RE: Site Plan Review 001-15 Strawberry Hill Nominee Trust
1030 Falmouth Road, Hyannis Map 250, Parcel 065
Proposal: This project involves the removal of an existing building and construction of a new one-
story office building with-up to three tenants. The new office building will increase the
existing gross floor area with a total of 17,974 gross s.f. proposed. Complete site
improvements are proposed including new parking and entrance, improvements to
stormwater management, and new public utility connections including removal of an
existing septic system and connection to public sewer.
Dear Mr. Eddy:
Please be advised that subsequent to the formal site plan review meeting held January 8, 2015,
revised plans have been administratively approved subject to the following:
• Approval is based upon, and must be substantially constructed in accordance with plan entitled
"Strawberry Hill Nominee Trust, Site Development Plans"consisting of 18 sheets,prepared for
Strawberry Hill Nominee Trust by Baxter Nye Engineering& Surveying dated November 14,
2014 with final revisions December 29, 2015 reflecting site plan review committee comments,
including Hyannis Fire Department approval of ladder truck turning, fire hydrant and FDC
location and landscape plan; floor plans and elevations, 3 sheets,prepared by Architect Worx
Architectural Design, Easton, MA dated December 28, 2015.
• Permits to remove the existing septic system will be required from the Health Department.
• A photometric plan.depicting zero (0) light spillage onto abutting properties will be required at
the building permit stage.
• Applicant must obtain all other applicable permits, licenses and approvals required.
Upon completion of all work, a registered engineer or land surveyor shall submit a letter of
certification, made upon knowledge and belief in accordance with professional standards that all
work has been done in substantial compliance with the approved site plan (Zoning Section 240-105
(G). This document shall be submitted prior to the issuance of the final certificate of occupancy.
A copy of the approved site plan will be retained on file.

Proposal: This project involves the removal of an existing building and construction of a new two-
story medical office building. A smaller footprint and an approx. 200 s.f, reduction in
GFA is proposed from original application and 22 additional parking spaces are provided
due to the identification of medical office use. Complete site improvements are proposed
including new parking and entrance, improvements to storm water management, and new
public utility connections including removal of an existing septic system and connection
to public sewer.
Dear Mr. Eddy:
Please be advised that the above revised previously-approved proposal has been found to be
administratively approvable subject to the following:
• Approval is based upon, and must be substantially constructed in accordance with plan entitled
"Strawberry Hill Nominee Trust, Site Development Plans"consisting of 18 sheets dated
November 14,2014 with final revisions April 13, 2017 reflecting revised smaller building
footprint and addition of 22 parking spaces and a full second story; also, Stormwater
Management Report dated December 30, 2014 revised May 3, 2017 both prepared for
Strawberry Hill Nominee Trust by Baxter Nye Engineering& Surveying 3 sheets; and, floor
plans&elevations prepared by MedCom Architectural Group, Bourne, MA dated April 14,
2017.

• A Conditional Use Special Permit is required from the Zoning Board of Appeals for medical
office use in the BB District.
• Permits to remove the existing septic system will be required from the Health Department.
• A photometric plan depicting zero (0) light spillage onto abutting properties will be required at
the building permit stage and approved by DPW.
• Applicant must obtain all other applicable permits, licenses and approvals required.
