Sunday, January 9, 1994 @ 12:15 PM received a phone call at home from Hyannis Fire Dept. to respond to an oil spill at,96 Stetson Street,-Hyannis. Donna Miorandi arrived at 1PM, Owners of the home were present and they are Mr. & Mrs. Leonard D, Bacon�Phone 775-2522. Tank in basement is 38 years old. Last oil delivery was on 12/29/93 for 43.2 gallons by Point Oil leaving them with approximately 125 gallons in tank: Tank sprung a leak and noticed (smelled) by Mr. Bacon. He immediately put down speedi-dri and placed a bucket under tank to collect leaking oil. Capt. Cabral was on=site (and others) when I arrived. Fire Dept. plugged the tank and put a patch on its. . Enviro-Safe was called to -jump out the tank. Point Oil was not returning any calls at all. Nelson Coal & Oil was contacted and came with a 5 gallon reservoir as an emergency to get the owners an alternate heat supply for the nights. Monday morning Nelson Coal would hook them up with a new 275 gallon tank: Enviro-Safe & Nelson Coal arrived at 2:50PM. Taking bung off tank Enviro-Safe pumped out 115-125nals. Owners have Metroplitan Life Ins. Company. Enviro-Safe returned on foilowing Wednesday and performed clean-up of contaminated speedi-dri. I Date: August 4, 2004 Incident: Underground propane tank,valve "popped", leaking Time of Response: 1:30 pm Location: 67 Stetson Street, Hyannis, MA 02601 Residents: Roy Okurowski & Susan Okurowski, Owners 1st Responders: Hyannis Fire Department: Lt. Cadrin and staff, Eric Hubler, Fire Inspector, Hyannis Police. 2"d Responders: David Stanton, Amy Wallace, TOB Public Health Division Notes: • Spoke to Lt. Cadrin first. Relief valve of underground propane storage tank "popped",may have been hit with lawnmower. • Age, capacity of tank not known at this time by owner or anyone at scene. This type of tank is rare/old and not something that anyone at scene can recall inspecting or responding to accidents. • House was hooked up to natural gas approx. ten years ago. • Roy O. began to discuss whether he could fill the tank with sand after the gas is emptied out. He can, but he will still have to cut into tank. • Removal of tank is discussed. FD or Health regulation could require removal of tank? Discussion between D. Stanton, E. Hubler& Lt. Cadrin. • Discussion —under certain conditions your can burn it (propane) off. •:c • .Roy O: asked, "if it is off-gassed, can it be left alone (in the ground)?" • D. Stanton: Tanks usually have to come out within 2 years of non-use. • TOB Gas Inspector from Bldg. Dept. is on his way. + s • Specs of tank need to be known to determine what to do. • Discussion—in future, send flier to alert other homeowners of the risks with these types of tanks left on their property. • Regardless of what Gas Inspector says, the tank will have to be off-gassed and removed. Considerations: if it is under/supporting a structure— how to remove? • Determination: Utility Company must be called to contain/control gas and determine the length of time it will leak, removal, how to remove? 3rd Responder: Gas Inspector Burnham from TOB Building Division — 1:55 pm • (Amerigas is on their way) • CMR 9—if not in use, has to come out of ground. • Current status at this point: The tank must be off-gassed. Must be removed in near future. Propane lines to house have been cut (approx. in 1991) 4tn Responder: Amerigas Utility Company: Steve Lovell, Lead Technician —2:25 pm • Steve assessed valve, dug up ground around it, and produced a gauge that read "10% left", if this is accurate. • All responders still assessing HOW the cap came off. Explanations: 1)Yard work/lawnmower hit valve and has hit it several times in past. 2)Wagon hit valve and knocked it off. 3) Roy O. looked at it after wagon hit it and it was leaking. • Susan O. (homeowner)stated to A. Wallace that she had told her husband for years "the tank should be looked at." Roy O. is an engineer and currently not working. He knew about the tank and attempted to look at valve while they were doing yard work that afternoon. 5tn Responder: Gene Dziedzina, Amerigas Manager(508) 775-0686 2:40 pm • Amerigas hasn't seen a tank like this before, highly unusual. • Gene D. assists in digging up tank valves and tank"chimney" that surrounds valves. • Gene D. assesses that utility company/propane supplier should be called Vt when dealing with a tank of this nature. Agrees with A. Wallace that the order of contact should be: 1) Utility company, 2) Fire Dept. to assess (if necessary) 3) Health Dept. (for removal requirements). 