PRELIMINARY SUBDIVISION PLAN - SUBMISSION CHECKLIST
This form must be completed by the plan preparer,signed and dated and returned with a completed copy of Form B
attached and the appropriate filing fee. No application shall be considered complete until all the required materials
have been submitted. For the complete submission requirements,see Section 3-5 of the Rules and
Regulations.
PLEASE CHECK EACH ITEM:
Two copies of the completed Form B and Form B Checklist.
