Sufficient characterization has been completed and sufficient data exists to conclude that there is petroleum impacted groundwater at the site and that the impacted groundwater is derived from an off-site source; in all likelihood, a previously identified petroleum release site with ' Release Tracking Number (RTN) 4-14257. Opinions expressed in this document are subject to the limitations set forth in Appendix A. ' 2.0 SITE DESCRIPTION The DPS site is located at 379 Main Street, Hyannis, Massachusetts. A Site Locus Map is provided as Figure 1. A Site Map is provided as Figure 2. The site consists of approximately 0.24 acres and contains a single building of wood and steel construction on a concrete slab and poured concrete foundation. The building is a four unit commercial condominium occupied by a ' restaurant, a hair salon and a tailor shop. The property boundaries are shown on Figure 2. The property is bound to the north by West Main Street, across which is a garden center, to the west by Suomi Road, across which is a Cumberland Farms convenient store and gasoline filling ' station, to the east by a restaurant and to the south by residential property and a landscaping company. ' 3.0 SITE CHARACTERIZATION In January 2008, Green Seal Environmental (GSE) initiated a Preliminary Environmental Site ' Assessment (PESA) on the subject parcel. The PESA revealed the following information about the site: ' As stated above; the property occupies 0.24 acres and includes one commercial condominium building with four units. The majority of the parcel is occupied by the building, which is ' surrounded by an asphalt parking lot. Limited quantities of oil and/or hazardous material are stored in the basement of the building and were properly contained. The property is served with natural gas and municipal sewer, hence there is no fuel storage or wastewater discharge on the ' site. A review of historical records indicates that there is no history of storage or use of petroleum at the subject site. 1 ' Green Seal Environmental,Inc. Downgradient Property Status 379 Main Street ' Hyannis,MA March 2008 A number of monitoring wells are located to the west of the building within the layout of Suomi ' Road and on the Cumberland Farms property. The wells are associated with a petroleum release on the Cumberland Farms property. In addition, a shed, that houses soil vapor recovery and treatment equipment, is located on the Cumberland Farms property. Following the identification of a release of petroleum on the adjacent property through both the observation of the monitoring wells and its identification in an environmental agency database ' report, GSE determined it would be appropriate to conduct additional investigative efforts on the subject site. GSE completed a file review of the Cumberland Farms release at the Massachusetts Department of Environmental Protection (MassDEP) and conducted a Limited Site Investigation ' (LSI) on the subject site (379 West Main Street). Further descriptions of the release and the LSI are provided in the following sections. ' 4.0 DESCRIPTION OF RELEASE The following information is derived from reports reviewed during the MassDEP file review. In ' 1998, a petroleum release was reported to MassDEP and was assigned RTN 4-14257. MassDEP required certain response actions in accordance with the Massachusetts Contingency Plan (MCP - ' 310 CMR 40.0000). An Immediate Response Action (IRA) was undertaken at the Cumberland Farms site entailing the removal and disposal of 80 cubic yards of petroleum impacted soil. Following the IRA, Phase II and Phase III reports were filed by ENSR for Cumberland Farms. ' In 2002 ENSR was conducting feasibility testing for a proposed Soil Vapor Extraction/Air Sparging (SVE/AS) active remediation system at the Cumberland Farms facility. During the conduct of the test, ENSR encountered 0.65 feet of free-floating petroleum in a test well. In response, ENSR reported a second release to MassDEP and RTN 4-17412 was assigned. The second RTN was subsequently closed and Cumberland Farms continued to conduct remedial ' activities under the original RTN (4-14257). Ongoing remedial activities include operation of the SVE/AS system. ' Site investigations related to the Cumberland Farms report groundwater flow direction to be towards the southeast. According to the reports, the likely source of the petroleum is either underground storage tanks (USTs) on the west side of the property or the fuel dispensing system ' located on the north end of the property. This configuration would place the.south end of the subject site (379 West Main Street) in the path of and downgradient of the release. This is supported by the placement of the SVE/AS system in Suomi Road to the southeast of the USTs and dispensing system, and adjacent to the 379 West Main parcel. 