Scope of Work The scope of work to be completed as part of this site investigation shall include the development of the following elements: - Documentation of existing environmental conditions , such as water body and wetland locations or other environmentally sensitive areas, local geology , and presumed groundwater flow d direction; - Documentation of the locations of man-made structures including buildings, drainage ditches, storage tanks, septic systems and other subsurface utilities, and, - Documentation of the use, storage and disposal of hazardous materials on the site: this includes a review of governmental records pertaining to possible past releases, fuel tank storage permits and other applicable permits , ' and , includes interviews with abutters, and other persons with knowledge of the handling of oil or other hazardous material at the site. To accomplish this, Mason Environmental Services has performed the following tasks: - A site specific, detailed, reconnaissance by Mason Environmental Services Registered Environmental Property Assessor; A record search at local government agencies and the Department of Environmental Protection to update information regarding past releases of oil or other hazardous materials on the site or on adjacent properties; !A r. Page 2 � i 1 - Interviews and record searches at the Barnstable County Registry of Deeds , the Hyannis Assessor ' s Office, the site owner, and other knowledgeable persons pertaining to the site's history and usage; - Interviews and record searches at the Hyannis Fire Department, the Hyannis Board of Health and with other local agency officials and abutters as to the past and present usage of oil or other hazardous materials at the subject site and adjacent sites, and, P - A review of references provided by the U . S . Geological Survey , the Association for the Preservation of Cape Cod, the Cope Cod Commission, the Massachusetts Division of Fisheries and Wildlife , and the Massachusetts Coastal Zone o Management to identify areas of environmental concern in the vicinity of the subject site. i II. Site Description and Environmental Characteristics P This site occupies land zoned as Residential (RF-1 ) , and located in an area of exclusive summer and year round residences. The subject property has a history of commercial use, and is considered non-conforming relative to its zoning status. Refer to Figure 3 for a site schematic with pertinent features. Extant at the subject site are three buildings , referred to in this report as: the candle factory, the schoolhouse, and the warehouse. The candle factory is a single storied, concrete framed building with a poured concrete foundation and a full sized , finished basement. The exterior siding is masonry, and the front ! of the building is cedar shingled. The ground floor is mostly empty and contains some stored, household items, furniture and appliances . There are also toilet and offices facilities on the ground floor. The basement is mostly empty and contains an overhead garage door in the back of the building for drive-in' truck access . There are also two, 6 , 000 gallon wax storage tanks, and, what appears to be a fill pipe extending from the concrete floor in the basement . This building is surrounded by asphalt , and has two, steel covers located outside of, and in front of the building. These steel covers may protect fill pipes for the wax tanks in the basement. There is a steel cover outside of, and adjacent to the building. This cover may be an access to the septic system. There is also, what appears to be , another fill pipe outside of , and immediately adjacent to the building. The schoolhouse is a two storied, wood framed building with cedar clapboard siding and an asphalt shingled roof. This building is surrounded by asphalt, and, has a stone foundation and no basement, but a small crawl space under the floor. The ground floor is empty, and the second floor has contained an apartment in the past. The warehouse building is a single stor yt framed and wood sided building with an asphalt wood shingled roof . This building is surrounded b y halt, used by the owner as a small wood-working s shop, and is nd for q the storage of small boats, vehicles and some household � items. This building has a sprinkler system throughout,1 which is supplied by a water main on Scudder Avenue. ► This site is rolling, kettle and kame topography with a undeveloped and wooded, kettlehole containing a small ► wetland area abutting the site to the northeast . The building footprints occupy a subject pro ert PProximately 10$ of the P y . The area surrounding the three buildings is asphalt paved to accommodate 40 parking spaces. The remainder of the site is a wooded area and undeveloped. There are numerous trees felled on the property as a result of the most recent hurricane, and cut wood and piles of woodchips are present. Precipitation or other liquids incident to any w areas of the subject location will infiltrate hi ohld permeable soils . Liquids incident to the paved area immediately abutting Scudder Avenue will be delivered in a westerly direction to catch basins located on that street. Liquids incident to other paved areas at the site will be delivered in an northeasterly direction toward the wetland area. The