Martin McNeely of the C-O-MM Fire Department. The following violations were observed: Unregistered underground fuel storage tank that is of unknown age and has not been tested via the Soil Vapor Analysis. Due to the unknown age of the tank we must presume that it is twenty years old and the tank must be removed because of it's age and the fact that it is located within a critical zone of contribution to public water supply wells. You are directed to have this tank removed by February 22, 1993. You must first obtain a removal permit from the Centerville-Osterville-Marstons Mills Fire Department. I have enclosed tank removal information for you. You are in violation of Article XXXIX because of the toxic and hazardous materials stored outside on the ground. The materials include paint cans, paint thinner, pennetrol and rustoleum along with other unlabelled containers. Webster, Jr. is pleased to submit this IRA Completion Statement and Permanent Solution Statement(PSS)with No Conditions. BACKGROUND The MassDEP on March 18, 2016 assigned Release Tracking Number RTN 4-26037 to the site due to fuel oil staining of soils around 3 Above Ground Storage Tanks at the site. The property is located at 3640 Falmouth Rd. in Marstons Mills, MA. The three AST locations are shown on Figure 1 which is from the Barnstable GIS System. The three ASTs are located next to two unoccupied homes which have been damaged by fire. The 13.43 acre lot is primarily undeveloped woodland. IMMEDIATE RESPONSE ACTIONS The MassDEP gave verbal approval to remove 3 ASTs , to excavate 15 CY of petroleum impacted soil.and to assess.the results of the IRA Response Actions. ➢ Removal of AST#1, #2 and#3 -April 11, 2016 Frank Corp. Environmental Services of New Bedford, MA obtained a Permit from the Centerville-Osterville-Marstons Mills Fire Dept. to remove 3 Above Ground Storage Tanks (ASTs)not connected to heating systems shown on Figure 1. On April 11, 2016 the three ASTs were removed from the site. Excavation of petroleum contaminated soil around AST#1 and AST#2 was completed. Excavation around AST#3 was initiated but not completed on April 11, 2016. P.O. The receipt by Northland is acknowledged on the Hazardous Waste Manifest included in Appendix C. ➢ Completion of the Excavation Around AST#3 on April 15, 2016 Frank Corp. with a mini excavator completed excavation under the entire concrete pad which was removed from the former AST#3 area. A total of 19.68 tons of petroleum impacted soil was taken to Aggregate Industries in Stoughton,MA for asphalt batching. A copy of the Bill of Lading for the impacted soil is included in Appendix C. ➢ Confirmatory Soil Sampling from Three Excavation Areas The confirmatory soil samples were sent to ESS Laboratory in Cranston, R.I. for VPH/EPH analysis. The Laboratory finding compared to the MassDEP S-1/GW-1 and S-1/GW-3 Cleanup Standards are shown on Table 1. All laboratory results for all five confirmatory samples were below the MassDEP Method 1 Cleanup Standards. ➢ IRA Completion Statement An IRA Completion Statement(BWSC105)based upon completion of all IRA Response Actions was prepared and submitted via eDEP. A copy is also included in Appendix A. PERMANENT SOLUTION STATEMENT (PSS)WITH NO CONDITIONS Exposure Point Concentration(EPCs)of the site based upon confirmatory soil sampling and analytical results in Table 1 are all below the MassDEP Method 1 Cleanup Standard for S- 1/GW-1 and S-1/GW-3 soils. The Disposal Site Boundaries are identified on Figure 4. Photos in Appendix B also Document the Disposal Site Boundaries around the 3 former ASTs. Section 40.0973(7) states "A Condition of No Significant Risk of harm to health,public welfare and the environment exists if no Exposure Point Concentration is greater than the applicable MCP Method 1 Standards." A Permanent Solution Statement(PSS)with No Conditions (BWSC)has been submitted to MassDEP through eDEP.
