Mr. Perry is the 1 Potentially Responsible Party (PRP) for the site. The RAO was developed in accordance with Subpart J - Response Action Outcomes of 310 CMR 40.0000, the Revised 1995 Massachusetts Contingency Plan (MCP) and it incorporates all other applicable sections of 1 the MCP. It was developed under the oversight and direction of a Licensed Site Professional. ' 1.2 Limitations The scope of this report is limited to response actions taken by AET relative to the release ' described below, and is strictly limited to the horizontal and vertical limits of the excavation shown on Figure 1 and labeled the "Response Action Outcome Area". The Response Action Outcome Area is 15' x 12' x 15' deep. It is located on the east side of the ' house to the right of the entrance (Appendix A-Photograph 1). Quadrangle: Cotuit, 1974 (Figure 2) Assessor's Reference: Map 36, Parcel 34 (Figure 3) 1 2.2 Brief Site Description The site is a level lot on the corner of Putnam Avenue and Main Street (Figure 3). It is 0.22 acres and completely developed by a two-story, wood-framed house, unpaved driveway and garden tool shed. The Cotuit Water Department supplies.drinking water to 1 the site and to the surrounding houses from its public water supply system. A town pumping well, referred to as Station#3, is located approximately 800 feet west of the site, placing the site within the Zone II of this and other town wells (Figure 4). Household 1 septic is disposed of on-site through an on-site septic system. Runoff is controlled on-site. The only impervious surfaces are the rooftop areas. 1 Precipitation shed from the rooftops infiltrates into the ground. Precipitation elsewhere on the site also infiltrates into the ground. The site is vegetated with ornamental bushes and shrubs. Areas of the site around the buildings are grassed and an unpaved driveway is located along the east side of the house. 1 2.3 Site Geology/Hydrogeology 1 The site is located on a glacial outwash plain. Soils are comprised of well sorted sands and gravels. (MES) of Pocasset, 1 Massachusetts as discussed in a report titled "Immediate Response Action Plan, Release 4- 10036, 5 Putnam Avenue, Cotuit, Massachusetts", prepared for Ms. Julie Hutchenson of the Department of Environmental Protection (DEP) dated March 9, 1994. According to 1 Mr. Perry the UST had been out of service since at least 1988. Upon removal, the feed line into the UST was found to be leaking, and had contaminated soil beneath it. 3.2 Prior Immediate Response Actions On December 23, 1993 MES excavated to a depth of approximately 12 feet and removed 1 28 cubic yards of contaminated soil. Soil samples were collected from the excavation sidewalls at 10 feet below grade and at the excavation bottom, and analyzed for Total Petroleum Hydrocarbons (TPH). Two of the sidewall samples had TPH levels of 820 and 1 3,200 parts per million (ppm) respectively, and the bottom sample had a TPH of.4,500 ppm. The excavation was discontinued due to the threat of structural damage to the ' foundation and backfilled with clean fill. MES submitted the IRA Plan to complete the remedial measure by capping the area with a clay/polyethylene closure seal. 1 3.3 Description of AET's Immediate Response Action 1 3.3.1 Assessment ' ' On March 20, 1995 AET personnel collected soil and groundwater samples at the site in the area of the release (Figure 5). The sample locations were labeled GP 1 Through GP4. Sample location GP I was in the center of the release area, GP3 was at the southern edge 1 of the release and GP2 was outside of the release area. Soil boring GP4 was an angle boring which allowed for sample collection beneath the building under the basement (Appendix A - Photograph 2). A more detailed description of the initial assessment is included in an AET report titled "Immediate Response Action Plan and Immediate Response Action Transmittal Form, Barnstable - RTN 4-10036, 5 Putnam Avenue, Cotuit, MA", prepared for Mr. Dan Crafton and dated April 26, 1995. The analytical results indicated TPH contamination of soil at various depths in the release area(Table 1). TPH was not detected below a depth of 16 feet BGS nor was-it detected in either groundwater sample, GPl-W or GP3-W. Laboratory reports are included in Appendix B. 3.3.2 Excavation ' Between April 24 and 25, 1995, soil in the area of the release was excavated by Enviro- Safe, Inc. of Sagamore Beach, Massachusetts. Clean fill, used to backfilled the previous excavation conducted by MES, was stockpiled on the property(Appendix A-Photograph ' 3). Structural shoring of the house allowed'for the excavation of contaminated soil along the foundation and beneath the house. The foundation wall in the vicinity of the excavation is inside the sill of the house by approximately two feet. This was a stone and mortar wall which collapsed as the work progressed. (Appendix A-Photograph 4). Soil between 7 and 15 feet BGS was excavated, loaded into a truck and taken to a recycling facility (Appendix A- Photograph 5). 