ten (10) page inspection report to the Public Health Division
Office.
[NOTE: There were no septic system plans submitted with the application. Also, there were no records found
regarding the existing septic system at the Public Health Division Office.]
