Mullane III Hyannis, MA. Date: November 71 1979 Engineer: Charles L. Rowley & Assoc. Plan No. R-70 Gentlemen: The Board of Health approves this sub-division. The following conditions must be met: The well and septic system shall be located within the prescribed boundaries of the lot. A building permit will not be approved by the Board of Health on the lot until the well is installed and certification submitted as to the bacteriological and mineral content of the water by a State Approved Laboratory. The dwelling shall connect to the Town sewer when it is available. A percolation test must be made on the lot, at leaching site, bef6re a building permit will be issued. The proposed septic system must con- form strictly to Title 5, State Environmental Code, and Town of Barnstable Rules and Regulations. Prior to Board of Health approval of the building permit, the sewage system and water supply must conform to Title 5, of the State Environ- mental Code and Town Health Regulations that are in effect on the date of said issuance.