3.4 Management of Remedial Wastes ' Approximately 31 tons were removed from the site and recycled into asphalt at the Bardon Trimount recycling facility in Stoughton, Massachusetts. The Bill of Lading and Certificate of Recycling are included in Appendix C. 3.5 Confirmatory Sampling ' Confirmatory soil samples were collected from the four sidewalls of the excavation at approximately 14 to 15 feet BGS, and from the bottom of the excavation at 15 feet BGS, ' and analyzed for TPH by Groundwater Analytical of Buzzards Bay, Massachusetts. The analytical results show that TPH was not detected in any of the four sidewall soil samples, while 56 ppm TPH was detected in the bottom soil sample (Table 2). Laboratory reports are included in Appendix B. 3.6 Environmental Monitoring Plan ' The threat of vertical migration of 42 fuel oil to the water table is minimal. Therefore, further environmental monitoring is not necessary. This opinion is based on the following: ' a) the low levels of TPH in confirmatory soil samples taken from the four sidewalls and the bottom of the excavation (Table 2), ' b) the great distance between the bottom of the excavation and groundwater (approximately 30 feet), c) groundwater contamination was not detected in groundwater samples collected during ' AET assessment work described above (Table 1), and d) the low levels of TPH in soil samples collected during AET assessment work. ' -4- 1 o i Atlantic 4.0 SUPPORT OF RESPONSE ACTION OUTCOME 1 4.1 Uncontrolled Sources 1 In December of 1994 MES removed the UST and contaminated soil down to 12 feet eliminating them as uncontrolled sources of Oil and/or Hazardous Material (OHM). 1 Further excavation of contaminated soil under AET supervision has eliminated most of the remaining contaminated soil as an uncontrolled source of OHM. 1 4.2 Achieving Background Levels Background TPH levels across the site, outside of the Response Action Outcome Area, 1 are assumed to be below detection limits based on the absence of TPH in composite soil samples collected at sample location GP2 which is outside of the release area, and the fact that the use of the site has been residential throughout its history. Confirmatory soil 1 samples from the four sidewalls of the excavation indicate that TPH levels in the Response Action Outcome Area, in soils above 15 feet BGS, have been remediated to background levels (Table 2). This has been achieved by the complete excavation and removal of 1 contaminated soil above 15 feet, and its replacement with clean fill. 1 TPH levels in the Response Action Outcome Area in soils below the bottom of the excavation are well below the Cleanup Standard for S-1 soils of 500 ppm. TPH levels between 15 feet BGS (the bottom of the excavation) and 16 feet BGS (below which no 1 TPH was detected in soil samples) are in the area of 50 ppm. This is based on analytical results of soil sampling on March 20, 1995, that show a TPH level of 50 ppm in a soil sample composite from the interval 13 to 16 feet BGS (Table 1), and a confirmatory soil sample collected at the bottom of the excavation that show_a TPH level of 56 ppm (Table 1 2). These levels, over time, will be reduced to background by natural degradation processes. 1 It was determined on-site that achieving background was infeasible based on economic factors. Excavating the remaining contaminated soil would have jeopardized the building. 1 At 15 feet, the basement floor showed signs of undermining which would have threatened the support structures that were set up in the basement to hold up the building. Structural damage to the house would have resulted in a dramatic increase in the cost of the project. 1 4.3 AchievingLevel of No Significant Risk g 1 Analytical results of confirmatory soil samples taken from the sidewalls and bottom of the excavation, indicate all fuel oil-contaminated soils with a TPH level above the Method-1 Cleanup Standard for S-1 Soils (500 ppm) have been removed from the Response Action Outcome Area. Some contaminated soil.remains between the interval 15 to 16 feet BGS. 1 -5- 1 Atlantic This soil, however, is deep enough to render it inaccessible to human receptors for all current and reasonably foreseeable future uses of the property. The only environmental receptor near the release area is groundwater. Groundwater ' samples from two monitoring locations in and near the release area indicate that groundwater has not been impacted by the release (Table 1). Since groundwater is approximately 45 feet BGS, it is not threatened by the remaining low levels of TPH in the 1 soil. Further, with the low levels of TPH, and the distance between the public supply well and the release area, it is our opinion that the public supply well is not threatened by the release. 1 i 1 1 . . 1 1 1 _ I _6 1 s o 1 Atlantic . 5.0 SUMMARY ' Based on the fact that all contaminated,soil with a TPH level greater than the Method-1 Cleanup Standard of 500 ppm has been excavated and removed from the release area, and ' that no human or environmental receptors are threatened by the remaining contaminated soil, we believe that a condition of No Significant Risk exists and a Permanent Solution has been achieved within the Response Action Outcome Area. The work included installing gauges at five locations along the river, both upstream and downstream of the pumping well, weekly ' recording of river height from the gauges and of groundwater levels from existing monitoring wells around the pumping ' well. Analysis of the data when the pumping well was on and when it was off, to establish the relationship between groundwater extraction and river height. ' Monitoring the round and surface water response to a 9 P stormwater runoff system installed in front of a saltwater public beach. Work included installation and weekly sampling of groundwater monitoring wells and stormwater runoff leaching chambers. • Review of percolation standards and loading allowances of state sewage disposal regulations. The work was particularly ' concerned with the adequacy of current standards when applied to the fast percolating soils of Cape Cod.
