Box 1224, Hyannis, MA 02601 Dear Board Members: Enclosed please find six (4) copies of Plot Plans dated October 22, 1999, an Application for Disposal System Construction Permit with fee ($100.00) , Direct Abutter's List, Locus Map, and variance application fee($65.00) for the above referenced project. This project proposes new construction of a three bedroom dwelling, a private well, and an on-site subsurface sewage disposal system(SSDS) on a 23,810 square foot lot(Map 124 Parcel 36) on Mountain Ash Road in Marstons Mills. The "SSDS" has been designed for a three bedroom house per 310 CMR 15.240(5). The on-site soil absorption system (SAS) is set back over 122' from the on-site proposed well. The proposed on-site SAS is set back over 117' from the abutter's well to the north. The proposed on-site SAS is setback over 135' from the abutter's well to the south. The proposed system is in total compliance with Title V (all private wells>100 feet) except for nitrogen loading under 310 CMR 15.214, but does require a few variances from the Town of Barnstable Local Regulations to properly site a 3-bedroom system. The items under Local Authority and State Regulations that the applicant requests to vary from are as follows: Town of Barnstable Well Regulations Part XII Section 2.00 (a) Existing Private Wells less than 150 feet from proposed soil absorption system. Proposed Private Well less than 150 feet from proposed soil absorption system. (b) Proposed 3 bedroom house on less than 30,000 square feet of land. (c) Proposed Private Well and SSDS on less than 40,000 square feet of land.
