Outcome Statement on behalf the Mr. Fred Burbank, homeowner of 33 Chase
Street, Hyannis, Massachusetts: The appropriate Bureau of Waste Site Cleanup
Transmittal Form (BWSC-104) is attached and included as Appendix A.
A release of approximately 50 gallons of home heating oil occurred in the
basement of the residential home on November 11, 2001. Although the
basement floor is concrete, some of the oil penetrated,the floor and entered the
soil. Emergency response actions and additional remedial actions have removed
the bulk of the impacted soil. Additional remedial actions include the
installation of a passive soil vent to allow for biological breakdown of residual_
contamination in-an attempt to approach background conditions.
