Pawlowski Hyannis, Massachusetts Date: March 15, 1974 Engineer: Eldredge Surveying Co. Plan No. E 404 Gentlemen: The Board of Health approves this preliminary subdivision. The following conditions must be met: Town water shall be made available to each lot. If impossible- to obtain town water from appropriate water district, a letter to that effect must be obtained:. The developer must submit a copy of a master plan to the Board showing the exact location of the wells and septic systems throughout the subdivision. Each well and septic system shall be located within the prescribed boundaries of each individual lot. Building permits will not be approved by the Board of Health on individual lots until the well is installed and certification submitted as to the bacteriological and mineral content of the water by a State Approved Laboratory. Each dwelling shall connect to the Town Sewer when it is available® The developer must furnish the Board a statement from a Registered Engineer that the installation of private sewage systems on each lot will not- adversely affect surface or sub-surface public or private water r.esources of Mall lots in subdivision; (2) the- adjacent land (whether developed or not) ; (3) a defined aquifer recharge area. A percolation test must be made on each lots at leaching site, before a building permit will be issued. Each proposed septic system must conform strictly to -Article XI of the Massachusetts State Sanitary Code and Town of Barnstable Rules and Regulations. Prior to Board of Health approval of each building •permit, the sewage system and water supply must conform to State and Town Health Regulations that are in effect on the date of said issuance.