2 ' Green Seal Environmental,Inc. i Downgradient Property Status 379 Main Street ' Hyannis,MA March 2008 5.0 LIMITED SITE INVESTIGATION—379 West Main Street ' 5.1 Subsurface Investigation ' GSE completed a limited subsurface investigation at the 379 West Main Street property. The purpose of the investigation was to determine if groundwater and soil at the property are impacted by petroleum hydrocarbons above regulatory standards; if a report to the MassDEP was ' necessary; and if the potential existed for the filing of a DPS Opinion with MassDEP in accordance with 310 CMR 40.0180. ' The subsurface investigation was conducted over a period of three days. On February 26, 2008, GSE provided oversight to Desmond Well Drilling, Inc. (Desmond) while they installed three soil borings and groundwater monitoring wells at the site. A track-mounted GeoprobeTM drilling ' rig was used to collect soil samples and install the monitoring wells. Well locations are shown on Figure 2. Boring logs are provided in Appendix B. Six to seven soil samples were collected from each boring during advancement of the GeoprobeTM and were field screened for total organic vapors (TOVs) using a photoionization ' detector (PID) following the MassDEP recommended Jar Headspace Method. The sample with the highest result from borings MW-3 and MW-2 were collected in a laboratory supplied sample container for analysis. These samples were from either the groundwater/soil interface or just ' below. No sample was submitted from the boring for MW-1 because there was no evidence of impact. The soil samples were labeled and then transferred, via chain-of-custody procedures, to the laboratory for analysis. The soil samples collected were identified as MW-2/S-6, and MW-3/S-6. After the desired depth was reached, 2-inch diameter groundwater monitoring wells were installed in each boring. Each well was finished with protective flush-mounted casings. On February 28, 2008 GSE returned to the site to collect groundwater samples. The wells were ' developed with a low flow GeopumpTM to purge each well and collect samples for laboratory analysis. Three to five volumes of groundwater was evacuated from each well prior to sample collection. The collected soil and groundwater samples were transferred to Groundwater Analytical, Inc. of ' Buzzards Bay following chain-of-custody procedures and analyzed for VPH with target VOCs and Methyl Tertiary Butyl Ether(MTBE). GSE conducted a level survey of the monitoring wells in order to determine the direction of groundwater flow. It was determined that groundwater flows to the southeast across the property. 3 ' Green Seal Environmental,Inc. ' Downgradient Property Status 379 Main Street ' Hyannis,MA March 2008 5.2 Laboratory Results - Soil ' The laboratory results for the soil analyses show concentrations of the VPH carbon range C9-C12 aromatics above the Reportable Concentration (RC) for soil classified in the MCP as S-1 in the sample collected from monitoring well MW-3. The soil sample was collected from a depth of 20-24 feet, which corresponds with the depth of the water table. Although not above their applicable RCs, several VOCS were reported in the soil sample collected at MW-3. Analytical ' results are summarized on Table 1. Laboratory analytical reports are provided as Appendix C. 5.3 Laboratory Results - Groundwater ' r 1 for the groundwater analyses,show elevated concentrations of VPH carbon The laboratory results o gro ' ranges in the groundwater samples collected from monitoring wells MW-2 and MW-3. The results in monitoring wells MW-2 and MW-3 for the C5-C8 carbon range are 1,800 ug/L and 2,700 ug/L respectively, compared to the RC of 300 ug/L for groundwater classified in the MCP as GW-1 (RC GW-1). The results in monitoring wells MW-2 and MW-3 for the C9-C12 carbon ' range are 860 ug/L and 5,500 ug/L respectively, compared to the RC GW-1 of 700 ug/L. The results in monitoring wells MW-2 and MW-3 for the C9-C10 carbon range are 5,800 ug/L and ' 10,000 ug/L respectively, compared to the RC GW-1 of 200 ug/L. Select VOCs were also present above RCs in MW-2 an MW-3. All results in the MW-1 sample ' were below Laboratory Method Detection Limits. Analytical results are summarized on Table 2. Downgradient Property Status 379 Main Street ' Hyannis,MA March 2008 6.0 CONCLUSIONS iBased on the scope of this