site contains and abuts a small wetland area. This wetland is fed in part by two, subsurface conduits leading from roadway catch basins located on Scudder Avenue. This site is located approximately southwest of Schoolhouse Pond a 500 feet east of Halls River, and a ' PProximately 700 feet of Nantucket Sound. PProximately 2,500 feet north This site directly abuts a zone of contribution to the Barnstable Water Company's municipal supply wells M Mason Environmental Services, Inc. ' Page 8 P 1 number BW-384 , BW-229, and BW-376 . The alphanumeric codes are U.S . Geological Survey reference numbers . These are 12 or 18 inch diameter wells, each with a Pumping capacity of 1 , 700 gallons a minute . These municipal wells are located approximately 1, 500 feet north of the site. There are no known private, water supply wells located at the site.Refer to Figure 4 . 1 I IV. Site Layout i The subject Property is abutted to the: North, by residential -property, and a wetland and wooded area; Northeast, by a wetland and wooded area; East, by wooded and residential areas; South, by Lake Avenue, a municipal and unpaved right of way, and then private residences, and, Southwest and West by Scudder Avenue, and then private residences. V. Site Utilities Underground utilities at this site include: Natural gas for heating is supplied by Colonial Gas to the candle factory building, and the warehouse from a gas main on Scudder Avenue . At present, there is no gas service to these buildings . There is no heat available to the schoolhouse. Municipal water is available to all buildings at. this site, and is supplied by the Barnstable Water is Mason Environmental Services Inc. ' Page 10 Company from a water main on Scudder Avenue . At present , there is no water service to this r location. Septage disposal for the candle factory building is facilitated by an on-site , private 1 , 000 gallon septic tank and a 1, 000 gallon leaching pit. This system was inspected on 2/7/90 and found to be in good condition . Septage from the schoolhouse building is facilitated by two cesspools . These ` were also inspected and found to be in poor condition. There is no septage facilities for the warehouse building. L Aboveground utilities at this site includes: Electricity and telephone 1 buildings at the site by Commonwealth and New England Telephone . These utilities are facilitated by overhead lines running from utility poles on Scudder Avenue. There is a wood fired stove for heatin located the warehouse building. 9 ated in VI. Underground Storage Tanks i Mason Environmental Services has contacted the Hyannis Fire Prevention Officer and the Barnstable Health Department to inquire about the presence of underground storage tanks located at the subject property .Available information from both sources indicates that there are no underground storage tanks located at the subject site at this time. Refer to Appendix A. VII. On-Site Storage of Hazardous Materials The exterior of the buildings did not reveal any overt i ® Mason Environmental Services, Inc. Page 11 al)® evidence of unnaturally stressed or unhealthy vegetation, surficial soil staining, discoloration or odors associated with surficial or shallow sub- surficial contamination. There was no evidence of drums or other hazardous material storage containers or ® areas, or leachate breakouts. There was what appears to be a fill pipe extending from the ground at the southwest corner of the candle factory building. There was no evidence of excavation or filling activities at this site with the following exceptions : a patch of asphalt is missing from the paved area in front of the �1 warehouse building. . The interior of the candle factory building revealed A the presence of what appeared to be a fill pipe protruding from the concrete basement floor . Also located in the basement are two, 6 , 000 gallon , wax above ground storage tanks . There was no other evidence of oil or other hazardous materials at this location, or in the interiors of the other buildings. Documentation from the current site- owner indicated that on 7/11/90, Enviro-Safe Corporation of Sagamore, removed all residual, oil and other hazardous materials from the site which were associated with the previous candle manufacturing processes . These materials included : wax , waste oil , butyl stearate , lacquer adhesive , chemtest cleaning compound, and cyasorb. These materials were delivered to licensed, hazardous waste handling facility, Clean Harbors of Braintree . Refer to the Massachusetts Hazardous Waste Manifests presented as Appendix B. VIII. Summary of Site Inspection On September 25 , and October 3 , 1991 a Mason Environmental Services, Inc. Registered Environmental Property Assessor conducted. a visual and olfactory v Mason Environmental Services, Inc. Page 12 inspection of the interiors and exterior subject site. This inspection provided the following information germane to this report: An inspection of the exterior of the site provided no evidence of any oil or hazardous materials use, storage - or handling with the following exception: what appears to be a fill pipe at the corner of the candle factory. An inspection of the interiors of the buildings showed two, 6, 000 gallon wax storage tanks and what appears to be a fill pipe