The double plate at the base of the wall was reduced to a single plate and the anchor bolts were replaced with concrete pins 24"O.C. 6.4 We investigated the condition of the inadequate bearing and/or gap at the double 2x10 joist(detail 10/SS)on the interior bearing wallss at the left side. A 1/4"to 1/2"gap exists below or above the 2x10 joist and also the wood blocking. Our office stated this condition must be corrected by adding plywood shims as needed. 6.5 The wood blocking and broken stud on the I st level of the left wing(previously noted to be defective)still remain to be corrected. 6.6 Temporary wood blocks were added to replace the angle clips at stud wails at the elevators and will eventually be replaced with the angle clips as stated by the superintendent. The double plate at the base of the wall was reduced to a single plate and the anchor bolts were replaced with concrete pins 24"O.C. 6.4 We investigated the condition of the inadequate bearing and/or gap at the double 2x10 joist(detail 10/S5) on the interior bearing walls at the left side. A 1/4"to 1/2"gap exists below or above the 2x10 joist and also the wood blocking. Our office stated this condition must be corrected by adding plywood shims as needed. 6.5 The wood blocking and broken stud on the 1 st level of the left wing (previously noted to be defective) still remain to be corrected. 6.6 Temporary wood blocks were added to replace the angle clips at stud walls at the elevators and will eventually be replaced with the angle clips as stated by the superintendent. EXCEPTIONS: 5.3 We found a number of areas of the interior load bearing walls where the wood blocking is not wide enough between the wood studs and should be replaced. 5.4 One Stud is broken and needs to be replaced(marked with orange paint). 5.5 At the CMU fire wall on the right side of the building one vertical reinforcing bar is missing at each free end of the wall. According to the superintendent,this wall will be extended 6"in both directions. Once the extension is made the missing bars can be drilled into the slab and extended vertically through the wail. 5.6 Add one missing vertical reinforcing bar at the corner of the elevator on the right side of the building. As per your request, we have completed a site inspection at the above captioned. The inspection was completed by the undersigned on 11'August 1998 and we were accompanied by Mr. Mick Fitzgerald, interim Superintendent for Congress Construction. The purpose of the inspection was to review the character of the on-site soils and to make recommendations regarding the re-use of the soil within the building footprint. Presented below is a summary of our observations and recommendations. • Foundation construction is currently in progress with the majority of the perimeter footings and grade walls already in place. Work was continuing on the perimeter footings and grade walls while no interior footings have, as yet, been placed. • At the time of our visit, no earthwork was in progress. • The site has been adequately cleared and grubbed within the building footprint and the layer of sandy LOAM topsoil has been removed. Two large stockpiles containing LOAM, roots and other organic matter are currently outside the building footprint. We understand this material is.schedule for re-use in planted areas. • Where the subgrade is exposed in footing trenches, the subgrade soils are consistent with our findings during the subsurface investigation and are adequate for supporting the footings as described in our report. • The soil excavated from the footing trenches consists of natural granular soil with varying gradations and varying proportions of gavel. This excavate has been stockpiled at several locations outside the building footprint. Based on our visual examination of the material, the moisture content appears very low and is probably less than about two per cent. Some backfilling using the excavate soil has been done within the building footprint and within some footing trenches on the outside of the grade wall. We understand all backfill was placed in thin lifts and compacted with a vibratory plate compactor. 133 East Main Street • Marlborough, Massachusetts 01762 08N18/98 10:11 '$617 884 4329 ARCHITECTURAL TM Z 003/003 Wbutehall Estates-Hyannis,Massachusetts GO Project No.961274 12 August 1998 Page#2 Although the gradation of the natural granular soil excavated from the footing trenches does not strictly meet the specified gradation for structural backfib, this soil contains no appreciable organic matter or other deleterious material and should be re-used for structural backfill. The excavated soil is all granular however the proportions of sand-sized and gravel-sized fractions vary considerably over the site. Laboratory determination of the maximum dry density done by Briggs Associates show results varying from about 118 pcf for the finer material to about 133 pcf for the material containing a higher proportion of gravel with the optimum moisture content at about 5 to 6 per cent. This relatively large range of the maximum dry density makes field quality control difficult. We recommend the natural granular soil excavated from the footing trenches be re-used as structural backfill. Due to the low moisture content this material will be difficult to compact in its present state. We strongly recommend that sufficient water be added to increase the placement moisture content to about 4%to 5%. The soils must be placed in thin lifts, not exceeding twelve inches loose measure, and compacted using appropriate vibratory compaction equipment. Since the gradation of the material varies, field determinations of the maximum dry density may be required in order to maintain good quality control and ensure compliance with the Building Code requirements_ Good construction practice dictates that backfill be placed both inside and outside the grade walls or other foundation walls to eliminate unbalanced earth pressures which can cause damage. Alan White, Simon Thomas and John Lavell were also present. This is a 40B Comprehensive Permit(Zoning Board of Appeals approval) and no formal Site Plan Review would be required. RC Zoning District, GP Overlay District. Mr. Ford believes this project complies with all zoning restrictions other than for use. 6,500 sq.ft.building. Described assisted living logistics. Parking for 50+vehicles and plenty of room for future parking if needed. The residents here would not generally drive. Will be connected to sewer. Addressed Health Division comments.
