NOTE FOR THE FILE
The Health Department received a complaint from James
Hughes, a tenant of 558 Craigville Beach Road, who claims
the Disposal Works Installer, Rodger Roberts, installed a
water supply line through the holes of a leaching galley
after the Health Inspector and Designing Engineer had
already left.
Several days later, on March 5, 1992, Thomas McKean
telephoned the Designing Engineer, Stephen Wilson, and set
up a meeting to be held that day at 2 :00 P.M..
At 2:06 P.M. , Stephen Wilson arrived. The following persons
also attended the meeting: Health Inspectors Jerome
Dunning, Edward Barry, Donna Miorandi and the Director,
Thomas McKean. Both Stephen Wilson and the Health Inspector
Edward Barry stated they did not observe any water line
pipes during their inspections of the system. Also, the
system was redesigned to ensure the septic system would not
be installed near the water supply pipes.
Thomas McKean also questioned the stone measurement
discrepancies. Health Inspector Edward Barry stated he only
observed 30" of stone around the system. Stephen Wilson, who
designed the system for 48" of stone, stated he felt the
amount of stone installed was sufficient. The system was
overdesigned on the plan anyhow.
On March 5, 1992, at approximately 10:30 P.M. , Thomas McKean
spoke to Rodger Roberts. Mr. Roberts stated he did not place
any water pipe through the leaching galley of the septic
system. He further stated he placed the water pipe at the
end of the system on top of stone.

Members of the Board:
This letter is to inform you the above noted septic system was
constructed in substantial accordance with the approved plans. The
following field changes were made by the contractor and approved by
the designing engineer.
1. The system was moved farther away from Craigville Beach Rd.
to avoid an existing water service.
2 . No riser was placed on the distribution box.
3 . There is a constant pipe slope from the distribution box to
the leaching galleys.

contact this office.

TIME: 3;30 pm April 10, 1991
ENGINEER: Steve Wilson of Levy,Eldredge and Wagner Associates
889 ',^Jest Main St. ,Centerville,MA.
I inspected the installed system,2000 gal septic tank, 'D''box
and four(4) galleys.I noted the following: ����,,//���
1 . System not located. as shown on plan ll`�
2. Line out of 'D' box not level for two feet as shown on plan:^
3. No riser on 'D' box as shown on plan:
4- Stone around galleys was measued as 30 inches on the ends,
left side as 36 inches and right side as 32 inches.The plan
indicated stone as being 48 inches all around the galleys.
As .I was inspecting the system,Steve Wilson arrived at the
site.I conferred with Steve about the above discrepenc*es and the
response was:
1 . We had to move the system because of the water line.
2. Havingthe line out of the 'D' box level is not necessary.
3. Having the riser for the 'D' box is not necessary.
4. The inches -of stone arouddthe galleys is OK.

This was not visible when I arrived because the galleys were
already in place.