in the basement of the candle factory. There was no other evidence of any oil or other hazardous materials use , storage , handling . The abandoned apartment, above the old schoolhouse, was not available for inspection. Ix- Site history In order to obtain information concerning the history of the subject property, Mason Environmental Services, Inc . has conducted a file review at the Barnstable County Registry of Deeds , and has contacted the Yarmouth Assessors Office and other knowledgeable persons. This site was purchased by the present owner, Mr. Donald McKeag in 1989. The site has been inactive from that time until the present. Prior to Mr. McKeag, the site was owned by the Candle Corporation of America. Prior to this the property was owned by Leonard Silver Mfg. Co. of E. Boston, MA. Leonard Silver Mfg. Co. purchased the property from Harker China Co. of Ohio in 1986 . Harker China Co. purchased the property from Old Harbor A . Mason Environmental Services, Inc. Page 13 Industries, Inc. ( formerly Old Harbor Candle Co. ) of Hyannisport in June 1975. Old Harbor Candle Co. purchased the property from Old Harbor Realty Trust in January 1969. Old Harbor Realty Trust purchased the property from June and Olaf Johnson ► of Centerville in 1954 . ► Jane and Olaf Johnson purchased the property from Albert J. Davis of Needham in June 1945. Albert Davis purchased the property from Arnold and Dora Okuro in November 1944 . Arnold and Dora Okuro purchased the property from Victor Adams in January 1937. Prior to 1937 the records are incomplete. X. Agency File Reviews State, county and municipal agencies, as well as owners and other knowledgeable persons were contacted in order to obtain information concerning the possible release of oil or other hazardous materials at or near the subject site. This information has been collected via telephone conversations , file reviews , personal interviews and visits . The following paragraphs summarize this review. Conversations with the Hyannis Fire Department Prevention Officer, Lt. D. Chase, indicated that there were no underground storage tanks, and no reported spill/leak incidents at the subject site. Records obtained from the Barnstable Health Department did not indicate any violations, or mismanagement with respect to oil or other hazardous materials at the site or abutting sites . These files did indicate a spill/leak incident at 35 Scudder Avenue. Refer to 1 Hason Environmental Services Inc.. Page 14 Appendix A for this information. Record searches at the Barnstable Conservation Commission indicated that this site contains a small wetland area, and abuts a mapped Zone of Contribution to the Barnstable Water Company's supply wells. These file reviews also showed that, pursuant to an Enforcement Order issued on 9/6/901 a Notice of Intent filed on 9/28/90, and an Order of Conditions issued on 10/26/901 the site owner applied for and was granted permission to stabilize the existing slope to the wetland area with massed vegetation, and, clean all debris from the wetland and bordering areas. This project was issued a file number, #SE 3-21861 by the Massachusetts Department of Environmental Protection. Refer to Appendix C for a copy of this issuance from the state, and the Order of Conditions from the Conservation. Commission. The Cape Cod Commission's Water Resources and Land Use Map was reviewed. This reference indicates that there are no existing road salt storage wastewater treatment facilities, septage lagoons, open or closed landfills, junkyards, or cleaned-up hazardous waste sites located in the vicinity of the subject property. This reference did indicate that the site abuts a Zone of Contribution to the Barnstable Water Company's municipal supply wells. Refer to Figure 5. Mason Environmental Services, Inc . has also reviewed the 1991 Atlas of Estimated Habitats of Rare Wetlands Wildlife produced by the Massachusetts Division of Fisheries and Wildlife, the Cape Cod Critical Habitat Atlas produced by the Association for the Preservation of Cape Cod, and, the Areas of Critical Environmental Concern published by the Massachusetts Coastal Zone Management. These references did not list any areas of environmental concern in the immediate vicinity of the subject property. Refer to Figure 6 and Figure 7 . These deposits include recessional moraines, kames, and outwash plain deposits. Also present are deposits generated by glaciolucustrine activity, swamps and marshes , beaches and dunes , artificial fill and cranberry bogs. Refer to Figure 8. In the Hyannis quadrangle the glacial deposits have been mapped by Robert Oldale ( 1971 ) and others . The subject site lies in an area mapped as the Barnstable Outwash-Plain Deposits. These deposits are described as mostly gravelly sands with some pebble to small boulder gravel . Page 22 Seismic investigation on Cape Cod conducted by Oldale and others show that the unconsolidated glacial deposits in the area of the subject site are underlain 1 by crystalline, metamorphic and igneous basement rocks at level depth of approximately 400 feet below mean sea Groundwater flow direction beneath the subject area was estimated based on a preliminary evaluation of USGS =a topographic maps, the USGS Hydrologic Information Atlas (HA 692 ) , the Cape Cod