Project Description 1. a. Project Location (use maps and plans to identify the location of the area subject to this request): Harbor Point Road Street Address Barnstable City/Town 352035 Assessors Map/Plat Number Between 35203500N and 35203500L Parcel/Lot Number b. Area Description (use additional paper, if necessary): Existing vegetated detention/infiltration basin (BMP) used for Stormwater Management: Approximately 5,800 square feet with an average depth of 2 feet.Inadvertently constructed over 40 years ago in the completion of an approved DEP tight tank waste disposal system (approved J une 22, 1987). c. Plan and/or Map Reference(s): Stormwater Mitigation & Maintenance Plan for BMP at Cape Cod Village Title 11/10/22 Date Title Date Title Date 2. a. Work Description (use additional paper and/or provide plan(s) of work, if necessary): This BMP has performed well but needs a maintenance plan that will conform it with an approved view cut permit (DEP File No. SE3-0346 dated March 19, 2002) to assure its continued operation maximizes the removal of pollutants. Specific activity is as follows: 1. Inspect the BMP for damage/dune influx (annual); 2. Prune vegetation in BMP and adjacent swale (annual); 3. Mow adjacent grass lawn and vegetated filter strips (as needed): 4. Monitor and remove invasive vegetation (as needed); Conformance with existing Certificate of Compliance DEP file no SE3-0346 dated 19-March-2002. Identify provisions of the Wetlands Protection Act or regulations which may exempt the applicant from having to file a Notice of Intent for all or part of the described work (use additional paper, if necessary). The affected area appears to be outside of but bordering the 100 foot buffer. See attached Wetland and Wetland Change Areas Map: source is maps.massgis.stte.ma.us/images/dep/omv/wetviewer.htm 3. a. If this application is a Request for Determination of Scope of Alternatives for work in the Riverfront Area, indicate the one classification below that best describes the project. Signatures and Submittal Requirements I hereby certify under the penalties of perjury that the foregoing Request for Determination of Applicability and accompanying plans, documents, and supporting data are true and complete to the best of my knowledge. I further certify that the property owner, if different from the applicant, and the appropriate DEP Regional Office were sent a complete copy of this Request (including all appropriate documentation) simultaneously with the submittal of this Request to the Conservation Commission. Failure by the applicant to send copies in a timely manner may result in dismissal of the Request for Determination of Applicability.