investigation and subject to the attached Limitations, GSE makes the following conclusions: 1. Based on historical research and the subsurface investigation conducted at the site, there is no known source of petroleum hydrocarbons on the property. Groundwater flow has ' been determined by GSE and others to flow to the southeast. The Cumberland Farms property is adjacent to and upgradient of the subject site. The laboratory results identify petroleum hydrocarbons at the site in soil and in groundwater, which is consistent with the historical use and documented release from the Cumberland Farms Property. Given the above rationale, it is GSE's opinion that the most likely source of the petroleum hydrocarbons identified in this investigation is the release of petroleum hydrocarbons on the Cumberland Farms property. 2. Several analytes in both the soil and groundwater samples exceed the applicable RCs for ' those compounds. According to the Massachusetts Contingency Plan (MCP), the owner of the subject site is required to submit a report to MassDEP within 120 days disclosing ' these results. 3. GSE is therefore submitting, on behalf of the owner of 379 West Main Street, a Release ' Notification & Notification Retraction Form (BWSC 103), and a Downgradient Property Status (DPS) Transmittal Form (BWSC 115). The two BWSC forms are being submitted simultaneously with this report and copies of the forms are included as Appendix D. Program Overview, 1 Groundwater Analytical conducts an active Quality Assurance program to ensure the production of high 1 quality, valid data. This program closely follows the guidance provided by Interim Guidelines and Specifications for Preparing Quality Assurance Project Plans, US EPA QAMS-005/80 (1980), and Test Methods for Evaluating Solid Waste, US EPA, SW-846, Update III (1996). 1 Quality Control protocols include written Standard Operating Procedures (SOPS) developed for each analytical method. SOPS are derived from US EPA methodologies and other established references. Standards are prepared from commercially obtained reference materials of certified purity, and documented ' for traceability. Quality Assessment protocols for most organic analyses include a minimum of one laboratory control sample, one method blank, one matrix spike sample, and one sample duplicate for each sample preparation batch. 1 All samples, standards, blanks, laboratory control samples, matrix spikes and sample duplicates are spiked with internal standards and surrogate compounds. All instrument sequences begin with an initial calibration verification standard and a blank; and excepting GUMS sequences, all sequences close with a continuing calibration standard. GC/MS systems are tuned to appropriate ion abundance criteria daily, or for each 12 1 hour operating period,whichever is more frequent. Quality Assessment protocols for most inorganic analyses include a minimum of one laboratory control ' sample, one method blank, one matrix spike sample, and one sample duplicate for each sample preparation batch. Standard curves are derived from one reagent blank and four concentration levels. Curve validity is verified by standard recoveries within plus or minus ten percent of the curve. ' B Definitions Batches are used as the basic unit for Quality Assessment. A Batch is defined as twenty or fewer samples of the same matrix which are prepared together for the same analysis, using the same lots of reagents and the same techniques or manipulations,all within the same continuum of time, up to but not exceeding 24 hours. ' Laboratory Control Samples are used to assess the accuracy of the analytical method. A Laboratory Control Sample consists of reagent water or sodium sulfate spiked with a group of target analytes representative of the method analytes. Accuracy is defined as the degree of agreement of the measured value with the true or ' expected value. Percent Recoveries for the Laboratory Control Samples are calculated to assess accuracy. Method Blanks are used to assess the level of contamination present in the analytical system. Method Blanks consist of reagent water or an aliquot of sodium sulfate. Method Blanks are taken through all the appropriate ' steps of an analytical method. Sample data reported is not corrected for blank contamination. Surrogate Compounds are used to assess the effectiveness of an analytical method in dealing with each sample matrix. Surrogate Compounds are organic compounds which are similar to the target analytes of 1 interest in chemical behavior, but which are not normally found in environmental samples.
