Dear Mr. Perry,
The Club has decided that a concrete block foundation with a crawl space would obviously be better
than placing the building on posts as shown on the approved plan. Per our conversation of this
morning, I am notifying you that we are making this field change and excavation for this will begin on
Wednesday,November 3ra. The contractor, Sean Coutinho,will call for an inspection once it is
poured.

Enclosed please find plans for a proposed comfort station to be located within the confines of the Cummaquid Golf
Course as shown. The structure is to be constructed with the exact same materials as are being used for the
maintenance facility approved by your Commission previously and as you can see is of a style that is in keeping
with your requirements.
On behalf of the Cummaquid Golf Course,I would like to respectfully request a Certificate of Appropriateness for
this project.
