I am reaching out to request a six(6) month extension for building permit#B-18-1211—1336 Phinney's
Lane. The permit has already been granted one extension and currently expires on 6/19/19. Eversource
has confirmed that a line shutdown will be required for the installation which will have to be scheduled
in a couple of months. With this letter I have included a copy of the permit and a $50.00 check for the

Site Description:
The recently completed upgrade at the pre-existing wireless telecommunications site located at the above
referenced location consists of(9) new(replacement) panel antennas and related ancillary equipment
installed on the exiting antenna mounts (replacing (9) pre-existing panel antennas on the pre-existing
transmission structure), (6) newly installed 1-5/8" coaxial signal cables &(9) 3/8" RET cables routed
adjacent to the pre-existing cables to and up along the pre-existing transmission structure on T-Brackets.
(9) Newly installed Remote Radio Head (RRH) units were installed within the existing Verizon Wireless
equipment shelter. Existing site electric and telephone services were utilized/upgraded for utility services
to equipment installation.
Engineering Field Visit
On September 06,.201.8 Chappell Engineering Associates, LLC performed a field inspection of the
Verizon Wireless telecommunications installation described above to verify general conformance to the
engineering construction documents (Revision#0 noted as"Issued for Review" dated 11/10/15). The
following site photos and field visit form document our field visit, and the enclosed Final Construction
Completion Affidavit is attached as requested.