5th Responder: Hyannis Fire Dept. Captain —3:15 pm • Captain was briefed of situation. • Captain checked valve in basement area for odor—none detected. • Amerigas prepared to transfer propane into tanks • Lt. Cadrin confirmed follow up to tank removal would be conducted by Health Dept. • D. Stanton and A. Wallace discussed creating a notice for homeowners with similar tanks to make them aware of the dangers/health risks, how to have the tanks assessed (by utility company first), and the appropriate procedure to follow to address this proactively. This does have the potential to happen again. • Information will be distributed to homeowners as needed. • Amerigas stayed at location, all other responders dispersed. Cadrin and staff, Eric Hubler, Fire Inspector, Hyannis Police. 2"d Responders: David Stanton, Amy Wallace, TOB Public Health Division Notes: • Spoke to Lt. Cadrin first. Relief valve of underground propane storage tank "popped",may have been hit with lawnmower. • Age, capacity of tank not known at this time by owner or anyone at scene. This type of tank is rare/old and not something that anyone at scene can recall inspecting or responding to accidents. • House was hooked up to natural gas approx. ten years ago. • Roy O. began to discuss whether he could fill the tank with sand after the gas is emptied out. He can, but he will still have to cut into tank. • Removal of tank is discussed. FD or Health regulation could require removal of tank? Discussion between D. Stanton, E. Hubler& Lt. Cadrin. • Discussion —under certain conditions your can burn it (propane) off. • Roy O. asked, "If it is off-gassed, can it be left alone(in the ground)?" • D. Stanton: Tanks usually have to come out within 2 years of non-use. • TOB Gas Inspector from Bldg. Dept. is on his way. • Specs of tank need to be known to determine what to do. • Discussion — in future, send flier to alert other homeowners of the risks with these types of tanks left on their property. • Regardless of what Gas Inspector says, the tank will have to be off-gassed and removed. Considerations: if it is under/supporting a structure— how to remove? • Determination: Utility Company must be called to contain/control gas and determine the length of time it will leak, removal, how to remove? 3rd Responder: Gas Inspector Burnin from TOB Building Division — 1:55 pm • (Amerigas is on their way) • CMR 9— if not in use, has to come out of ground. • Current status at this point: The tank must be off-gassed. Must be removed in near future. Propane lines to house have been cut (approx. in 1991) 41h Responder: Amerigas Utility Company: Steve Lovell, Lead Technician —2:25 pm • Steve assessed valve, dug up ground around it, and produced a gage that read "10% left", if this is accurate. • All responders still assessing HOW the cap came off. Explanations: 1)Yard work/lawnmower hit valve and has hit it several times in past. 2) Wagon hit valve and knocked it off. 3) Roy O. looked at it after wagon hit it and it was leaking. • Susan O. (homeowner) stated to A. Wallace that she had told her husband for years "the tank should be looked at." Roy O. is an engineer and currently not working. He knew about the tank and attempted to look at valve while they were doing yard work that afternoon. 5'h Responder: Gene Dziedzina, Amerigas Manager(508) 775-0686 2:40 pm • Amerigas hasn't seen a tank like this before, highly unusual. • Gene D. assists in digging up tank valves and tank"chimney" that surrounds valves. • Gene D. assesses that utility company/propane supplier should be called 1s` when dealing with a tank of this nature. Agrees with A. Wallace that the order of contact should be: 1) Utility company, 2) Fire Dept. to assess (if necessary), Health Dept. (for removal requirements). 5th Responder: Hyannis Fire Dept. Captain — 3:15 pm • Captain was briefed of situation. • Captain checked valve in basement area for odor— none detected. • Amerigas prepared to "flare off' gas. • Lt. Cadrin confirmed follow up to tank removal would be conducted by Health Dept. • D. Stanton and A. Wallace discussed creating a notice for homeowners with similar tanks to make them aware of the dangers/health risks, how to have the tanks assessed (by utility company first), and the appropriate procedure to follow to address this proactively. This does have the potential to happen again. • Information will be distributed to homeowners as needed. • Amerigas stayed at location, all other responders dispersed.