Commission Water Table Contour Map and other sources. Page 24 - Interviews and record searches with state, county and municipal agencies and officials, and other knowledgeable persons did not indicate any mismanagement or impropriety associated with the handling, storage or usage of oil or other hazardous material at the site or abutting sites. A visual/olfactory inspection of the exterior of the subject site did not reveal an surficial expressions of contamination ,ationt or or indicate any mismanagement or impropriety with respect to the handling, storage or usage of oil or other hazardous material. A visual inspection of the interiors of the site showed the presence of two, 6 , 000 gallon, above ground storage tanks in the basement, and, a fill Pipe extending from the concrete floor in the basement of the candle factory building. There was no other indication of any mismanagement or impropriety with respect to the handling, storage or usage of oil or other hazardous' material. The abandoned apartment was not available for inspection. A review of references provided by the Association for the Preservation of Cape Cod, the Cape Cod Commission , the Massachusetts Division of Fisheries and Wildlife, and the Massachusetts Coastal Zone Management identified the presence of an on-site wetland, and an abutting Zone of Contribution to municipal supply wells. There were no other areas of environmental concern in the vicinity of the -subject site; Based on the field examination at the subject property, and regulatory agency file reviews p o of Mason Environmental Services Inc • that, opinion at s time, there is no evidence to indicate a release oft oil or hazardous materials has occurred on the site or at Hason Environmental Services, Inc. Box 86 ® Hyannis Port, MA . 02647 ® Re: Addendum to the MGL Chapter 21E Investigation 537 Scudder Avenue, Hyannis Port 0 Dear Mr. McKeag: ® At the request of the property owner , and as a result of ® information collected during the MGL 21E Level 1 Environmental Site Investigation, Mason Environmental Services, Inc. Registered ® Environmental Property Assessor has collected and reviewed additional data regarding the location of the pre-existing , gasoline, underground storage tanks associated with the former ` gas station known to have operated at site . Masn Environmental Services, Inc. has also advanced etwo test boring and installed two groundwater monitoring wells, and, collected and analyzed groundwater samples which are meant to represent ® groundwater quality flowing beneath the area where the pre- existing underground storage tanks were located. The following paragraphs detail this information. Upon examination of a photograph showing the subject property at the time when the gasoline station was operating , and ® conversations with a former employee of the station, it is the opinion of Mason Environmental Services , Inc . that , this information is sufficient to document the former location of the ® two, gasoline underground storage tanks . The area where these tanks are reported to have been, has since been excavated for ® the for the purposes of creating a basement space for the present ® building. There are no records of any tanks encountered during this excavation, and is there is no record of any tanks being present at the site. to 0 o 0 i Page 2 Mason Environmental Services, Inc. ® In order to determine whether the pre-existing gasoline , underground storage tanks had acted to impact groundwater 69 quality at the site , Mason Environmental Services , Inc . has completed an field exploration program as part of the present evaluation to assess the potential presence of oil or other ® hazardous materials in the groundwater at the site. This program O consisted of the execution of two test borings and the installation of two groundwater monitoring wells (MW1, MW2 ) , and p the collection and laboratory analysis of groundwater samples Q from the monitoring wells. ® A monitoring well consisting of a ten foot by two inch, . 010 o slotted, PVC, wellscreen attached to a solid PVC riser pipe was installed in each of the test borings. All PVC connections are flush-threaded to preclude the use of solvents or glues in order to prevent contamination. The wellscreen was set to straddle the water table encountered in the borehole during the boring operation and the anticipated fluctuation zone. The annular space was backfilled with clean, silica sand to a point two feet above the well screen, and then a two foot thick Bentonite seal was placed above this . These wells were finished with a cemented roadbox, flush mounted at grade level. Well installation details are presented on the Boring Logs presented as Appendix A. Exploration locations were selected to obtain groundwater samples from assumed downgradient areas to the pre-existing gasoline, underground storage tanks. Standards for the collection, custody and documentation of samples taken , as well as , quality assurance/quality control (QA/QC) and laboratory certification requirements have been maintained pursuant to Department Policy #WSC-300-89 - Minimum Standards for Analytical Data for Remedial _Response Actions. Water samples taken in the field are collected with a pre- cleaned , dedicated , PVC, bailer and dispensed to laboratory cleaned, 40ml or 500ml, glass vials with a Teflon septa or Teflon gasket lid. The vials are filled to .zero headspace and capped to exclude air bubbles. These samples are preserved with acid, as applicable, and refrigerated until their delivery to a state ® certified, contract laboratory. Groundwater analysis was performed by a state certified, contract ® laboratory - Groundwater Analytical , Inc . of Buzzards Bay , Massachusetts. Analytical results are presented in Appendix B. r 0 Page 3 Mason Environmental Services, Inc. Soils encountered in the test borings were physically characterized and logged by a Mason Environmental Services, Inc. field geologist. A visual/olfactory inspection of the soils from these test borings did not indicate any evidence of oil or other petroleum contamination in the near or subsurface. Refer to Appendix A for the Boring Logs and soil descriptions. On October 30 , 1991 , MW1 and MW2 were sampled . Prior to collecting groundwater samples, three to five times the initial standing volume of water in the wells was removed, in a manner which minimizes well turbulence , in order to insure representative , aquifer quality, formation sampling . These samples were analyzed for the presence of Volatile Organic Ida-locarbons and Aromatics (VOC's) as per EPA standard methods 601 and 602 . Analytical results are expressed in parts per billion (ppb) and are presented in Table 1 and in Appendix B. This program closely follows the �uidance provided by Interim Guidelines and Specifications for Preparing uality Assurance Project Plans, US EPA QAMS-005/80 (1980) , and Test Methods for Evaluating Solid Waste, US EPA SW-846, Third Edition (1986) . Quality Control protocols include Standard Operating Procedures (SOPS) developed for each analytical method. SOPS are derived from US EPA methodologies and other established references. Equipment and facility maintenance conform to Good Laboratory Practices (GLPs) . Standards are prepared from commercially obtained reference materials of certified purity, and documented for traceability. Quality Assessment protocols for most organic analyses include a minimum of one calibration standard, one method blank, one matrix spike, and one matrix spike duplicate for each sample batch. All samples, standards, blanks, and matrix spikes are spiked with internal standards and surrogate compounds. GC/MS systems are tuned to BFB ion abundance criteria daily, or for each 12 hour operating period, whichever is more frequent. Quality Assessment protocols for most inorganic analyses include a minimum of one calibration standard, one method blank, one sample duplicate, one matrix spike and one matrix spike duplicate for each sample batch. Standard curves are derived from one reagent blank and four concentration levels. Curve validity is verified by standard recoveries within plus or minus ten percent j of the curve. Analytical Batches are used as the basic unit for Quality Assessment. An Analytical Batch is defined as twenty or fewer samples which are analyzed together with the same method sequence and the same lots of reagents and with the same manipulations common to each sample within the same time period or in continuous sequential time periods. Method Blanks are used to assess the level of contamination present in the analytical system. Method Blanks consist of reagent (analyte-free) water or sand. Method Blanks are taken through all the appropriate steps of an analytical method. A universal blank matrix does not exist for solid samples. Sample data is reported on a blank-corrected basis. Matrix Spike/Matrix Spike Duplicates are used to assess accuracy and precision. A Matrix Spike/Matrix Spike Duplicate consists of two replicate samples (or two reagent blanks if sample volumes are inadequate) spiked with a group of target compounds representative of the method analytes. Accuracy is defined as the degree of agreement of a measured value with the true or expected value. Percent Recoveries for the Matrix Spike are calculated to assess accuracy. Precision is defined as the degree of mutual agreement of independent measurements as the result of repeated application of the method. Relative Percent Differences between the Matrix Spike and Matrix Spike Duplicate are calculated to assess precision. Surrogate Compounds are used to assess the effectiveness of the method in dealing with each sample matrix. Surrogate Compounds are organic compounds which are similar to organic analytes of interest in chemical behavior, but which are not normally found in environmental samples. Dear Sir: On March 11, 1980, all owners of underground storage tanks were sent a copy of a Board of Health regulation effective February 14, 1980, -governing underground fuel and chemical storage, and a card to fill out and return. The card listing any underground storage - tanks located on your property -at Rte. 132 - Hyannis . has not been returned. Please return the card as soon as possible -to complete our records. This tank ,is below groung and is kept warm (1554F) . At 140®-F this wax becomes a solid and wont flow. The Fire Dept, felt that the tank should be under- ground - as it could not contaminate the water table in any way - if it were to leak it would harden at the tank- & plug it self off.
